Many novel therapeutic options for depression exist that are either not mentioned in clinical guidelines or recommended only for use in highly specialist services. The challenge faced by clinicians is when it might be appropriate to consider such ‘non-standard’ interventions. This analysis proposes a framework to aid this decision.
Declaration of interestIn the past 3 years R.H.M.W. has received support for research, expenses to attend conferences and fees for lecturing and consultancy work (including attending advisory boards) from various pharmaceutical companies including Astra Zeneca, Cyberonics, Eli Lilly, Janssen, LivaNova, Lundbeck, MyTomorrows, Otsuka, Pfizer, Roche, Servier, SPIMACO and Sunovion. D.M.B.C. has received fees from LivaNova for attending an advisory board. In the past 3 years A.J.C. has received fees for lecturing from Astra Zeneca and Lundbeck; fees for consulting from LivaNova, Janssen and Allergan; and research grant support from Lundbeck.
In the past 3 years A.C. has received fees for lecturing from pharmaceutical companies namely Lundbeck and Sunovion. In the past 3 years A.L.M. has received support for attending seminars and fees for consultancy work (including advisory board) from Medtronic Inc and LivaNova. R.M. holds joint research grants with a number of digital companies that investigate devices for depression including Alpha-stim, Big White Wall, P1vital, Intel, Johnson and Johnson and Lundbeck through his mindTech and CLAHRC EM roles. M.S. is an associate at Blueriver Consulting providing intelligence to NHS organisations, pharmaceutical and devices companies. He has received honoraria for presentations and advisory boards with Lundbeck, Eli Lilly, URGO, AstraZeneca, Phillips and Sanofi and holds shares in Johnson and Johnson. In the past 3 years P.R.A.S. has received support for research, expenses to attend conferences and fees for lecturing and consultancy work (including attending an advisory board) from life sciences companies including Corcept Therapeutics, Indivior and LivaNova. In the past 3 years P.S.T. has received consultancy fees as an advisory board member from the following companies: Galen Limited, Sunovion Pharmaceuticals Europe Ltd, myTomorrows and LivaNova. A.H.Y. has undertaken paid lectures and advisory boards for all major pharmaceutical companies with drugs used in affective and related disorders and LivaNova. Summary
Most contemporary philosophers would accept that the intelligence is provided by the material brains, and thus would be disinclined to challenge the possibility of artificially intelligent devices on the ground of their materiality. The questions and problems about artificial intelligence (AI) that remain can be divided into those that are largely independent of particular approaches to AI, and those that are prompted by more specific ideas about artificially realizable cognitive architectures. The chapter shows that questions about representations and their use play an important role in many challenges to AI. Connectionist devices are characterized by their rule for finding a set of connection weights that will yield patterns on their output units that are appropriate to each pattern on their input units. Dynamical systems theory (DST) views cognition as depending on a continuous interaction of a cognitive agent with its surroundings.

We present a route for direct growth of boron nitride via a polyborazylene to h-BN conversion process. This two-step growth process ultimately leads to a >25x reduction in the root-mean-square surface roughness of h-BN films when compared to a high temperature growth on Al2O3(0001) and Si(111) substrates. Additionally, the stoichiometry is shown to be highly dependent on the initial polyborazylene deposition temperature. Importantly, chemical vapor deposition (CVD) graphene transferred to direct-grown boron nitride films on Al2O3 at 400 °C results in a >1.5x and >2.5x improvement in mobility compared to CVD graphene transferred to Al2O3 and SiO2 substrates, respectively, which is attributed to the combined reduction of remote charged impurity scattering and surface roughness scattering. Simulation of mobility versus carrier concentration confirms the importance of limiting the introduction of charged impurities in the h-BN film and highlights the importance of these results in producing optimized h-BN substrates for high performance graphene and TMD devices.

This review brings together research findings on cervical relaxation in the ewe and its pharmacological stimulation for enhancement of the penetration needed for transcervical insemination and embryo transfer. On the basis that the success of artificial insemination is the percentage of ewes lambing, a review is made of recent research aimed at understanding and minimising the sub-lethal effects of freezing and thawing on the viability of spermatozoa, their membrane integrity and their ability to migrate through cervical mucus, as these characteristics have a major influence on fertility, particularly when semen is deposited, artificially, in the os cervix. Milestones of achievement are given for transcervical intrauterine insemination, embryo recovery and transfer and the birth of lambs of pre-determined sex, firstly following intracytoplasmic sperm injection, then laparoscopic intrauterine insemination using highly diluted flow-cytometrically sorted fresh semen and subsequently by os cervix insemination using sexed semen that had been frozen and thawed. Diversity of research endeavour (applied, cellular, molecular), research discipline (anatomy, histology, immunology, endocrinology) and research focus (cell, tissue, organ, whole animal) is embraced within the review as each has significant contributions to make in advancing recent scientific findings from the laboratory into robust on-farm transcervical insemination and embryo transfer techniques.

Large social aggregations are among the most highly organized events associated with mobile hunter-gatherers. The Bull Brook Paleoindian site in Ipswich, Massachusetts provides the strongest case for large-scale Paleoindian aggregation in North America, with 36 discrete concentrations of artifacts arranged in a large circle. Avocational archaeologists who salvaged the site in the 1950s interpreted it as a single occupation. Professionals first rejected and then revived this hypothesis, but the site remained insufficiently analyzed to evaluate. New research supports the single occupation hypothesis with a fully reconstructed site plan and the first complete analysis of artifact distributions. Clear spatial structure of activities within the ring-shaped site plan provides a window on social contexts that are also visible in smaller Paleoindian settlements.
Randomised controlled trial of CD–ROM-based cognitive–behavioural self-care for bulimia nervosa

Background
Cognitive–behavioural self-care is advocated as a first step in the treatment of bulimia nervosa.
AimsTo examine the effectiveness of a CD–ROM-based cognitive–behavioural intervention in bulimia nervosa and eating disorder not otherwise specified (NOS) (bulimic type) in a routine setting.
MethodNinety-seven people with bulimia nervosa or eating disorder NOS were randomised to either CD–ROM without support for 3 months followed by a flexible number of therapist sessions or to a 3-month waiting list followed by 15 sessions of therapist cognitive–behavioural therapy (CBT) (ISRCTN51564819). Clinical symptoms were assessed at pre-treatment 3 months and 7 months.
ResultsOnly two-thirds of participants started treatment. Although there were significant group × time interactions for bingeing and vomiting, favouring the CD–ROM group at 3 months and the waiting-list group at 7 months, post hoc group comparisons at 3 and 7 months found no significant differences for bingeing or vomiting. CD–ROM-based delivery of this intervention, without support from a clinician, may not be the best way of exploiting its benefits.

Summary
In this chapter I examine the transparency argument for representationalism. Its central idea is this.
(T1) When we attempt to focus on (alleged) qualities of our experiences, we find that we cannot do so. What we find instead is only properties of the things that are represented in our experiences. The experiences themselves are transparent; we look right through them, so to speak, to the qualities of the things represented in them.
If we accept this idea at face value, then
(T2) Experiences must be either (a) things we are not directly aware of at all or (b) things we are directly aware of without being aware of any of their qualities.
(T3) Awareness of experiences without awareness of any of their qualities is both peculiar in itself and useless in accounting for phenomenal consciousness. Thus, we ought to reject alternative (b).
(T4) Alternative (a) is acceptable. Experiences can be regarded as brain events that represent things as being colored, flavored, and so on. In having them we are aware of the qualities of things, not of experiences. We are not directly aware of experiences; rather, we infer them as events that represent to us the qualities of things.
Therefore,
(T5) Experiences are transparent representers of qualities of things.
Against this argument, qualitative event realists will say that the properties that ordinary things actually have are P-properties, and nothing is represented to us in ordinary experience as having those.

Summary
The main outlines of QER can be set out in the following response to our Basic Question.
Besides the reflectance profiles, light waves, retinal changes, neural events, and behavioral responses reviewed in Chapter 1, normal cases in which a person sees a red apple will involve the apple's looking red to that person. An apple's looking red requires a distinctive kind of conscious occurrence – a red experience – that is something in its own right, and not reducible to behavioral responses, or dispositions to behavioral responses, or brain state bases of such dispositions. Experiences are caused by neural events, but are not identical to or reducible to them or to any other material events. Experiences are constituted by phenomenal qualities; indeed, phenomenal qualities occur only in experiences and are the essence of consciousness in the most fundamental sense of that term.
An initial motivation for realism about experiences should already be evident and can be summarized as follows. Something is happening in afterimage cases, and that something is very similar to part of what goes on in seeing. “Experiences”, and “ways in which things look (or, appear)”, are more or less well-established ways of talking about this kind of something. Differences among experiences cannot consist simply in how experiences are related to different kinds of perceived things, for there are various kinds of experiences in afterimaging, illusions, and dreams but no perceived things to which they stand in the right relations.

Summary
Many forms of materialism can be regarded as accepting a substantial portion of what qualitative event realists say about phenomenal consciousness. In rejecting QER, they attempt to give an alternative account of something that is recognized on both sides. One version of materialism, however, rejects QER in a much more radical way: it regards the qualitative events of QER as illusions. What is to be accounted for is not those events, for there are none, but only the language of seeming (looking, appearing, etc.), and the tendency of philosophers to postulate seemings (appearances, phenomenal occurrences, qualia, etc.) as presuppositions of such language. This skeptical version of materialism is closely related to the view I called “Minimalism” in Chapter 1, and its answer to the question of how color comes into experience can be given as follows.
(S) When Eve sees a red apple in daylight, the apple's P-red surface reflects light with some set of wavelength intensities. This light impinges on Eve's retina, where it causes changes that initiate a number of processes in the brain. Some of these brain processes may lead to behavior appropriate to red things, including reports that something red is seen. Under other conditions, some of these processes may interact with other processes and result only in “proto-judgments”, which may (or may not) lead to statements to the effect that something seems to be red but isn't. Under still other conditions, some of these processes may be substantially inhibited by other processes and thus not lead to much of anything.
