“It Is A Carrot-Stick Model”: A Qualitative Study of Rural-Serving Clinician and Rural-Residing Veteran Perceptions of Requirements to Quit Smoking prior to Elective Surgery

Introduction. Some medical centers and surgeons require patients to stop smoking cigarettes prior to elective orthopaedic surgeries in an effort to decrease surgical complications. Given higher rates of smoking among rural individuals, rural patients may be disproportionately impacted by these requirements. We assessed the perceptions and experiences of rural-residing Veterans and clinicians related to this requirement. Methods. We conducted qualitative semistructured one-on-one interviews of 26 rural-residing veterans, 10 VA orthopaedic surgery staff (from two Veterans Integrated Services Networks), 24 PCPs who serve rural veterans (14 VA; 10 non-VA), and 4 VA pharmacists. Using the knowledge, attitudes, and behavior framework, we performed conventional content analysis. Results. We found three primary themes across respondents: (1) knowledge of and the evidence base for the requirement varied widely; (2) strong personal attitudes toward the requirement; and (3) implementation and possible implications of this requirement. All surgery staff reported knowledge of requirements at their institution. VA PCPs reported knowledge of requirements but typically could not recall specifics. Most patients were unaware. The majority of respondents felt this requirement could increase motivation to quit smoking. Some PCPs felt a more thorough explanation of smoking-related complications would result in increased quit attempts. About half of all patients reported belief that the requirement was reasonable regardless of initial awareness. Respondents expressed little concern that the requirement might increase rural-urban disparities. Most PCPs and patients felt that there should be exceptions for allowing surgery, while surgical staff disagreed. Discussion. Most respondents thought elective surgery was a good motivator to quit smoking; but patients, PCPs, and surgical staff differed on whether there should be exceptions to the requirement that patients quit preoperatively. Future efforts to augment perioperative smoking cessation may benefit from improving coordination across services and educating patients more about the benefits of quitting.
Changes in diet quality and body weight over 10 years: the Multiethnic Cohort Study

High-quality diets have been found to be beneficial in preventing long-term weight gain. However, concurrent changes in diet quality and body weight over time have rarely been reported. We examined the association between 10-year changes in diet quality and body weight in the Multiethnic Cohort Study. Analyses included 53 977 African Americans, Native Hawaiians, Japanese Americans, Latinos and Whites, who completed both baseline (1993–1996, 45–69 years) and 10-year follow-up (2003–2008) surveys including a FFQ and had no history of heart disease or cancer. Using multivariable regression, weight changes were regressed on changes in four diet quality indexes, Healthy Eating Index-2015, Alternative Healthy Eating Index-2010, alternate Mediterranean Diet and Dietary Approaches to Stop Hypertension scores. Mean weight change over 10 years was 1·2 (sd 6·8) kg in men and 1·5 (sd 7·2) kg in women. Compared with stable diet quality (< 0·5 sd change), the greatest increase (≥ 1 sd increase) in the diet scores was associated with less weight gain (by 0·55–1·17 kg in men and 0·62–1·31 kg in women). Smaller weight gain with improvement in diet quality was found in most subgroups by race/ethnicity, baseline age and baseline BMI. The inverse association was stronger in younger age and higher BMI groups. Ten-year improvement in diet quality was associated with a smaller weight gain, which varied by race/ethnicity and baseline age and BMI. Our findings suggest that maintaining a high-quality diet and improving diet quality over time may prevent excessive weight gain.
Combined anthocyanins and bromelain supplement improves endothelial function and skeletal muscle oxygenation status in adults: a double-blind placebo-controlled randomised crossover clinical trial

Anthocyanins and bromelain have gained significant attention due to their antioxidative and anti-inflammatory properties. Both have been shown to improve endothelial function, blood pressure (BP) and oxygen utility capacity in humans; however, the combination of these two and the impacts on endothelial function, BP, total antioxidant capacity (TAC) and oxygen utility capacity have not been previously investigated. The purpose of this study was to investigate the impacts of a combined anthocyanins and bromelain supplement (BE) on endothelial function, BP, TAC, oxygen utility capacity and fatigability in healthy adults. Healthy adults (n 18, age 24 (sd 4) years) received BE or placebo in a randomised crossover design. Brachial artery flow-mediated dilation (FMD), BP, TAC, resting heart rate, oxygen utility capacity and fatigability were measured pre- and post-BE and placebo intake. The BE group showed significantly increased FMD, reduced systolic BP and improved oxygen utility capacity compared with the placebo group (P < 0·05). Tissue saturation and oxygenated Hb significantly increased following BE intake, while deoxygenated Hb significantly decreased (P < 0·05) during exercise. Additionally, TAC was significantly increased following BE intake (P < 0·05). There were no significant differences for resting heart rate, diastolic BP or fatigability index. These results suggest that BE intake is an effective nutritional therapy for improving endothelial function, BP, TAC and oxygen utility capacity, which may be beneficial to support vascular health in humans.
Variation in seedling root traits in wild barley (Hordeum vulgare L. ssp. spontaneum) germplasm

Improved root architecture of cultivated barley can improve crop performance in drought-prone areas. In this study, seedlings of 315 wild barley (Hordeum vulgare subsp. spontaneum) accessions from the Wild Barley Diversity Collection (WBDC) were grown under hydroponic conditions for 8 d after germination and then root characteristics were analysed. Significant differences were observed among the accessions with regard to seminal root number (SRN), root length (RL), specific root length (SRL), root fresh weight and root dry weight (RDW). Principal component analysis explained about 81% of the total variation for ten traits. Principal component (PC) 1, PC2 and PC3 explained about 38, 30 and 13% of the total variation among the accessions. The two most prominent contributors in each PC were RL and SRL, RDW and SRN, and the longitude and latitude of the collection sites, respectively. Accessions WBDC266, WBDC302, WBDC286 and WBDC011 had the longest RL and the highest RDW, specific dry root weight and SRL, respectively. These accessions may be useful genetic resources for the improvement of these root traits in cultivated barley.

Polycrystalline silicon thin films with a thickness of 0.5-2μm were grown on 25nm thick Ni prelayers by d.c. magnetron sputtering from a Si target. In contrast to the conventional sputtering, the use of a thin Ni film allows the production of high crystallinity silicon at a temperature of 475°C and higher. The Ni disilicide grains formed at the Ni - growing Si film interface provide sufficient sites for the epitaxial growth of Si. The Si films with resistivity of 102-103Ω-cm possess a carrier lifetime of up to 11μs and a diffusion length of up to 3.4 μm, which makes them applicable to various microelectronic devices. As an example, Schottky diodes fabricated on 0.5μm thick Si films exhibit a forward-to-reverse current ratio of 107. The technique is easily implemented on a variety of substrates.

The effect of ion beams on the formation of Si nanoclusters from a-SiOx films and their luminescence properties is investigated. a-SiOx films with Si content ranging from 33 to 50 at. % were deposited by Electron Cyclotron Resonance Plasma Enhanced Chemical Vapor Deposition (ECR-PECVD) of SiH4 and O2. Prior to anneal, some samples were implanted with 380 keV Si to a dose ranging from 5.7 × 1014 cm−2 to 5.7 × 1016 cm−2. All films were rapid thermal annealed under flowing Ar environment, and hydrogenated after anneals to passivate defects and to enhance the luminescence of Si nanoclusters. For films with Si content less than 40 at. %, ion beam slightly reduces the photoluminescence (PL) intensity and induces a slight blueshift of the luminescence. For films with Si content greater than 40 at. %, ion beam greatly increases the PL intensity. Based on the effect of the ion beams dose and the ion specie, we propose that ion beams damage greatly promotes nucleation of small Si clusters from the a-SiOx matrix.

Microwave chemical vapor deposition (MCVD) is utilized to deposit nanocrystalline silicon (nc-si) thin films onto a variety of substrates for application to thin film transistors (TFT's) and solar cells. It is especially important to gain reproducible control of the processing. Thus, an in-situ mass spectrometer (MS) records the plasma conditions with variation of process conditions such as gas selection, pressures, partial pressures, and substrate temperature. These data are correlated with electrical and optical properties of the films. Raman spectra show a FWHM of 11/cm with position at 522/cm as desired for crystalline Si. Typical film thickness is 100nm with grain size of 20-30 nm, using standard deposition, and 50-80 nm when the substrate is intensely optically illuminated during deposition, called photon assist (PA). Hydrogen dilution serves to increase the crystallinity of the films. The ratio of photo-to dark conductivity exceeds 10+5 with dark conductivity as low as 1.5 × 10-10 S/cm. Thin film transistors have been fabricated with Ion/Ioff of 10+7. Hetrojunction solar cells were fabricated using amorphous Si/ nc-Si/ crystlline Si giving a conversion efficiency of above 10.5%, without an antireflection coating. The use of MS in device design will be emphasized.

ECR silane plasmas for the deposition of a-Si:H and μc-Si films were investigated by in-situ mass spectroscopy (MS) using a quadrupole residual gas analyzer. The results showed that the intensities of ionic and neutral species (H, H2, He, Ar, Si and SiHx) in the 2 % SiH4/He plasma are strongly dependent on the deposition conditions such as chamber pressure, input power and hydrogen dilution. In all cases, the prevalence of Si ions was observed over SiH, SiH2 and SiH3 ions, suggesting a high decomposition rate of the silane in the plasma. In particular, the population of atomic hydrogen in the plasma seems to play a key role in the properties of both a-Si:H and μc-Si films. For example, the increased intensity of atomic hydrogen, compared to that of molecular hydrogen, resulted in the better quality a-Si:H film, showing a higher photo and dark conductivity ratio of ~105. The intensity of the hydrogen species was especially sensitive to the chamber pressure. The correlation between MS spectra and film properties is presented.

Amorphous (a-Si) and microcrystalline silicon (μc-Si) are widely used in photovoltaics and thin film transistors. These products suffer from instability and less than desired electrical properties. We have utilized photon-assisted electron cyclotron resonance chemical vapor deposition (PA-ECRCVD) resulting in great improvement in both areas. For example, PA-ECRCVD compared with conventional ECR-CVD gives carrier lifetime of 1.35 4ts compared to 0.17 μs, and photovoltaic solar cell efficiency of 10 % compared to 5.9 %. Moreover, the PAECRCVD cell only degraded to 9.8 % compared to the ECR-CVD cell degradation to 5.5 %, under long term exposure to tungsten lamp illumination. In addition, PA-ECRCVD gives much enhanced crystallinity in μc-Si as revealed by atomic force microscopy and Raman spectroscopy.
