Summary

What follows is a complete list of all those interviewed, along with copies of the different informed consent forms used. The variation is due to my having been at different institutions over the course of this project, and due to different legal guidelines operating at different times. It was not always possible to provide all participants with the forms, and in some cases, the interviewees were uninterested in the informed consent requirements. In all cases, however, a verbal explanation of the terms of the informed consent agreement was made.
