South Africa has embarked on major health policy reform to deliver universal health coverage through the establishment of National Health Insurance (NHI). The aim is to improve access, remove financial barriers to care, and enhance care quality. Health technology assessment (HTA) is explicitly identified in the proposed NHI legislation and will have a prominent role in informing decisions about adoption and access to health interventions and technologies. The specific arrangements and approach to HTA in support of this legislation are yet to be determined. Although there is currently no formal national HTA institution in South Africa, there are several processes in both the public and private healthcare sectors that use elements of HTA to varying extents to inform access and resource allocation decisions. Institutions performing HTAs or related activities in South Africa include the National and Provincial Departments of Health, National Treasury, National Health Laboratory Service, Council for Medical Schemes, medical scheme administrators, managed care organizations, academic or research institutions, clinical societies and associations, pharmaceutical and devices companies, private consultancies, and private sector hospital groups. Existing fragmented HTA processes should coordinate and conform to a standardized, fit-for-purpose process and structure that can usefully inform priority setting under NHI and for other decision makers. This transformation will require comprehensive and inclusive planning with dedicated funding and regulation, and provision of strong oversight mechanisms and leadership.

Gravitational waves from coalescing neutron stars encode information about nuclear matter at extreme densities, inaccessible by laboratory experiments. The late inspiral is influenced by the presence of tides, which depend on the neutron star equation of state. Neutron star mergers are expected to often produce rapidly rotating remnant neutron stars that emit gravitational waves. These will provide clues to the extremely hot post-merger environment. This signature of nuclear matter in gravitational waves contains most information in the 2–4 kHz frequency band, which is outside of the most sensitive band of current detectors. We present the design concept and science case for a Neutron Star Extreme Matter Observatory (NEMO): a gravitational-wave interferometer optimised to study nuclear physics with merging neutron stars. The concept uses high-circulating laser power, quantum squeezing, and a detector topology specifically designed to achieve the high-frequency sensitivity necessary to probe nuclear matter using gravitational waves. Above 1 kHz, the proposed strain sensitivity is comparable to full third-generation detectors at a fraction of the cost. Such sensitivity changes expected event rates for detection of post-merger remnants from approximately one per few decades with two A+ detectors to a few per year and potentially allow for the first gravitational-wave observations of supernovae, isolated neutron stars, and other exotica.
Longitudinal relationship between expressed emotion and cannabis misuse in young people with first-episode psychosis

Carers’ expressed emotion (EE) and patients’ cannabis misuse are two of the most robust predictors of psychotic relapse. We aimed to examine the temporal relationship between EE and cannabis misuse. Sixty-three key carers of young people with first-episode psychosis (FEP) were assessed at baseline and 7-month follow-up. EE was measured in carers using the Family Questionnaire (FQ) and cannabis misuse in patients using the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST). Correlational and hierarchical logistic regression analyses were conducted to examine the temporal relationship between EE components (i.e. criticism and emotional over involvement) and cannabis misuse. Carers’ criticism at baseline significantly predicted cannabis misuse according to the ASSIST at 7-month follow-up. The association remained significant after controlling for baseline symptom severity and social functioning (B = 0.15, P = .02). Conversely, baseline cannabis misuse was not associated with carers’ criticism at 7-month follow-up. Patients in families with high criticism showed a tendency to increase cannabis misuse over time whereas the opposite trend was observed in those with carers with low criticism. A family environment characterized by high criticism may become a key risk factor for worsening cannabis misuse over time in young people with FEP. Further studies should investigate the potential mechanisms (e.g., patient's anxiety or perceived stress) through which criticism increases cannabis misuse in FEP.

A growing interest in constellations of small satellites has recently emerged due to the increasing capability of these platforms and their reduced time and cost of development. However, in the absence of dedicated launch services for these systems, alternative methods for the deployment of these constellations must be considered which can take advantage of the availability of secondary-payload launch opportunities. Furthermore, a means of exploring the effects and tradeoffs in corresponding system architectures is required. This paper presents a methodology to integrate the deployment of constellations of small satellites into the wider design process for these systems. Using a method of design-space exploration, enhanced understanding of the tradespace is supported , whilst identification of system designs for development is enabled by the application of an optimisation process. To demonstrate the method, a simplified analysis framework and a multiobjective genetic algorithm are implemented for three mission case-studies with differing application. The first two cases, modelled on existing constellations, indicate the benefits of design-space exploration, and possible savings which could be made in cost, system mass, or deployment time. The third case, based on a proposed Earth observation nanosatellite constellation, focuses on deployment following launch using a secondary-payload opportunity and demonstrates the breadth of feasible solutions which may not be considered if only point-designs are generated by a priori analysis. These results indicate that the presented method can support the development of future constellations of small satellites by improving the knowledge of different deployment strategies available during the early design phases and through enhanced exploration and identification of promising design alternatives.

The discovery of the first electromagnetic counterpart to a gravitational wave signal has generated follow-up observations by over 50 facilities world-wide, ushering in the new era of multi-messenger astronomy. In this paper, we present follow-up observations of the gravitational wave event GW170817 and its electromagnetic counterpart SSS17a/DLT17ck (IAU label AT2017gfo) by 14 Australian telescopes and partner observatories as part of Australian-based and Australian-led research programs. We report early- to late-time multi-wavelength observations, including optical imaging and spectroscopy, mid-infrared imaging, radio imaging, and searches for fast radio bursts. Our optical spectra reveal that the transient source emission cooled from approximately 6 400 K to 2 100 K over a 7-d period and produced no significant optical emission lines. The spectral profiles, cooling rate, and photometric light curves are consistent with the expected outburst and subsequent processes of a binary neutron star merger. Star formation in the host galaxy probably ceased at least a Gyr ago, although there is evidence for a galaxy merger. Binary pulsars with short (100 Myr) decay times are therefore unlikely progenitors, but pulsars like PSR B1534+12 with its 2.7 Gyr coalescence time could produce such a merger. The displacement (~2.2 kpc) of the binary star system from the centre of the main galaxy is not unusual for stars in the host galaxy or stars originating in the merging galaxy, and therefore any constraints on the kick velocity imparted to the progenitor are poor.

Nucella lapillus and Littorina littorea afford examples of turbinately coiled shells with no space between the columella and the inner face of the whorls. Three constants are sufficient to determine the form of such shells: λ, the ratio between diameters of successive whorls; β, the semi-apical angle and ρ, the ratio of aperture length and breadth in the apertural plane passing through the axis. The variation of these three constants with size was examined and shown to be relatively small and insignificant in Nucella, but in Littorina it was significant.
For Nucella and Littorina these shell characteristics were used to relate for the nth whorl, the length along the shell spiral (ln) to the shell height (Hn), the latter being the measure usually employed in growth studies. This relation was derived mathematically and confirmed empirically as
where α is the angle of the logarithmic spiral given by α = tan−1 2π sin β/loge λ. The increased resolution of the micro-growth band increments, which are measured along the shell spiral was directly related to the ln:Hn ratios.
Since the formula for the ln/Hn ratio (= shell conversion factor) includes λ, β and ρ, the three constants necessary to determine shell form in turbinately coiled shells of the type described, the shell conversion factor may be a better index of shell shape than indices based on a single constant. In species where the shell grows isometrically this ratio will remain constant, whereas changes in shell shape with size will influence this ratio. Such changes in shell shape during growth can be quantified in terms of the shell constants and when included in calculating the shell conversion factor, will show the relative magnitude of shell shape changes brought about by growth. The shell conversion factor may also be used to study the effects of ecological variations on the geometry of the shell.
The conical shell of the limpet, Patella vulgata, is geometrically a much simpler system in which direct shell measurements were used to relate length along the direction of maximum growth to the antero-posterior diameter of aperture.

Whereas there is a wealth of literature on the feeding of herbivorous holo-plankton, notably copepods, feeding by herbivorous larvae of benthic animals has been somewhat neglected. This paper considers the functional feeding response of veliger larvae of Ostrea edulis L., Crassostrea gigas (Thunberg) and Mytilus edulis L. fed on several micro-algal species. It also provides some data on the influence of temperature on feeding, and assesses the energy needs of the larvae in relation to their potential ingestion rate and to the availability in the sea of micro-organisms on which they feed.

Tidal micro-growth bands were used to study the annual cycle of shell growth of individuals dated predominantly by the immersion mark technique, in three common intertidal gastropods Nucella lapillus (L.), Littorina littorea (L.) and Patella vulgata L. growing naturally in the Menai Straits. Growth in all three species approximated to the von Bertalanaffy type. All three species grew throughout the year, N. lapillus and L. littorea with early autumn and summer maxima, respectively, and P. vulgata with two maxima, one in late spring and the other in early autumn. P. vulgata alone were observed during exposure to a 6-day period of sub-zero air temperature in January 1982 when shell growth was totally interrupted. In Nucella and Littorina shell growth was found to be closely related to ambient temperature but in Patella there was a summer depression in growth. The file technique when combined with the immersion mark technique in date marking, was found to be inferior to the immersion mark technique on its own, since filing occasionally affected growth adversely.
From among the factors determining the seasonal changes in growth rate, temperature was of major importance. Seasonal reproduction may also have an influence, especially in P. vulgata where gametogenesis coincides with the depression in growth rate during summer months.
Alternative methods used in gastropod growth studies were compared. In the light of inadequacies associated with other methods of measuring growth, micro-growth band measurements in gastropods offers a new method with high resolution suitable both for short and long-term growth studies. It has potential also for ecological and physiological aspects of growth measurements.
Differential predictors of critical comments and emotional over-involvement in first-episode psychosis

Background
Little research has focused on delineating the specific predictors of emotional over-involvement (EOI) and critical comments (CC) in the early course of psychosis. The purpose of this study was to investigate the differential relationships of EOI and CC with relevant predictors in relatives of first-episode psychosis (FEP) patients.
MethodBaseline patient-related factors including psychotic symptoms, depression and duration of untreated psychosis (DUP) and carer attributes comprising CC, EOI, burden of care and carers' stress and depression were assessed in a cohort of 63 remitted FEP patients and their relatives. Carers were reassessed at 7 months follow-up.
ResultsBaseline analysis showed that EOI was more strongly correlated with family stress compared with CC, whereas CC yielded a stronger association with DUP than EOI. Carers' CC at follow-up was not significantly predicted by either baseline family stress, burden of care or patient-related variables. Conversely, baseline EOI predicted both family stress and burden of care at 7 months follow-up. Finally, family burden of care at follow-up was a function of baseline EOI and patients' depressive symptoms.
ConclusionsThis study provides preliminary support to the postulate that EOI and CC may be influenced by separate factors early in the course of psychosis and warrant future research and therapeutic interventions as separate constructs. Implications for family interventions in the early phase of psychosis and the prevention of CC and EOI are discussed.

We describe evidence that certain inductive phenomena are associated with IQ, that different inductive phenomena emerge at different ages, and that the effects of causal knowledge on induction are decreased under conditions of memory load. On the basis of this evidence we argue that there is more to inductive reasoning than semantic cognition.

Long-Term Mortality in Anorexia Nervosa: A 20–Year Follow-Up of the St George's and Aberdeen Cohorts

Two cohorts of anorexia nervosa patients were followed up for a mean of 20 years. All except 4% of each cohort was traced. The crude mortalities were: St George's, 4%; Aberdeen, 13%. The SMRs were: St George's, 136; Aberdeen, 471. If the untraced were assumed to be dead, crude mortalities were 7.6% and 15.9% respectively, and SMRs were 276 and 592 respectively. Causes of death were complications of the illness and suicide. Medical treatment may reduce early mortality, while comprehensive medical and psychotherapeutic treatment may reduce late mortality.

Patients with anorexia nervosa show a decrease in psychoneurotic symptomatology as measured by the Crown-Crisp Experiential Index (CCEI) as their clinical status improves. Those who remain ill continue to display a moderate degree of varied psychopathology 4 to 8 years after their presentation. Vomiters and those with anxiety on eating with others can be distinguished on the CCEI from those without such features. Lower anxiety, phobia, obsessional and somatic scores at presentation are predictive of good outcome in terms of weight and menstrual function 4 to 8 years later.
