Summary
Thematically, formally and structurally, Wallace’s writing concerned itself with the infinite, from the antinomies of set theory and the obese Bombardini in The Broom of the System to the featureless horizon of Peoria in The Pale King, by way of the title of Infinite Jest and the brief and not wholly successful exploration of Cantorian mathematics in Everything and More, the idea of the infinite was never far from any of Wallace’s writing. Moreover, the structures of the writing continually reinscribe this obsession with infinity, with none of the novels conforming to a traditional boundaried structure and the collections of short fiction troubling the very concept of order in their use of pagination and enumeration. This chapter illuminates the importance of infinity to Wallace’s writing by exploring its formal and thematic development through his career, demonstrating that infinity worked as a conceptual counterpoint to solipsism, both an existential threat and a source of profound hope for the disassociated subject of contemporary culture.

The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.

Objectives
Performing an extended Focused Assessment with Sonography in Trauma (eFAST) exam is common practice in the initial assessment of trauma patients. The objective of this study was to systematically review the published literature on diagnostic accuracy of all components of the eFAST exam.
MethodsWe searched Medline and Embase from inception through October 2018, for diagnostic studies examining the sensitivity and specificity of the eFAST exam. After removal of duplicates, 767 records remained for screening, of which 119 underwent full text review. Meta-DiSc™ software was used to create pooled sensitivities and specificities for included studies. Study quality was assessed using the Quality in Prognostic Studies (QUADAS-2) tool.
ResultsSeventy-five studies representing 24,350 patients satisfied our selection criteria. Studies were published between 1989 and 2017. Pooled sensitivities and specificities were calculated for the detection of pneumothorax (69% and 99% respectively), pericardial effusion (91% and 94% respectively), and intra-abdominal free fluid (74% and 98% respectively). Sub-group analysis was completed for detection of intra-abdominal free fluid in hypotensive (sensitivity 74% and specificity 95%), adult normotensive (sensitivity 76% and specificity 98%) and pediatric patients (sensitivity 71% and specificity 95%).
ConclusionsOur systematic review and meta-analysis suggests that e-FAST is a useful bedside tool for ruling in pneumothorax, pericardial effusion, and intra-abdominal free fluid in the trauma setting. Its usefulness as a rule-out tool is not supported by these results.

Migrations of most animal taxa are declining as a result of anthropogenic pressures and land-use transformation. Here, we document and characterize a previously unknown multi-country migration of Burchell's zebra Equus quagga that is the longest of all recorded large mammal migrations in Africa. Our data from eight adult female zebras collared on the border of Namibia and Botswana show that in December 2012 all individuals crossed the Chobe River and moved due south to Nxai Pan National Park in Botswana, where they spent a mean duration of 10 weeks before returning, less directly, to their dry season floodplain habitat. The same southward movements were also observed in December 2013. Nxai Pan appeared to have similar environmental conditions to several possible alternative wet season destinations that were closer to the dry season habitat on the Chobe River, and water availability, but not habitat or vegetation biomass, was associated with higher-use areas along the migratory pathway. These results suggest a genetic and/or cultural basis for the choice of migration destination, rather than an environmental one. Regardless of the cause, the round-trip, straight-line migration distance of 500 km is greater than that covered by wildebeest Connochaetes taurinus during their well-known seasonal journey in the Serengeti ecosystem. It merits conservation attention, given the decline of large-scale ecological processes such as animal migrations.

Four accelerator mass spectrometry (AMS) facilities undertook an interlaboratory exercise designed to examine the reliability and reproducibility of radiocarbon determinations on bone by dating a sample of elk (Alces alces) from Miesenheim IV. This specimen is derived from a secure geological context directly beneath the Laacher See tephra, which provides a precise terminus ante quern of ∼11,060 yr BP (∼13,050 cal yr BP). Regrettably, the results of the intercomparison exercise were complicated by evident contamination of the bone sample by exogenous organic material. This contaminant, probably humic acid, resulted in a wide span of ages (10,010 ± 30 to 11,100 ± 45 BP). The only method that yielded an accurate determination, consistent with the age of the tephra, was Oxford's single amino acid technique, which targets hydroxyproline. An acid hydrolysis step seems to have been crucial in breaking the bonds between the bone collagen and the contaminant.
Seabird conservation status, threats and priority actions: a global assessment

We review the conservation status of, and threats to, all 346 species of seabirds, based on BirdLife International’s data and assessments for the 2010 IUCN Red List. We show that overall, seabirds are more threatened than other comparable groups of birds and that their status has deteriorated faster over recent decades. The principal current threats at sea are posed by commercial fisheries (through competition and mortality on fishing gear) and pollution, whereas on land, alien invasive predators, habitat degradation and human disturbance are the main threats. Direct exploitation remains a problem for some species both at sea and ashore. The priority actions needed involve: a) formal and effective site protection, especially for Important Bird Area (IBA) breeding sites and for marine IBA feeding and aggregation sites, as part of national, regional and global networks of Marine Protected Areas; b) removal of invasive, especially predatory, alien species (a list of priority sites is provided), as part of habitat and species recovery initiatives; and c) reduction of bycatch to negligible levels, as part of comprehensive implementation of ecosystem approaches to fisheries. The main knowledge gaps and research priorities relate to the three topics above but new work is needed on impacts of aquaculture, energy generation operations and climate change (especially effects on the distribution of prey species and rise in sea level). We summarise the relevant national and international jurisdictional responsibilities, especially in relation to endemic and globally threatened species.

A log-coffin excavated in the early nineteenth century proved to be well enough preserved in the early twenty-first century for the full armoury of modern scientific investigation to give its occupants and contents new identity, new origins and a new date. In many ways the interpretation is much the same as before: a local big man buried looking out to sea. Modern analytical techniques can create a person more real, more human and more securely anchored in history. This research team shows how.

A retrospective ecological study was undertaken to identify social and environmental factors associated with increased incidence of meningococcal disease in university halls of residence. A standardized questionnaire was sent to UK universities and colleges of higher education outside London, for distribution to halls containing at least 50 students. Incidence rate ratios of invasive meningococcal disease were obtained for a range of social and environmental variables. Multi-variable Poisson regression analysis identified 3 factors as having a strong association: a high proportion of first year undergraduate residents (P = 0·0008), decreasing smokiness of the hall bar (P < 0·0001), and opening of hall bar before 1990 (P = 0·0001). The inverse relationship between disease incidence and smokiness of bars was an unexpected finding, and may be due to confounding factors. Universities should continue to promote awareness of meningococcal disease, encourage vaccination of first year students against serogroup C disease, and where appropriate, take measures to reduce overcrowding.

Within the setting of a regional secure unit, all doses of medication given p.r.n. over three months were ascertained and the details of each administration determined from prescription charts and a semistructured interview with the nursing staff involved. Thirty-two patients were resident for all or part of the study, all compulsorily detained. Only 15 were given medication p.r.n.) compared with the other patients they were significantly younger and more likely to be detained under civil orders than under the criminal provisions of the Mental Health Act 1983. Rarely was the medication the sole intervention in a crisis. In terms of effects on the overall treatment programme of the patient, medication given p.r.n. seemed to have an impact in only one small subgroup.
