The treatment of PTSD in refugees and asylum seekers using imagery rescripting within an NHS setting

Background:
Refugees and asylum seekers present with high levels of post-traumatic stress disorder (PTSD), whilst little research has been conducted to assess the effectiveness or acceptability of psychological interventions for this group. Imagery rescripting is effective in reducing distressing intrusive memories within a range of conditions. The current study evaluates this approach for the treatment of PTSD in refugees and asylum seekers within a UK NHS service.
Aims:To evaluate the clinical outcomes of using imagery rescripting for the treatment of PTSD in UK-based refugees and asylum seekers.
Method:Ten adult service-users from an NHS specialist service with a primary diagnosis of PTSD were recruited as part of routine service delivery. A multiple baseline design was used with participants randomly allocated to a baseline varying from 5 to 9 weeks. A baseline wait-period was followed by up to five sessions of psychoeducation and treatment preparation, in turn followed by up to 10 sessions of imagery rescripting. The Post-traumatic Symptom Scale (PSS) and Physical Health Questionnaire-9 (PHQ-9) were collected every week during baseline, at end of treatment and weekly for 5 weeks after treatment, and again at 12-week follow-up. Data were analysed with mixed regression.
Results:Results indicate a significant improvement both in PTSD symptoms and mood, and that this was attributable to the imagery rescripting phase of the intervention, and not the passage of time or non-specific therapy factors.
Conclusions:Evidence indicates imagery rescripting to be a safe and effective treatment choice for PTSD in refugees and asylum seekers.
A protocol for managing dissociative symptoms in refugee populations

This article describes a clinical protocol for supporting those presenting with post-traumatic stress disorder (PTSD) and dissociative symptoms, particularly dissociative flashbacks, based on a cross-culturally applicable model. The protocol is discussed from the perspective of working with a refugee and asylum seeker population, although many of the principles will be applicable to clients from any background presenting with these dissociative symptoms. The protocol addresses the assessment and formulation of a client’s dissociative symptoms. It includes guidance on sharing psycho-education with clients regarding the evolutionary function of dissociation and developing practical strategies to monitor and manage dissociative symptoms. The strengths and limitations of this protocol are also discussed.

(1) Be able to understand a cross-culturally applicable model of dissociation and how it applies to clinical practice when working with clients presenting with dissociative symptoms, particularly dissociative flashbacks, in the context of a diagnosis of PTSD.

The solvation of cations and anions in a lithium-containing electrolyte was studied using electrospray ionization mass spectrometry (ESI-MS) combined with nuclear magnetic resonance (NMR) and electrochemical testing. The purpose of these experiments was to develop an understanding of the solvation of the small, hard Li+ cation and the more cryptic nature of the solvation of poorly-coordinating anions such as PF6- and BF4-. It has long been held that the passivation of graphitic anodes in lithium ion batteries is a solvation-driven process, meaning that whatever solvent molecules surround the Li+ cation will provide the raw material for the formation of the solid electrolyte interphase (SEI) layer. Because the SEI is a critical component, and because a binary solvent system is normally used in lithium batteries, it is necessary to understand the competitive nature of lithium solvation. Conversely, the anion can be chemically active even if poorly coordinating; therefore, it was desired to see if a competitive solvation condition exists for the anion as well. Results indicate that Li+ has a strong preference for cyclic carbonates like ethylene carbonate (EC) over linear carbonates, where the anions had a mixed preference. It is thought that anion solvent preference might dictate oxidative chemistry that occurs on the cathode, while the anion also significantly participates in the formation of SEI on the anode.
