Registry-based trials: a potential model for cost savings?

Background/Aims:
Registry-based trials have emerged as a potentially cost-saving study methodology. Early estimates of cost savings, however, conflated the benefits associated with registry utilisation and those associated with other aspects of pragmatic trial designs, which might not all be as broadly applicable. In this study, we sought to build a practical tool that investigators could use across disciplines to estimate the ranges of potential cost differences associated with implementing registry-based trials versus standard clinical trials.
Methods:We built simulation Markov models to compare unique costs associated with data acquisition, cleaning, and linkage under a registry-based trial design versus a standard clinical trial. We conducted one-way, two-way, and probabilistic sensitivity analyses, varying study characteristics over broad ranges, to determine thresholds at which investigators might optimally select each trial design.
Results:Registry-based trials were more cost effective than standard clinical trials 98.6% of the time. Data-related cost savings ranged from $4300 to $600,000 with variation in study characteristics. Cost differences were most reactive to the number of patients in a study, the number of data elements per patient available in a registry, and the speed with which research coordinators could manually abstract data. Registry incorporation resulted in cost savings when as few as 3768 independent data elements were available and when manual data abstraction took as little as 3.4 seconds per data field.
Conclusions:Registries offer important resources for investigators. When available, their broad incorporation may help the scientific community reduce the costs of clinical investigation. We offer here a practical tool for investigators to assess potential costs savings.

Recent years have seen an exponential increase in the variety of healthcare data captured across numerous sources. However, mechanisms to leverage these data sources to support scientific investigation have remained limited. In 2013 the Pediatric Heart Network (PHN), funded by the National Heart, Lung, and Blood Institute, developed the Integrated CARdiac Data and Outcomes (iCARD) Collaborative with the goals of leveraging available data sources to aid in efficiently planning and conducting PHN studies; supporting integration of PHN data with other sources to foster novel research otherwise not possible; and mentoring young investigators in these areas. This review describes lessons learned through the development of iCARD, initial efforts and scientific output, challenges, and future directions. This information can aid in the use and optimisation of data integration methodologies across other research networks and organisations.

In substantial numbers of affected populations, disasters adversely affect well-being and influence the development of emotional problems and dysfunctional behaviors. Nowhere is the integration of mental and behavioral health into broader public health and medical preparedness and response activities more crucial than in disasters such as the 2009-2010 H1N1 influenza pandemic. The National Biodefense Science Board, recognizing that the mental and behavioral health responses to H1N1 were vital to preserving safety and health for the country, requested that the Disaster Mental Health Subcommittee recommend actions for public health officials to prevent and mitigate adverse behavioral health outcomes during the H1N1 pandemic. The subcommittee's recommendations emphasized vulnerable populations and concentrated on interventions, education and training, and communication and messaging. The subcommittee's H1N1 activities and recommendations provide an approach and template for identifying and addressing future efforts related to newly emerging public health and medical emergencies. The many emotional and behavioral health implications of the crisis and the importance of psychological factors in determining the behavior of members of the public argue for a programmatic integration of behavioral health and science expertise in a comprehensive public health response.

The close interplay between mental health and physical health makes it critical to integrate mental and behavioral health considerations into all aspects of public health and medical disaster management. Therefore, the National Biodefense Science Board (NBSB) convened the Disaster Mental Health Subcommittee to assess the progress of the US Department of Health and Human Services (HHS) in integrating mental and behavioral health into disaster and emergency preparedness and response activities. One vital opportunity to improve integration is the development of clear and directive national policy to firmly establish the role of mental and behavioral health as part of a unified public health and medical response to disasters. Integration of mental and behavioral health into disaster preparedness, response, and recovery requires it to be incorporated in assessments and services, addressed in education and training, and founded on and advanced through research. Integration must be supported in underlying policies and administration with clear lines of responsibility for formulating and implementing policy and practice.

This chapter provides an introduction to the various types of psychological treatments currently being provided to children and adolescents. Because the chapter is considered as an overview of psychological interventions, it emphasizes the various types of interventions and reviews some of the techniques of those interventions rather than focusing on the effectiveness of any particular type of intervention. While a table has been provided to review the overall efficacy and effectiveness of psychotherapeutic interventions in this patient age-grouping, more detail will be provided for the effectiveness of these interventions in the sections that discuss specific disorders. Thus the table here probably underestimates the effectiveness of these interventions because they probably are not equally effective across all mental health conditions that impact children. Since the major social context in which children operate is the home (and the school), there is mention made of family therapy as well. The section covers both non-directive and directive individual as well as group therapies. The approaches include dynamic, behavioural and cognitive-behavioural. Family and systemic therapy is reviewed as well as parent and family skills training. Such training and educational interventions also take place in social skills training and problem solving groups.

In child and adolescent psychiatry, educational and alternative interventions frequently take place in school, with treatment directed either at the identified individual or with a group of individuals with similar problems, or at the entire class or school or large subsets of the school. There may be educational programmes directed at the teachers who need to be able to handle these troubled children. But interventions can be multi systemic as well and these are in essence directed at the family to help them cope, set limits and deal with the difficult child. But the difficult child may also be removed from the family system and sent to foster care or sent to inpatient hospital. Evidence for any of these interventions is weak but the trend for many of these active and intense interventions suggests a more positive outcome than standard or less intensive treatment.
Interventions in schools
Various interventions are made to help manage young people's psychological difficulties in schools. Examples of these include but are not limited to:
(a) Consultation or training, e.g. to the teacher of a child who has ADHD, may help better manage the child in class
(b) Interventions to deal with school-wide issues such as bullying
(c) Prevention/early intervention programmes designed to inhibit the escalation of difficulties into clinical problems. These programmes may be applied to children in small groups or as a universal programme to classes of children.
(d) Tactics and strategies to help with the psychological difficulties seen in children and staff after traumatic episodes or disasters (Goenjian et al., 1997; March et al., 1998; Yule, 1992).

This chapter reviews principles and issues that need to be considered in the prescribing of psychotropic medications to children and adolescents. The brain is still developing during this period, and pruning of neurons and synapses goes on during different periods of childhood and adolescence. Fat distribution and protein-binding of medications in this group is different from adults, and issues related to absorption rate and metabolism rate also reveal differences not only between this age grouping and adults but also within different periods of childhood and adolescence. All this suggests that even more caution needs to be taken when prescribing psychotropics to children, even though there are many studies, as revealed by the tables that accompany this chapter, that suggest effectiveness of pharmacologic treatment of psychiatric disorders during childhood and adolescence.
General issues in paediatric psychopharmacology
Currently, an estimated 6% of young people under the age of 20 years in the United States receive prescriptions for psychotropic medication. This number has increased two- to threefold over the past 15 years and now closely approximates rates found in adult populations (Zito et al., 2003). Prescriptions for adolescents and preschoolers have particularly been on the rise in recent years (Martin & Leslie, 2003; Zito et al., 2000). For preschoolers, a threefold increase in stimulant usage in the early 1990s is especially notable (Zito et al., 2000).
In the UK, use of medication is much less and has been less studied.
