Evaluating variation in pre-operative evaluation and planning for children undergoing atrial or ventricular septal defect repair

Background:
CHD care is resource-intensive. Unwarranted variation in care may increase cost and result in poorer health outcomes. We hypothesise that process variation exists within the pre-operative evaluation and planning process for children undergoing repair of atrial septal defect or ventricular septal defect and that substantial variation occurs in a small number of care points.
Methods:From interviews with staff of an integrated congenital heart centre, an initial process map was constructed. A retrospective chart review of patients with isolated surgical atrial septal defect and ventricular septal defect repair from 7/1/2018 through 11/1/2020 informed revisions of the process map. The map was assessed for points of consistency and variability.
Results:Thirty-two surgical atrial septal defect/ventricular septal defect repair patients were identified. Ten (31%) were reviewed by interventional cardiology before surgical review. Of these, 6(60%) had a failed catheter-based closure and 4 (40%) were deemed inappropriate for catheter-based closure. Thirty (94%) were reviewed in case conference, all attended surgical clinic, and none were admitted prior to surgery. The process map from interviews alone identified surgery rescheduling as a point of major variability; however, chart review revealed this was not as prominent a source of variability as pre-operative interventional cardiology review.
Conclusions:Significant variation in the pre-operative evaluation and planning process for surgical atrial septal defect/ventricular septal defect patients was identified. If such process variation is widespread through CHD care, it may contribute to variations in outcome and cost previously documented within CHD surgery. Future research will focus on determining whether the variation is warranted or unwarranted, associated health outcomes and cost variation attributed to these variations in care processes.

Summary
Harry Mortimer Hubbell received his formal education in New Haven, Connecticut: a graduate of Hillhouse High School, he entered Yale to win his B.A., M.A., and Ph.D. But his progress to the doctorate was interrupted by periods of teaching in New York State and New Jersey. A combination of teaching and administrative authority in these positions is reflected in his career at Yale, where he was appointed in 1911; here, whether as instructor or Talcott Professor, a full teaching program never blunted his readiness to undertake academic committee work and finally the chairmanship of his department. It is not surprising that his delight in interpreting the values of classical literature led him in 1924 to introduce a course in Classical Civilization designed to interest those who had little or no Latin and Greek in the achievements of the ancient world. In this he was a pioneer, for few classical scholars at that time, not excepting colleagues, found such a program congenial or significant. He remained actively interested in this field throughout his life, and the proliferation of similar or derived courses in this country and elsewhere bears out his judgement. A man of such quiet energy as his could not settle into inactivity. On his retirement in 1950 a Visiting Professorship in the University of California at Berkeley was followed by a Fulbright Fellowship at the American Academy in Rome; he was one of the first John Hay Whitney Professors, joining the faculty at Goucher College, Maryland, where his enthusiasm raised a class of six to forty students.

This volumes begins with a long essay on the nature and structure of Saturnian verse. This is followed by two studies of Plautus (the Menaechmi seen as a comedy of errors and the prologue of the Poenulus as an editor's conflation of several scripts). There is an essay on nine graffito epigrams from Pompeii, and an analysis of the poetic quality of the scientific passages in the De Rerum Natura. Catullus 64 is studied as an epitome of the whole age of heroes; and there are two essays on Horace (his handling of the rhetorical recusatio in the odes to Bacchus and his lyric prayers for poetic inspiration). The volume ends with an investigation into how much Ovid actually knew of the law, and how he exploited this knowledge with piquancy and inventiveness in his writings.

Summary
The present essay does not seek to advance any new set of theories with regard to the Saturnian. Its purpose is rather to criticize some of those already current and to restate others in such a way as to make them more convincing to the skeptics. There is no dearth of correct observations in the scholarly literature devoted to the Saturnian, and there is even one general discussion, that of Leo, which seems to me to be right in many of its essential conclusions. What is most needed at present is some attempt to disengage valid observations from the fanciful contexts in which they are often embedded and, more important, some attempt to point the way, by example if possible, out of a methodological difficulty that has never been handled in a completely satisfactory fashion and is too often ignored altogether. The character of the difficulty is best stated in the words of A. W. De Groot, the scholar who has faced it most openly and coped with it most successfully:
La méthode usuelle d'investigation consiste à vérifier l'hypothèse, adoptée dès le début, que tel ou tel principe de versification est primaire … sans qu'on s'occupe sérieusement de la question de savoir si le principe reconnu comme primaire peut être secondaire, ou s'il peut y avoir deux principes primaires en même temps. Ainsi, par manque de contre-épreuve, on ne réussit ni à écarter tous les doutes sur ces points ni à déterminer le caractère et la mesure d'interdépendance de toutes les tendances observées.
