Influence of socio-economic status on Shiga toxin-producing Escherichia coli (STEC) infection incidence, risk factors and clinical features

Shiga toxin-producing Escherichia coli (STEC) infection can cause serious illness including haemolytic uraemic syndrome. The role of socio-economic status (SES) in differential clinical presentation and exposure to potential risk factors amongst STEC cases has not previously been reported in England. We conducted an observational study using a dataset of all STEC cases identified in England, 2010–2015. Odds ratios for clinical characteristics of cases and foodborne, waterborne and environmental risk factors were estimated using logistic regression, stratified by SES, adjusting for baseline demographic factors. Incidence was higher in the highest SES group compared to the lowest (RR 1.54, 95% CI 1.19–2.00). Odds of Accident and Emergency attendance (OR 1.35, 95% CI 1.10–1.75) and hospitalisation (OR 1.71, 95% CI 1.36–2.15) because of illness were higher in the most disadvantaged compared to the least, suggesting potential lower ascertainment of milder cases or delayed care-seeking behaviour in disadvantaged groups. Advantaged individuals were significantly more likely to report salad/fruit/vegetable/herb consumption (OR 1.59, 95% CI 1.16–2.17), non-UK or UK travel (OR 1.76, 95% CI 1.40–2.27; OR 1.85, 95% CI 1.35–2.56) and environmental exposures (walking in a paddock, OR 1.82, 95% CI 1.22–2.70; soil contact, OR 1.52, 95% CI 2.13–1.09) suggesting other unmeasured risks, such as person-to-person transmission, could be more important in the most disadvantaged group.

In an attempt to demonstrate the pathogenicity of a newly discovered mycoplasma (strain G37) isolated from the human genital tract, six female marmosets (Callithix jacchus) were inoculated intravaginally. Four of the animals were infected as indicated by repeated recovery of the organisms on vaginal swabbing, and infection persisted for 72–149 days or more. In addition, the infected marmosets exhibited a serum antibody response detected most easily by an immunofluorescence technique, and a persistent vaginal polymorphonuclear leucocyte response not seen in two uninfected and in two uninoculated animals.

Twenty-nine strains of penicillinase-producing Neisseria gonorrhoeae (PPNG) and 30 non-penicillinase-producing strains, all isolated in the Gambia, were characterized in terms of their plasmid content, auxotype and protein-I serovar. Sixty-two per cent of the PPNG strains contained the 3·2 MDa penicillinasecoding plasmid, and 38% had the 4·4 MDa plasmid. All the PPNG strains contained the 2·6 MDa cryptic plasmid but lacked the 24·4 MDa conjugative plasmid. In contrast, 46·7% of the non-PPNG strains harboured only the cryptic plasmid while 16·7% contained both the cryptic and conjugative plasmids. Seventeen per cent of the non-PPNG strains contained the conjugative plasmid only and 20% lacked plasmids.
The PPNG and non-PPNG strains also differed in terms of their protein-I serovar. Eighty-six per cent of the PPNG strains belonged to serogroup 1A, whereas the majority (60%) of non-PPNG strains belonged to serogroup IB. There was no significant difference in the auxotypes of the PPNG and non-PPNG strains, with both groups consisting predominantly of prototrophic and prolinerequiring strains, with a minority of strains requiring arginine. When the 59 strains were each characterized in terms of their combined plasmid profile, auxotype and serovar, 39 different combinations were noted, which indicates the heterogeneous nature of the gonococcal population found in the Gambia.

One hundred and twenty-four strains of non-penicillinase-producing gonococci isolated in Munich in 1986 were characterized in terms of their plasmid content and protein I serovar. Eighty-two per cent of the strains belonged to serogroup 1-B with over half belonging to either serovar 1B-2 or 1B-3. Half of the 22 serogroup 1A strains belonged to serovar 1A-2. Nineteen strains (15·3%) were found to lack the 2·6 Md cryptic plasmid although seven of these strains contained the 24·4 Md conjugative plasmid. Nine of the 105 strains which harboured the cryptic plasmid also contained the conjugative plasmid. The 19 strains which lacked the cryptic plasmid comprised 10 different serovars, indicating the heterogeneous nature of this group of organisms.

A fermentation-inhibition test for the measurement of growth-inhibiting anti-body to acid-producing mycoplasmas was performed in microtitre plastic plates. M. pneumoniae, M. fermentans and the Negroni agent were selected for study. Antibody could be titrated since specific antiserum inhibited mycoplasma growth and the concomitant production of acid, thus preventing a change in colour of phenol red which was incorporated in the growth medium. It was essential to use unheated horse serum as a component of the growth medium. The inhibitory effect of specific antiserum was much decreased when the horse serum was heat-inacti-vated, indicating the need for a heat-labile accessory factor. The additional use of unheated guinea-pig serum was essential for demonstrating the growth-inhibiting effect of specific antiserum on the Negroni agent. The test was reproducible, specific and sensitive. Sixteen-fold or greater antibody rises were demonstrated in paired sera from volunteers infected with M. pneumoniae. Tests with ninety-five random adult sera showed that 22% had antibody to M. pneumoniae at a titre of 1/5 or more and 13–5% had antibody to M. fermentans.
