Meaning and Purpose (MaP) therapy II: Feasibility and acceptability from a pilot study in advanced cancer

Objective
Meaning and Purpose (MaP) therapy aims to enhance meaning-based coping through a life review that focuses on the value and worth of the person, key relationships, sources of fulfillment, roles, and future priorities in living life out fully. We sought to test the feasibility and acceptability of a six-session model of MaP therapy against a wait-list control cohort in a pilot study seeking effect sizes on measures of adaptation.
MethodWe randomized patients with advanced cancer to MaP therapy or wait-list control, with measures administered at baseline and after 6–8 weeks. Wait-list patients could then crossover to receive therapy, with further measures collected postintervention. Adherence to the manualized model was sustained through weekly supervision and fidelity coding of recorded sessions. We used generalized estimating equations to control for baseline and any correlation of data.
ResultFrom 134 eligible participants, 57 (43%) consented, and 40 of 45 (89%) offered therapy completed 6 sessions. Key barriers to consenting patients were poor health (15 refusers and 4 withdrawals) and death intervened in 6 participants. MaP therapy generated adequate effect sizes in posttraumatic growth (new possibilities, appreciation of life, and personal strength) and life attitudes (choices and goal seeking) to permit calculation of power for a formal randomized, controlled trial.
Significance of resultsDelivery of this model of existentially oriented therapy is feasible and acceptable to patients. A properly powered randomized controlled trial is justified to examine the efficacy of this intervention.

Too many patients are injured in the course of care. Clinicians may mistakenly cause new harm to a patient or fail to take established steps to improve the presenting condition. Medical institutions within which they work may lack mechanisms to reduce errors or prevent them from harming patients. Many, perhaps even most, injuries are preventable, probably numbering in the hundreds of thousands a year for hospital care alone. Long ignored by medical practitioners and health-care payers and little appreciated by the public, the problem of medical injury is finally receiving high-level policy attention. Much credit goes to the Institute of Medicine (IOM) for its landmark report of November 1999, which marshaled the evidence about medical injuries and highlighted new approaches to systematic improvement of safety within systems of care.

Summary
INTRODUCTION
The Database on the Economics and Management of Endangered Species (DEMES) was developed for social scientists and policy analysts interested in the regulations affecting the management of endangered species. A mix of scientific, managerial, political, legal, and economic variables is contained in this database for species meeting one of the following criteria:
Species listed as threatened or endangered under the Endangered Species Act (ESA).
Species formerly listed under the ESA.
Species petitioned or proposed for listing between 1991 and July 1997 whose placement on the list was deemed unwarranted by Fish and Wildlife Service.
Vertebrate species with a ranking of N3 to N1 in the Nature Conservancy's (TNC) species database. TNC ranking represents the “relative rarity or endangerment of the species” and is comprised of a 5-point scale – N5 is not endangered; N1 is critically endangered. We included species with ranks between N1 and N3 to capture those species that might conceivably be considered for endangered species conservation. The database thus provides the ability to compare the management received under the ESA for an independently chosen set of endangered species.
A few additional species not meeting any of the above requirements with significant government spending.
The database contains variables describing the management of species under the ESA, and the political, legal, and social factors that might affect this decision making. The project hopes to provide an excellent starting point for researchers interested in understanding and improving the decisionmaking processes and institutional design of the ESA.

We show that if is a holomorphic function in the Dirichlet space of the unit disk, then almost all of its randomizations are multipliers of that space. This parallels a known result for lacunary power series, which also has a version for smoothness classes: every lacunary Dirichlet series lies in the Lipschitz class Lip1/2 of functions obeying a Lipschitz condition with exponent 1/2. However, unlike the lacunary situation, no corresponding “almost sure” Lipschitz result is possible for random series: we exhibit a Dirichlet function with norandomization in Lip1/2. We complement this result with a “best possible” sufficient condition for randomizations to belong almost surely to Lip1/2. Versions of our results hold for weighted Dirichlet spaces, and much of our work is carried out in this more general setting.
