Coinfections and antimicrobial use in patients hospitalized with coronavirus disease 2019 (COVID-19) across a single healthcare system in New York City: A retrospective cohort study

Background and objective:
With the coronavirus disease 2019 (COVID-19) pandemic, rates of in-hospital antimicrobial use increased due to perceived bacterial and fungal coinfections along with COVID-19. We describe the incidence of these coinfections and antimicrobial use in patients hospitalized with COVID-19 to help guide effective antimicrobial use in this population.
Setting:This study was conducted in 3 tertiary-care referral university teaching hospitals in New York City.
Methods:This multicenter retrospective observational cohort study involved all patients admitted with COVID-19 from January 1, 2020, to February 1, 2021. Variables of interest were extracted from a de-identified data set of all COVID-19 infections across the health system. Population statistics are presented as median with interquartile range (IQR) or proportions with 95% confidence intervals (CIs) as indicated.
Results:Among 7,209 of patients admitted with COVID-19, 663 (9.2%) had a positive culture from the respiratory tract or blood sometime during their initial hospital admission. Positive respiratory cultures occurred found in 449 (6.2%) patients, and 20% were collected within 48 hours of admission. Blood culture positivity occurred in 334 patients (4.6%), with 33.5% identified within 48 hours of admission. A higher proportion of patients received antimicrobials in the first wave than in the later pandemic period (82.4% vs 52.0%). Antimicrobials were prescribed to 70.1% of inpatients, with a median of 6 antimicrobial days per patient. Infection-free survival decreased over the course of hospitalization.
Conclusions:We detected a very low incidence of coinfection with COVID-19 at admission. A longer duration of hospitalization was associated with an increased risk of coinfection. Antimicrobial use far exceeded the true incidence and detection of coinfections in these patients.

Summary
In recent years, an international consensus has emerged claiming that developing nations must now, more than ever, merge the fields of foreign policy and health. To that end, in 2006, the Ministers of Foreign Affairs from Brazil, France, Indonesia, Norway, Senegal, South Africa, and Thailand established a ministerial accord agreement in Oslo, Norway, emphasizing that healthcare become an integral component of foreign policy (Amorim et al., 2006). Whereas improved diplomatic relations in health, also commonly referred to as global health diplomacy, has led to increased communication between nations, technical assistance, and disease preparedness (Long, 2011), unfortunately, it also has led to an increase in domestic healthcare inequalities and policy shortcomings. As I explain in this chapter, this shortcoming mainly has to do with how global health diplomacy has shifted politicians’ focus away from prioritizing domestic healthcare needs.

In the Americas, next to the United States, Brazil and Mexico have the highest prevalence of type-2 diabetes. In contrast to most studies, this article compares and analyzes the politics behind the implementation of type-2 diabetes self-care management programs (DSM), which is a new area of scholarly research. This article claims that Brazil outpaced Mexico with respect to the implementation of effective DSM programs, the product of positive policy spillover effects associated with the president and governing political party’s popular anti-poverty programs, and the enduring legacy of centralized ministry of health financial and human resource assistance to primary care programs in a context of decentralization. Brazil also benefited from having a strong partnership with international health agencies. None of these factors was present in Mexico. Findings suggest that more research needs to go into understanding the complex political and inter-governmental contexts facilitating DSM program implementation, which is a neglected area of research.

In recent years, several emerging economies have introduced national health insurance programs ensuring access to health care while offering financial protection from out-of-pocket and catastrophic expenses. Nevertheless, in several nations these expenses continue to increase. While recent research has emphasized the lack of funding, poor policy design and corruption as the main culprits, little is known about the politics of establishing federal regulatory agencies ensuring that state governments adhere to national insurance reimbursement and coverage procedures. This article fills in this lacuna by providing an alternative perspective, one that accounts for differences between nations in the creation of regulatory institutions, with an emphasis instead on governing elite strategies to campaign on access to health care during transitions to democracy, civil societal mobilization, constitutional constraints and the national electoral incentives to overcome ineffective decentralization processes. The cases of Indonesia and China are introduced as examples of how and why their differences in this political process accounted for Indonesia’s success and China’s failure to ensure financial protection.

Evan Lieberman's Boundaries of Contagion: How Ethnic Politics Have Shaped Government Responses to Aids proceeds from a simple question of great importance to millions of people: “Why have some governments responded to AIDS more quickly and more broadly than others?” In answering this question, Lieberman employs a range of methods and engages a range of scholarly literatures dealing with health policy, comparative public policy, and ethnic politics. Because the book addresses “big” issues and bridges conventional divides in political science, we have invited a number of colleagues working broadly in comparative politics to comment on it.—Jeffrey C.
