Approaches for enhancing the informativeness and quality of clinical trials: Innovations and principles for implementing multicenter trials from the Trial Innovation Network

One challenge for multisite clinical trials is ensuring that the conditions of an informative trial are incorporated into all aspects of trial planning and execution. The multicenter model can provide the potential for a more informative environment, but it can also place a trial at risk of becoming uninformative due to lack of rigor, quality control, or effective recruitment, resulting in premature discontinuation and/or non-publication. Key factors that support informativeness are having the right team and resources during study planning and implementation and adequate funding to support performance activities. This communication draws on the experience of the National Center for Advancing Translational Science (NCATS) Trial Innovation Network (TIN) to develop approaches for enhancing the informativeness of clinical trials. We distilled this information into three principles: (1) assemble a diverse team, (2) leverage existing processes and systems, and (3) carefully consider budgets and contracts. The TIN, comprised of NCATS, three Trial Innovation Centers, a Recruitment Innovation Center, and 60+ CTSA Program hubs, provides resources to investigators who are proposing multicenter collaborations. In addition to sharing principles that support the informativeness of clinical trials, we highlight TIN-developed resources relevant for multicenter trial initiation and conduct.

Background
Studying phenotypic and genetic characteristics of age at onset (AAO) and polarity at onset (PAO) in bipolar disorder can provide new insights into disease pathology and facilitate the development of screening tools.
AimsTo examine the genetic architecture of AAO and PAO and their association with bipolar disorder disease characteristics.
MethodGenome-wide association studies (GWASs) and polygenic score (PGS) analyses of AAO (n = 12 977) and PAO (n = 6773) were conducted in patients with bipolar disorder from 34 cohorts and a replication sample (n = 2237). The association of onset with disease characteristics was investigated in two of these cohorts.
ResultsEarlier AAO was associated with a higher probability of psychotic symptoms, suicidality, lower educational attainment, not living together and fewer episodes. Depressive onset correlated with suicidality and manic onset correlated with delusions and manic episodes. Systematic differences in AAO between cohorts and continents of origin were observed. This was also reflected in single-nucleotide variant-based heritability estimates, with higher heritabilities for stricter onset definitions. Increased PGS for autism spectrum disorder (β = −0.34 years, s.e. = 0.08), major depression (β = −0.34 years, s.e. = 0.08), schizophrenia (β = −0.39 years, s.e. = 0.08), and educational attainment (β = −0.31 years, s.e. = 0.08) were associated with an earlier AAO. The AAO GWAS identified one significant locus, but this finding did not replicate. Neither GWAS nor PGS analyses yielded significant associations with PAO.
ConclusionsAAO and PAO are associated with indicators of bipolar disorder severity. Individuals with an earlier onset show an increased polygenic liability for a broad spectrum of psychiatric traits. Systematic differences in AAO across cohorts, continents and phenotype definitions introduce significant heterogeneity, affecting analyses.

We present a preliminary analysis of known planetary nebulae (PNe) in M31 that were observed in the first year of the Panchromatic Hubble Andromeda Treasury HST Multi-cycle program. We use the properties of this sample to discuss PNe from this new multi-band survey.

A study of the 1978 application for permission to undertake exploratory drilling in South Davis Strait, northeastern Canada, resulted in the identification of problems with the federal Environmental Assessment and Review Process (EARP). These problems have recurred in other environmental impact assessments since 1978 in other parts of northern Canada, and some improvements have been made in approaches to them. However, further progress is needed if the EARP process is to become a more widely accepted method than at present of conducting evaluations of development impacts and the means of managing them in northern Canada.
This paper focuses on the procedural rather than the scientific aspects of EARP in northeastern Canada. Recommendations include: 1) applying environmental impact assessment to policies as well as development activities; 2) strengthening socio-economic expertise and approaches; 3) financing of interveners and support for a more balanced range of interests than was displayed in this case; 4) provision of sufficient notification and adequate information; 5) documentation of rationale for decisions; and 6) monitoring and evaluation by the Canadian Federal Environmental Assessment and Review Office (FEARO) to ensure compliance with set conditions and to facilitate the tackling of unforeseen problems.

A method is described of matching environmentally significant areas (ESAs) with the most appropriate form of management for each, selected from all the available forms. This involves comparing the purpose, reasons for significance, existing uses, and tenure, of each ESA, with the characteristics of agencies, protected area categories, legislation, and non-statutory arrangements for management, as well as examining agency coordination mechanisms or other means of implementation. The method was formulated for ESAs in the Northwest Territories (NWT) of Canada, but is considered to have wide applicability. It is illustrated with a test application to seven proposed ESAs in what is termed the eastern Beaufort Sea region.

A method is described of applying institutional arrangements to an individual environmentally significant area (ESA). It is assumed that an ESA has been recognized by park, wildlife, or other groups, on the basis of abiotic, biotic, and/or cultural, characteristics, as well as judgements about significance and constraints. National park, wildlife reserve, or other, institutional arrangements, may also have been suggested for the ESA. The goal is to decide whether these or other arrangements are appropriate— through analysis of relevant: (1) legislation; (2) agencies; and (3) reserve designation or land management types.

Although many people still view the Arctic as little affected by human activity, the region has been the home of Man for thousands of years and its wildlife and ecology have been changed in many ways in the last four hundred years as a result particularly of the commercial and technical activities of western industrial nations. Three forces are, however, now bringing more and more attention to the use and conservation of arctic lands and seas: (a) arctic petroleum exploration and development, which is becoming increasingly extensive offshore in Alaska, Canada, and Norway; (2) The Law of the Sea Convention, which is leading to the division of the formerly open Arctic into large spheres of national jurisdiction through the declaration of Exclusive Economic Zones (EEZ); and (3) the World Conservation Strategy, which is providing the conceptual basis for the international cooperation and comprehensive management that is clearly needed to introduce sustainable development in the Arctic.
It is suggested that one promising means of planning and implementing balanced use and conservation in the waters and adjacent lands of the Arctic is through the UNEP Regional Seas Programme. This institutional arrangement is already being used in similar resource-management circumstances in eleven other regions of the world. The Regional Seas Programme is action-oriented and includes four basic elements: (1) promotion of international and regional conventions, guidelines, and activities; (2) assessment of marine pollution and related effects; (3) coordination in protection, development, and management; and (4) support through training, education, and other means.

The idea of ecotourism is being promoted and supported, by growing numbers of people and groups in different parts of the world, as a major means of dealing with the damaging effects of tourism. Yet the meaning of the term varies among different people, projects, and places. Evidence from national parks, where this type of tourism has been promoted for many years, shows that such tourism can cause substantial long-term cumulative changes in environment. Concepts such as ecotourism, green tourism, and sustainable tourism development, are general in their nature and have to be described, planned, and assessed, in detail on the ground in terms of the socioeconomic and environmental conditions applying in different places. In this respect, careful planning and management procedures are needed not only for ecotourism but indeed for all forms of tourism.
