Summary
Periodical culture was the realm within which Wollstonecraft realized, in her words, a “new plan of life,” marked by “a little peace and independence.”1 While not the first female writer to publish criticism, Wollstonecraft has been rightly identified as “the first truly professional woman literary critic,”2 as she managed to support herself by working as a periodical contributor and editorial assistant for the last ten years of her life. Describing her ambition to her sister Everina, Wollstonecraft declared herself an original species, emboldened by the encouragement of her publisher Joseph Johnson: “Mr. Johnson … assures me that if I exert my talents in writing I may support myself in a comfortable way. I am then going to be the first of a new genus—.”3 In 1787, Johnson welcomed Wollstonecraft back to London after a series of professional and personal setbacks, including the closing of her school at Newington Green, the death of Fanny Blood, and her dismissal as governess by Lady Kingsborough. Johnson’s financial, intellectual, and emotional support would continue to play an integral role in Wollstonecraft’s career, but it was his launching of the Analytical Review in 1788 that provided Wollstonecraft with the means of making a living via a robust participation in the public sphere. In this way, periodicals served as a proving ground for Wollstonecraft’s social and political philosophy, and a means by which she developed her singular feminist voice.
Do combined ultrasound and electrocardiogram-rhythm findings predict survival in emergency department cardiac arrest patients? The Second Sonography in Hypotension and Cardiac Arrest in the Emergency Department (SHoC-ED2) study

Objectives
Point-of-care ultrasound (POCUS) is used increasingly during resuscitation. The aim of this study was to assess whether combining POCUS and electrocardiogram (ECG) rhythm findings better predicts outcomes during cardiopulmonary resuscitation in the emergency department (ED).
MethodsWe completed a health records review on ED cardiac arrest patients who underwent POCUS. Primary outcome measurements included return of spontaneous circulation (ROSC), survival to hospital admission, and survival to hospital discharge.
ResultsPOCUS was performed on 180 patients; 45 patients (25.0%; 19.2%–31.8%) demonstrated cardiac activity on initial ECG, and 21 (11.7%; 7.7%–17.2%) had cardiac activity on initial POCUS; 47 patients (26.1%; 20.2%–33.0%) achieved ROSC, 18 (10.0%; 6.3%–15.3%) survived to admission, and 3 (1.7%; 0.3%–5.0%) survived to hospital discharge. As a predictor of failure to achieve ROSC, ECG had a sensitivity of 82.7% (95% CI 75.2%–88.7%) and a specificity of 46.8% (32.1%–61.9%). Overall, POCUS had a higher sensitivity of 96.2% (91.4%–98.8%) but a similar specificity of 34.0% (20.9%–49.3%). In patients with ECG-asystole, POCUS had a sensitivity of 98.18% (93.59%–99.78%) and a specificity of 16.00% (4.54%–36.08%). In patients with pulseless electrical activity, POCUS had a sensitivity of 86.96% (66.41%–97.22%) and a specificity of 54.55% (32.21%–75.61%). Similar patterns were seen for survival to admission and discharge. Only 0.8% (0.0–4.7%) of patients with ECG-asystole and standstill on POCUS survived to hospital discharge.
ConclusionThe absence of cardiac activity on POCUS, or on both ECG and POCUS together, better predicts negative outcomes in cardiac arrest than ECG alone. No test reliably predicted survival.
Emergency department ultrasound for the detection of B-lines in the early diagnosis of acute decompensated heart failure: a systematic review and meta-analysis

Objectives
Dyspnea is a common presenting problem that creates a diagnostic challenge for physicians in the emergency department (ED). While the differential diagnosis is broad, acute decompensated heart failure (ADHF) is a frequent cause that can be challenging to differentiate from other etiologies. Recent studies have suggested a potential diagnostic role for emergency lung ultrasound (US). The objective of this systematic review was to assess the accuracy of early bedside lung US in patients presenting to the ED with dyspnea.
MethodsA systematic search of EMBASE, PubMed, and the Cochrane Library was performed in addition to a grey literature search. We selected prospective studies that reported on the sensitivity and specificity of B-lines from early lung ultrasound in dyspneic patients presenting to the ED. Selected studies underwent quality assessment using the Critical Appraisal and Skills Program (CASP) questionnaire.
Data Extraction and SynthesisThe search yielded 3674 articles; seven studies met inclusion criteria and fulfilled CASP requirements for a total of 1861 patients. Summary statistics from the meta-analysis showed that as a diagnostic test for ADHF, bedside lung US had a pooled sensitivity of 82.5% (95% confidence interval [CI]=66.4% to 91.8%) and a pooled specificity of 83.6% (95% CI=72.4% to 90.8%).
ConclusionsOur results suggest that in patients presenting to the ED with undifferentiated dyspnea, B-lines from early bedside lung US may be reliably used as an adjunct to current diagnostic methods. The incorporation of lung US may lead to more appropriate and timely diagnosis of patients with undifferentiated ADHF.
A traumatic tale of two cities: a comparison of outcomes for adults with major trauma who present to differing trauma centres in neighbouring Canadian provinces

Objectives
While the use of formal trauma teams is widely promoted, the literature is not clear that this structure provides improved outcomes over emergency physician delivered trauma care. The goal of this investigation was to examine if a trauma team model with a formalized, specialty-based trauma team, with specific activation criteria and staff composition, performs differently than an emergency physician delivered model. Our primary outcome was survival to discharge or 30 days.
MethodsAn observational registry-based study using aggregate data from both the New Brunswick and Nova Scotia trauma registries was performed with data from April 1, 2011 to March 31, 2013. Inclusion criteria included patients 16 years-old and older who had an Injury Severity Score greater than 12, who suffered a kinetic injury and arrived with signs of life to a level-1 trauma centre.
Results266 patients from the trauma team model and 111 from the emergency physician model were compared. No difference was found in the primary outcome of proportion of survival to discharge or 30 days between the two systems (0.88, n=266 vs. 0.89, n=111; p=0.8608).
ConclusionsWe were unable to detect any difference in survival between a trauma team and an emergency physician delivered model.

No evidence for evolution in the Far-Infrared-Radio correlation out to z ~ 2 in the ECDFS

The Far-Infrared Radio Correlation (FRC) is the tightest and most universal correlation known among global parameters of galaxies. Here we present the results of our investigation of the 70 μm FRC of starforming galaxies in the Extended Chandra Deep Field South (ECDFS) out to z > 2. In order to quantify the evolution of the FRC we used both survival analysis and stacking techniques, which gave similar results. We also calculated the FRC using total infrared luminosity and rest-frame radio luminosity, qTIR, and find that qTIR is constant (within 0.22) over the redshift range 0 - 2. We see no evidence for evolution in the FRC at 70 μm, which is surprising given the many factors that are expected to change this ratio at high redshifts.

This study examines, on a per-case basis, the social costs associated with contraceptive failures and resulting term pregnancies. To combat unintended pregnancy and escalating health care costs, the public sector needs to provide greater access to highly effective methods of contraception.
X-Ray Studies of Near-Frictionless Carbon Films.

Carbon-based coatings exhibit many attractive properties that make them good candidates for a wide range of engineering applications. Tribological studies of the films have revealed a close correlation between the chemistry of the hydrocarbon source gases and the coefficients of friction and wear rates of the diamond-like carbon films. Those films grown in source gases with higher hydrogen-to-carbon ratios had much lower coefficients of friction and wear rates than did films derived from source gases with lower hydrogen-to-carbon ratios. The mechanism for this low friction is as yet not properly understood. Ongoing structural characterization of the films at Argonne National Laboratory is gradually revealing this mechanism. Recent studies have included x-ray photoelectron spectroscopy (XPS), near edge x-ray absorption fine structure (NEXAFS) and x-ray reflectivity (XRR). XPS showed ∼10% oxygen at the surface, which was largely removed after a 1 minute sputter; NEXAFS showed a high sp2:sp3 ratio implying a highly graphitic material; and XRR has given a comprehensive depth profile, with three layers of increasing density as the substrate was approached. The paper discusses the results and correlation with previous friction measurements.
