Evaluation of a multisectoral intervention to mitigate the risk of severe acute respiratory coronavirus virus 2 (SARS-CoV-2) transmission in long-term care facilities

Objective:
A Canadian health authority implemented a multisectoral intervention designed to control severe acute respiratory coronavirus virus 2 (SARS-CoV-2) transmission during long-term care facility (LTCF) outbreaks. The primary objective was to evaluate the effectiveness of the intervention 14 days after implementation.
Design:Quasi-experimental, segmented regression analysis.
Intervention:A series of outbreak measures classified into 4 categories: case and contact management, proactive case detection, rigorous infection control practices and resource prioritization and stewardship.
Methods:A mixed-effects segmented Poisson regression model was fitted to the incidence rate of coronavirus disease 2019 (COVID-19), calculated every 2 days, within each facility and case type (staff vs residents). For each facility, the outbreak time period was segmented into an early outbreak period (within 14 days of the intervention) and postintervention period (beyond 14 days following the intervention). Model outputs quantified COVID-19 incidence trend and rate changes between these 2 periods. A secondary model was constructed to identify effect modification by case type.
Results:The significant upward trend in COVID-19 incidence rate during the early outbreak period (rate ratio [RR], 1.07; 95% confidence interval [CI], 1.03–1.11; P < .001) reversed during the postintervention period (RR, 0.73; 95% CI, 0.67–0.80; P < .001). The average trend did not differ by case type during the early outbreak period (P > .05) or the postintervention period (P > .05). However, staff had a 70% larger decrease in the average rate of COVID-19 during the postintervention period than residents (RR, 0.30; 95% CI, 0.10–0.88; P < .05).
Conclusions:Our study provides evidence for the effectiveness of this intervention to reduce the transmission of COVID-19 in LTCFs. This intervention can be adapted and utilized by other jurisdictions to protect the vulnerable individuals in LTCFs.

Mean fecundity of hemlock sawfly females emerging from cocoons collected on heavily defoliated western hemlock trees was significantly less than fecundity of females collected from lightly defoliated trees. Fecundity was highly correlated with female cocoon diameter and female forewing length, but was most highly correlated with both variables combined. Time-saving double sampling techniques are discussed for determining mean fecundities of new samples when mean cocoon diameter and forewing length are known.

Hemlock sawfly infestations occur most commonly in the warm southern half of southeast Alaska. Biotic factors that limit sawfly populations are a fungus, Entomophthora sphaerosperma, which kills disproportionately large numbers of female larvae; food quality as affected by host site and defoliation; and three ichneumonid parasitoid species, Delomerista japonica diprionis, Oresbius tsugae tsugae, and Itoplectis quadricingulatus, which attack prepupal larvae in cocoons. E. sphaerosperma was the most effective natural control in recent years, especially during wetter than normal summers. Parasites which attack after the fungus has decimated larval populations were most abundant during two dry summers. Parasitization and sawfly emergence were directly related, and both were highly inversely correlated with percentage fungus killed sawflies. Sawflies reared on lightly defoliated trees produced significantly more eggs than those reared on heavily defoliated trees, and fecundity was directly related to cumulative heat units during the summer. Between 1952 and 1974, most negative sawfly population trends south of Frederick Sound occurred when two wet summers occurred consecutively. Apparent widespread collapse of sawfly populations in 1974 was associated with low temperatures in 1973 which delayed sawfly development and reduced opportunities for successful oviposition.

Derived simply from the Greek work chronos, meaning time, the label ‘chronic’ denotes an illness of long duration or one of frequent recurrence. However, when chronic is paired with schizophrenia, as in ‘this person is a chronic schizophrenic’, the connotation becomes an expectation of deterioration, defect, or deficit states (Cutting, 1983). These perceptions about schizophrenia have pervaded and guided clinical judgements (Feighner et al, 1972; American Psychiatric Association, 1980, 1987), treatment programming (Bachrach, 1979; Lamb, 1981; Strauss & Glazer, 1982), policy formulation (Greenblatt, 1978; Talbott, 1979), and decisions about priority for funding (Kraft, 1981). These perceptions have also stripped hopes of recovery from patients and their families (Chamberlin, 1979; Lovejoy, 1984). Further, the use of phrases such as ‘deinstitutionalisation of chronic mental patients’ glosses over the large heterogeneity of patient types, courses of illness and recovery, and the actual shifts in composition and migrations of groups of patients within society (Lamb, 1979; Leighton, 1982; Harding & Ashikaga, 1982).

Summary
Are adult development and the course of mental disorder related to each other in major ways, or are they mutually irrelevant? Polar opposite views have dominated this issue. From the beginning of modern psychiatry in the nineteenth century, two major theories of the course of mental disorders, the psychoanalytic and the natural history theories, have upheld opposite extremes. The natural history view has suggested that any mental disorder, like any other type of disease, has a natural history, so that developmental considerations are essentially irrelevant to course. In fact, this model states that a disorder or disease process is defined by its course, and that course is what establishes the disease as a diagnostic entity. Thus, for example, in describing dementia praecox – later called the group of schizophrenias by Bleuler (1911/ 1950) – Kraepelin brought together three rather different syndromes under one diagnostic entity because he believed that they each had a deteriorating course and outcome (Kraepelin, 1902).
In contrast to that approach, the psychoanalytic orientation has suggested that the course of a mental disorder is intimately tied to psychological developmental mechanisms. In this view, the course of a psychosis, for example, has been perceived as the result of the degree to which id, ego, and superego pressures and mechanisms can be reconciled, which in turn is a function of their developmentally influenced strengths and patterns of functioning (Freud, 1924).
In this chapter we shall review briefly some recent major trends in understanding the course of mental disorders and adult aspects of development suggesting that these two domains are neither identical nor totally unrelated.

Our reintroduction of the concept of negative symptoms in schizophrenia (Strauss et al, 1974) has been followed by much productive research on this topic. As in the work of Andreason, Crowe, and others, the focus of most of this research has been on improving descriptive assessments of these symptoms (Andreason, 1982) and exploring their biological correlates (Crowe, 1981). This work has been extremely important, but psychological and social aspects of negative symptoms are also important (Barton, 1959; Wing & Brown, 1970; Gruenberg, 1967), and in these areas there has been little research. Nevertheless, it is crucial to explore possible psychological and social factors in negative symptoms in order to understand the symptoms more fully and to provide a basis for more adequate prevention and treatment. It is also important to clarify psychological and social factors in these symptoms to provide a basis for more effective biological research, since the heterogeneity of negative symptoms at a psychological level could hide significant biological correlates.

The research question of which mediating factors influence the long-term course of schizophrenia was not asked until recently because the expectation has been of uniformly poor outcome (Kraepelin, 1902; American Psychiatric Association, 1980). However, anecdotal clinical knowledge about heterogeneity in the long-term course of this severe illness has been firmly supported in the last 15 years by six recent longitudinal studies in Europe, Asia, and the USA (Blueler, 1972; Ciompi & Müller, 1976; Huber et al, 1979; Tsuang et al, 1979; Harding et al, 19876; Ogawa et al, 1987). Each of these studies, more methodologically rigorous than those of the past, has found multiple indices of wide heterogeneity in the long-term outcome of schizophrenia with trends toward significant improvement or recovery demonstrated in over half of each cohort.
