Comorbidities associated with 30-day readmission following index coronavirus disease 2019 (COVID-19) hospitalization: A retrospective cohort study of 331,136 patients in the United States

Objective:
Hospital readmission is unsettling to patients and caregivers, costly to the healthcare system, and may leave patients at additional risk for hospital-acquired infections and other complications. We evaluated the association between comorbidities present during index coronavirus disease 2019 (COVID-19) hospitalization and the risk of 30-day readmission.
Design, setting, and participants:We used the Premier Healthcare database to perform a retrospective cohort study of COVID-19 hospitalized patients discharged between April 2020 and March 2021 who were followed for 30 days after discharge to capture readmission to the same hospital.
Results:Among the 331,136 unique patients in the index cohort, 36,827 (11.1%) had at least 1 all-cause readmission within 30 days. Of the readmitted patients, 11,382 (3.4%) were readmitted with COVID-19 as the primary diagnosis. In the multivariable model adjusted for demographics, hospital characteristics, coexisting comorbidities, and COVID-19 severity, each additional comorbidity category was associated with an 18% increase in the odds of all-cause readmission (adjusted odds ratio [aOR], 1.18; 95% confidence interval [CI], 1.17–1.19) and a 10% increase in the odds of readmission with COVID-19 as the primary readmission diagnosis (aOR, 1.10; 95% CI, 1.09–1.11). Lymphoma (aOR, 1.86; 95% CI, 1.58–2.19), renal failure (aOR, 1.32; 95% CI, 1.25–1.40), and chronic lung disease (aOR, 1.29; 95% CI, 1.24–1.34) were most associated with readmission for COVID-19.
Conclusions:Readmission within 30 days was common among COVID-19 survivors. A better understanding of comorbidities associated with readmission will aid hospital care teams in improving postdischarge care. Additionally, it will assist hospital epidemiologists and quality administrators in planning resources, allocating staff, and managing bed-flow issues to improve patient care and safety.

Summary
The importance of the Turkana Basin (northern Kenya) and lower Omo Valley (southern Ethiopia) in our understanding of the environments in which early hominins lived, and on how climate changes may have influenced the emergence and disappearance of species, is undeniable (e.g., Bobe and Leakey, 2009; Levin et al., 2011). The majority of the Plio-Pleistocene sediments belong to the Omo Group, which consists of the Shungura, Usno, Nkalabong, and Mursi formations in Ethiopia and of the Nachukui and Koobi Fora formations in Kenya (e.g., Brown and McDougall, 2011). The Shungura, Nachukui, and Koobi Fora formations are well-studied (e.g., Howell and Coppens, 1976; de Heinzelin, 1983a, 1983b; Harris et al., 1988a, 1988b; Koobi Fora Research Project Volumes). The Mursi Formation of the lower Omo Valley, with a date of more than 4 million years, is among the oldest formations of the Omo Group in Ethiopia. However, it has been poorly documented despite its importance in informing us on the mid-Pliocene, a period in which the genus Australopithecus arose, but that is presently inadequately known. This chapter will present the formation with the history of the research in the area, including renewed fieldwork. It will be followed by a description of the geology and taphonomy and, finally, a preliminary analysis of the fauna found to date.

Summary
Venture capital can play a role in bridging the gap between laboratory-based research and therapies developed by pharmaceutical companies for Alzheimer’s disease (AD). The Dementia Discovery Fund (DDF), was formed to provide funding and expertise to support companies developing novel treatments for dementia. Combining a focus on dementia with the rigour of for-profit venture capital investing greatly improves the odds of developing an effective therapeutic. Investment decisions are based on credible, proven science with the potential to deliver a drug, supported by excellent management teams. The DDF supports companies both financially and strategically drawing on the vast experience of its team of venture investors and scientific experts. Building syndicates with other investors de-risks individual investments, also providing the company with access to more capital and range of expertise. With a recent increase in the amount of available risk capital, coupled with a better understanding of the pathophysiology of AD and the use of precision medicine approaches, venture capital can have a greater impact on the discovery and development of a life-changing drug for patients with AD.

Summary
Abstract
This chapter examines the process of adapting organized Islam to the institutional needs of the modern state. The adaptation processes that Islam is currently undergoing in Europe are compared to similar developments in the history of the Catholic Church.

Introduction
The process of adapting organized Islam to the institutional needs of the modern state in Europe is happening less than a century after the agreements between the Catholic Church and the European states, and two centuries after that of Judaism. However, Islam cannot really be considered late if we take into account the great number of Muslim-majority countries that endured colonial continuity after the withdrawal of the Ottoman Empire, with the arrival of the French, British, and Italian Empires. Only through national independence has Islam begun to find a role in state institutions, a process that has become clearer through the pressure for democratization. This chapter compares the adaptation processes that organized Islam is currently undergoing in Europe with similar developments in the Catholic Church in the nineteenth-century United States.
The Muslim diaspora
The democratization of Islamic nation-states is happening at the same time as the migration that has given birth to an overseas diaspora. We are thus witnessing the natural reaction of the older centres of Islamic religious authority to the appearance of new demographic groups. For example, there is growing investment by the capital cities of origin countries in the construction of monumental mosques in Europe for the use of their diaspora (Fuess, 2007). There is a range of training programmes for imams that are adapted to the language and culture of the European destination countries. And we can see the beginnings of hierarchical structures of ulama that are adapted to everyday life in Europe, of which the European Council of Moroccan Ulema is a prime example. In the past, these efforts were mostly uncoordinated and eclectic – and they still do not always manage to meet the actual demand for infrastructure and religious authority to respond to daily life. As of yet, none play the role of the ministry of waqf or traditional habous (‘religious endowments’) in Muslim-majority countries.

Using direct photoelectrochemical measurement of the photocurrent obtained from Cu2ZnSnS4 (CZTS) absorber layers made by a two-stage electroplating-sulfurisation process, the influence of processing conditions (temperature, time, and pressure) on material quality was investigated with a view to understanding the long sulfurisation times usually found in the literature. The improvement in photocurrent due to KCN etching was also studied, and seems to be due both to removal of surface phases and also slower etching of the bulk material. The optimum sulfurisation time was found to be around 50 minutes, despite evidence that sulfur incorporation and phase formation are complete within 5 minutes. Slow grain growth was suggested as a rate-limiting factor, and a rate constant was derived based on a simple model.

Brain imaging is one of the most remarkable technological advances towards understanding the relationship of behavior to brain anatomy and physiology. Brain images provide insight to understanding behavior. Additionally, the images themselves carry great impact, particularly when used to show differences in either the anatomy or the biological functioning of two different brains. For these reasons, brain images have increasingly been used in both criminal and civil trials.
After describing some general features of brain imaging, we will focus on functional magnetic imaging (fMRI), as many believe this technology has the most potential for advancing our understanding of how parts of the brain function, including perhaps linking specific functions with cognition and behavior. Brain imaging as a field is vast and therefore our discussion will be limited. First, we will assess the advantages and limitations of fMRI, including research efforts towards standardizing equipment thereby assuring reliability and reproducibility.

Introduction
Contemporary scholarly accounts of Islam in Europe have tended to portray one of two extreme visions: heaven on earth or hell in a hand basket. In the pessimistic version, foreign policy analyses and the scholarly literature are in rare concurrence about the meager chances for either inter-religious dialog or Muslim integration. These accounts bear witness to a showdown between intransigently secular states and an ambitious, fundamentalist religion whose followers aim to transform the continent into “Eurabia” (Ye'or 2005; Ferguson 2004; Savage 2004). To justify their gloom, these authors cite Islam's un-hierarchical nature and the impossibility of establishing legitimate representatives – or “one phone number” – for Muslim communities in Europe (Rémond 1999; Warner and Wenner 2002). Compounding this difficulty, these scholars emphasize, is the inadequacy of Europe's nineteenth-century State–Church institutions, which stumble from crisis to crisis with this new and agile religious challenger (Fetzer and Soper 2005; Shore 2004). The optimistic voices, on the other hand, come from the camps of post-nationalist theorists and from proponents of a reformed “Euro-Islam” that is divorced from overseers and financiers in the Muslim world (Soysal 1994). But those authors' cheerfulness is founded, respectively, on two formidable hypotheses: the diminishing importance of “host” state institutions for immigrant integration; and the “sending” states’ renunciation of religious influence over the Muslim Diaspora (AlSayyad and Castells 2002). A decade after they were first expounded, neither of these scenarios is in view.

Summary
The political transformation of South Africa, a long and continuing process marked by its turning point, the democratic elections of April 1994, has been accompanied by expectations of economic transformation. Under the political regime of apartheid, minority control had generated an economic system that was unsustainable: the extreme inequality of income and wealth, principally along ethnic lines, could not be compatible with social stability; economic growth had been negative for a decade, reflecting the structural weakness of the existing economy; and South Africa's relationship to world markets was fragmented by both protection and sanctions, especially since access to world financial markets was interrupted in 1985. With political transformation, structural economic change was an imperative in all three respects. “Nation building” requires old inequalities to be addressed and a reversal of declining average gross domestic product, while the restoration of normal international relations meant South Africa had to achieve a new role in the world economy, a global system that is itself rapidly changing.
Irrespective of policy initiatives, South Africa's economy would change to a greater or lesser degree in response to shocks, but the question is whether South Africa's new government could construct an economic strategy that delivers structural change appropriate to the new political environment at home and abroad. Since 1994 the government has maintained a policy regime similar to the old one. It has implemented neither a radically “liberal” strategy, comprising rapid deregulation and privatization of the large state sector it inherited and complete liberalization of foreign exchange markets, nor a radically “populist” policy of expanded public expenditure to address housing shortages, education inequalities, and unemployment.
