Effect of the COVID-19 Pandemic on Musculoskeletal Care in the Emergency Room

Objective:
Study the effect of the coronavirus disease (COVID-19) pandemic on emergency room (ER) utilization for musculoskeletal (MSK) complaints.
Methods:Data from ER encounters in a teaching hospital for years 2019 and 2020 were extracted from an electronic database. The ICD-10-CM codes were used to assign the primary diagnosis. Joinpoint regression was applied to a time-series to detect changes in daily encounters (all-cause and MSK). Total number and median daily encounters were calculated by year, sex, age groups, and diagnostic codes for each year.
Results:Overlapping intervals within March 2020 showed drops of 8.1 all-cause encounters per day (95% CI: 4.8-11.5), and 1.2 MSK encounters per day (95% CI: 0.8-1.7), resulting in net losses for the year of 33.2% for all-cause and 35.8% for MSK encounters. Reductions were observed for both sexes and all age groups. MSK codes with largest declines were low back pain, joint pain, and limb pain.
Conclusion:The COVID-19 pandemic resulted in major reductions in all-cause and MSK encounters in the ER of an academic hospital probably due to governmental restrictions and a change in patients behaviors. Persistence of the observed reduction suggests that patients requiring ER services delayed seeking care for MSK and other health conditions.
Do CVM Welfare Estimates Suffer from On-Site Sampling Bias? A Comparison of On-Site and Household Visitor Surveys

The problem of endogenous stratification associated with on-site sampling has been overlooked in the Contingent Valuation Method (CVM). We find that using on-site samples of visitors overstates visitor willingness to pay (WTP) estimates relative to a household sample of visitors, and substantially overstates the unconditional population values. We provide two methods of correcting WTP of on-site samples. The uncorrected on-site sample CVM yields WTP of $132 per trip, while visitor WTP obtained from a random sample of households had a value of $66 per trip. Adaptation of choice-based sampling correction estimator to the on-site CVM data yields $73 per trip, not statistically different from the visitor value from the household survey, but significantly different from the uncorrected on-site sample value.
Mathematical and Physical Modeling of Three-Phase Gas-Stirred Ladles

A very realistic 1:17 scale physical model of a 140-ton gas-stirred industrial steel ladle was used to evaluate flow patterns measured by Particle Image Velocimetry (PIV), considering a three-phase system (air-water-oil) to simulate the argon-steel-slag system and to quantify the effect of the slag layer on the flow patterns. The flow patterns were evaluated for a single injector located at the center of the ladle bottom with a gas flow rate of 2.85 l/min, with the presence of a slag phase with a thickness of 0.0066 m. The experimental results obtained in this work are in excellent agreement with the trends reported in the literature for these gas-stirred ladles. Additionally, a mathematical model was developed in a 2D gas-stirred ladle considering the three-phase system built in the physical model. The model was based on the Eulerian approach in which the continuity and the Navier Stokes equations are solved for each phase. Therefore, there were three continuity and six Navier-Stokes equations in the system. Additionally, turbulence in the ladle was computed by using the standard k-epsilon turbulent model. The agreement between numerical simulations and experiments was excellent with respect to velocity fields and turbulent structure, which sets the basis for future works on process analysis with the developed mathematical model, since there are only a few three-phase models reported so far in the literature to predict fluid dynamics in gas-stirred steel ladles.
A Joint Estimation Method to Combine Dichotomous Choice CVM Models with Count Data TCM Models Corrected for Truncation and Endogenous Stratification

We update the joint estimation of revealed and stated preference data of previously published research to allow for joint estimation of the Travel Cost Method (TCM) portion using count data models. The TCM estimation also corrects for truncation and endogenous stratification as well as overdispersion. The joint estimation allows for testing consistency of behavior between revealed and stated preference data rather than imposing it. We find little gain in estimation efficiency, but our joint estimation might make a significant improvement in estimation efficiency when the contingent valuation scenarios involve major changes in site quality not reflected in the TCM data.

Background
Congenital cardiac diseases are the most frequent congenital malformations. In adult patients, the mineralisation of the aorta due to cardiovascular disease is very common, but vascular mineralisation in paediatric cardiopathies is a topic less studied. This study shows that children with a complex congenital cardiopathy show a high degree of vascular mineralisation in the ascending aorta. This can be part of the cardiac failure pathophysiology due to congenital cardiopathies.
ObjectiveThe aim of this study was to determine the presence and degree of vascular mineralisation in samples of the ascending and descending aorta of children with complex congenital cardiopathies.
DesignWe conducted a cross-sectional study.
SubjectsWe obtained 34 vascular tissues from the autopsies of 17 children with congenital cardiac disease.
MethodsWe used a scanning electron microscope with an energy-dispersive X-ray spectroscopy in order to analyse the vascular tissues.
ResultsThe amount of minerals was two times higher in the ascending aorta than in the descending aorta of children with congenital cardiac disease.
ConclusionsThe study provides evidence that vascular mineralisation can start at an early age, and that it is higher in the ascending aorta than in the descending aorta.

Photoinduced Charge Transfer at Hybrid Semiconductor Interfaces

We monitor in real time photoinduced charge injection at the interface between a fluorinated copper phthalocyanine layer (CuPcF16) deposited by thermal evaporation on top of a p - doped GaAs (100) wafer. Literature data on the electron affinity of CuPcF16 (5.2 eV respect to vacuum level) combined with photoemission measurements indicates an energy offset of 1.1 eV for the GaAs conduction band respect to the CuPcF16 LUMO level. This suggests that charge transfer at the organic - inorganic interface is feasible. We study bilayers of GaAs and CuPcF16 thin films (25 nm) by pump - probe spectroscopy with 200 fs time resolution. Pump photons at 780 nm excites the CuPcF16 layer whereas probe photons in the visible range, reflected by the GaAs surface, monitor induced changes at the interface. We observe a strong photoinduced absorption band centered around 560 nm which appears during the pulse duration, shows a build-up dynamics and persists beyond 0.2 ns. This band cannot be attributed to single material contribution, as demonstrated by test experiments with single layers. By applying steady state (CW) electromodulated spectroscopy we identify charge state absorption in CuPcF16 in the same spectral region as the photoinduced absorption band. We thus assign our transient dynamics to formation of CuPcF16 ions at the interface, following charge injection. On account of the rapid charge formation we identify this system as a potential candidate for the fabrication of hybrid photodiodes.
