EMDR v. other psychological therapies for PTSD: a systematic review and individual participant data meta-analysis

Background
This systematic review and individual participant data meta-analysis (IPDMA) examined the overall effectiveness of eye movement desensitization and reprocessing (EMDR) in reducing posttraumatic stress disorder (PTSD) symptoms, achieving response and remission, and reducing treatment dropout among adults with PTSD compared to other psychological treatments. Additionally, we examined available participant-level moderators of the efficacy of EMDR.
MethodsThis study included randomized controlled trials. Eligible studies were identified by a systematic search in PubMed, Embase, PsyclNFO, PTSDpubs, and CENTRAL. The target population was adults with above-threshold baseline PTSD symptoms. Trials were eligible if at least 70% of study participants had been diagnosed with PTSD using a structured clinical interview. Primary outcomes included PTSD symptom severity, treatment response, and PTSD remission. Treatment dropout was a secondary outcome. The systematic search retrieved 15 eligible randomized controlled trials (RCTs); 8 of these 15 were able to be included in this IPDMA (346 patients). Comparator treatments included relaxation therapy, emotional freedom technique, trauma-focused cognitive behavioral psychotherapies, and REM-desensitization.
ResultsOne-stage IPDMA found no significant difference between EMDR and other psychological treatments in reducing PTSD symptom severity (β = −0.24), achieving response (β = 0.86), attaining remission (β = 1.05), or reducing treatment dropout rates (β = −0.25). Moderator analyses found unemployed participants receiving EMDR had higher PTSD symptom severity at the post-test, and males were more likely to drop out of EMDR treatment than females.
ConclusionThe current study found no significant difference between EMDR and other psychological treatments. We found some indication of the moderating effects of gender and employment status.

Respected as one of four ‘feuilles de qualité’ in nineteenth-century France, the Journal des débats politiques et littéraires published articles by some of the most talented writers/critics of its time. In ‘feuilletons’, large articles that ran across the bottom of the first and second pages, these authors gave perceptive critiques in high-quality prose and provided their readers with relief from the political news discussed on the page above. In January 1858 literary critic Hippolyte Rigault asserted that modern criticism communicated not just through forthright judgements but also through innuendo and nuance. A sophisticated readership could then be expected to take up the task of understanding the allusions and filling in the blanks. Like Rigault, Hector Berlioz (music critic of the Débats from 1835 to 1863) and Ernest Reyer (from 1866 to 1898) used both text and subtext to convey their assessments. This study, with the goal of examining how shades of approval and disapproval could be alluded to or directly revealed, traces how they wrote about their younger contemporary Georges Bizet in the years following Rigault's article.

The Press as Mirror
As the great Exposition of 1900 was nearing its close and the end of the nineteenth century was fast approaching, the journalists Eugène Allard and Louis Vauxcelles presented the readership of Le Figaro with the rationale for their forthcoming series, “Les Conquêtes du siècle.” They asked: “Are we ahead or behind? Should we be proud of ourselves or ashamed when we compare our work with the endeavors of foreigners?” Their efforts at digesting the accomplishments of the nineteenth century and ranking French contributions to various disciplines were designed to provide entertainment for the general reader. And so, between 25 September and 25 October 1900, they presented eight articles based on interviews with and letters from prominent composers and musicians, most of them French. In response to a preset list of questions, these figures commented on the evolution of the symphony, opera, and chamber music; identified some of the principal figures responsible for such change; and considered whether there had been any real progress in the general public’s appreciation of music.
Like others, Adolphe Jullien, the historian, critic, and biographer of Berlioz, scoffed at the notion of a “top ten” and noticed with amusement how these composers managed with “touching ingenuity” to turn their interviews into proud assessments of their own achievements; but he also observed with approval that those interviewed agreed on two points: first, that the most significant advances in nineteenth-century music had taken place in symphonic style and orchestral sound (including the “conquest” of the opera by the symphony); and second, that these advances stemmed from the influence of three seminal figures—Beethoven, Berlioz, and Wagner. In fact, while Beethoven ranked high on everyone’s list and Wagner was mentioned frequently, only a few spoke for Berlioz. Augusta Holmès, for one, put Berlioz and Wagner in the same sentence and gave credit to both for their contributions to the evolution of harmony, orchestration, and theatrical forms. Gustave Charpentier, acknowledging Berlioz’s contribution to the symphony and praising his own maître, Massenet, spoke largely of the social utility of art, and of his own art in particular.

The Royal College of Psychiatrists' Higher Specialist Training Handbook (1998) emphasises the need for training schemes to be well organised in order to provide an environment in which training needs can be met. Training programme directors are tasked to provide “regular meetings with the trainees as a group to discuss the scheme and its placements” as well as “occasional meetings with trainers to discuss the scheme and its further development”.

Summary
“Impact” – the forceful contact or collision of one body against another – is a particularly appropriate word to describe Hector Berlioz's effect upon his contemporaries, whether the composers of his own or of the next generation, the public, or colleagues in the press. Many in France acknowledged the genius of Berlioz the musician, but in his own time true appreciation of his achievement tended to reside mainly with a few ardent admirers, for Berlioz cultivated a style that was so distinctive, subjective, and exploratory that general audiences did not embrace the bulk of his works.
In 1870, only a year after his death, a youthful Adolphe Jullien stressed the personal character of Berlioz's music by characterizing the man and his art as one and the same: “he acts, he thinks, he lives in his works. Each page of his music is made in his own image.” Younger composers, intent on building their careers, tended to avoid the risky course of adopting wholesale his innovations and individual style. In 1871 Georges Bizet expressed both the deep admiration and the wariness symptomatic of his generation's attitude toward Berlioz as a model:
[W]hat makes for success is the talent and not the idea. The public […] only understands the idea later on, but to make it to this “later on” the artist's talent has to make the road accessible to the public, by means of appealing forms, and not to put people off from the start. In such a way Auber, who had so much talent and so few ideas, was almost always understood, while Berlioz, who had genius but no talent, almost never was.
