Late surgical site infections among solid organ transplant recipients: an unrecognized clinical entity

This study identified 26 late invasive primary surgical site infection (IP-SSI) within 4–12 months of transplantation among 2073 SOT recipients at Duke University Hospital over the period 2015–2019. Thoracic organ transplants accounted for 25 late IP-SSI. Surveillance for late IP-SSI should be maintained for at least one year following transplant.

Background: Shared Healthcare Intervention to Eliminate Life-threatening Dissemination of MDROs in Orange County, California (SHIELD OC) was a CDC-funded regional decolonization intervention from April 2017 through July 2019 involving 38 hospitals, nursing homes (NHs), and long-term acute-care hospitals (LTACHs) to reduce MDROs. Decolonization in NH and LTACHs consisted of universal antiseptic bathing with chlorhexidine (CHG) for routine bathing and showering plus nasal iodophor decolonization (Monday through Friday, twice daily every other week). Hospitals used universal CHG in ICUs and provided daily CHG and nasal iodophor to patients in contact precautions. We sought to evaluate whether decolonization reduced hospitalization and associated healthcare costs due to infections among residents of NHs participating in SHIELD compared to nonparticipating NHs. Methods: Medicaid insurer data covering NH residents in Orange County were used to calculate hospitalization rates due to a primary diagnosis of infection (counts per member quarter), hospital bed days/member-quarter, and expenditures/member quarter from the fourth quarter of 2015 to the second quarter of 2019. We used a time-series design and a segmented regression analysis to evaluate changes attributable to the SHIELD OC intervention among participating and nonparticipating NHs. Results: Across the SHIELD OC intervention period, intervention NHs experienced a 44% decrease in hospitalization rates, a 43% decrease in hospital bed days, and a 53% decrease in Medicaid expenditures when comparing the last quarter of the intervention to the baseline period (Fig. 1). These data translated to a significant downward slope, with a reduction of 4% per quarter in hospital admissions due to infection (P < .001), a reduction of 7% per quarter in hospitalization days due to infection (P < .001), and a reduction of 9% per quarter in Medicaid expenditures (P = .019) per NH resident. Conclusions: The universal CHG bathing and nasal decolonization intervention adopted by NHs in the SHIELD OC collaborative resulted in large, meaningful reductions in hospitalization events, hospitalization days, and healthcare expenditures among Medicaid-insured NH residents. The findings led CalOptima, the Medicaid provider in Orange County, California, to launch an NH incentive program that provides dedicated training and covers the cost of CHG and nasal iodophor for OC NHs that enroll.

Summary
Studies of predator sensitive foraging in nonhuman primates are in their infancy. Predator sensitive foraging (or threat sensitive foraging, Helfman 1989) represents the strategies that animals employ to balance the need to eat against the need to avoid being eaten. For decades, these trade-offs have been investigated, in both natural and laboratory settings, by dozens of scientists working with a wide array of invertebrate, piscine, avian, and mammalian species. Ecologists working with nonprimate taxa have developed theoretical models of considerable sophistication and have generated elegant data to test those models. However, only recently have primatologists begun to turn their attention to this area of research.
The fact that studies of primates lag behind those of other taxa is due, in part, to the difficulties of observing predation on primates or even evaluating the extent to which predation imposes selective pressures on primate populations. Furthermore, field research imposes additional challenges in controlling certain variables so as to isolate and assess the effects of others. However, primate research reveals complex interactions among variables and may therefore provide a more realistic portrait of animal ecology than do oversimplified laboratory experiments. Furthermore, given the diversity in primate morphology, social structure, and habitats, this taxon represents an important opportunity to test theoretical models. It is also valuable to study the principles of predator sensitive foraging in a group of animals that rely heavily upon learning and are remarkably flexible in their behavior, as these factors may lead to further diversity and specificity in foraging strategies. Altogether, primatology has the potential to make a significant contribution to this field of research.

Introduction
Primatologists have only recently begun to investigate the impact of predation on foraging strategies, and the many variables that might mediate this relationship. Traditionally, success in foraging and success in avoiding predators were treated as two independent phenomena. Both might depend upon group size, but the interaction was rarely considered (e.g., for capuchins, Miller 1992, de Ruiter 1986, Srikosamatara 1987). This study explores the extent to which group size influences perceived risk of predation and how this, in turn, affects food intake. Data come from an ongoing study of one population of wedge-capped capuchins (Cebus olivaceus), living at the Hato Piñero Biological Reserve in central Venezuela.
Previous studies with this population of capuchins revealed a strong link between group size and seasonality in food intake. Adult females living in larger groups maintain an essentially constant level of food intake throughout the year (approximately 1800 cm3 of food per female per day, on average). In contrast, those in smaller groups experience dramatic annual variation, consuming very little during the dry season (approx. 1100 cm3) but much more in the wet season (approx. 2900 cm3) (Miller 1996). This pattern is only partly the result of differences in foraging effort. During the wet season, small-group females devote significantly more time to feeding and foraging than do large-group females (68% and 55%, respectively, of their daily activity budgets) and so the disparity in food intake is predictable. However, during the dry season, foraging time is not significantly different (58% in larger groups and 54% in smaller groups) (Miller 1996).

The book was born out of my frustration over an apparent impasse in primatology and my excitement at discovering that other ecologists had, in fact, moved beyond that obstacle. From my first year in graduate school, I was frustrated by what I perceived as a false dichotomy, the seemingly endless debate among primate socioecologists over the adaptive functions of grouping: Did primates live in groups to reduce predator pressure or to increase foraging success? Surely, I always thought, both factors could be important. Then I got out to the field and was fascinated by the variability in behavior that I was observing among capuchin monkeys. Some monkeys took risks that others avoided, and these different strategies led to differential access to food and water. Clearly, the monkeys were behaving in ways that balanced their simultaneous desires for safety and full stomachs, and the particular balance that each individual struck was based (it seemed to me) upon how vulnerable that individual was (or perceived itself to be). Upon delving into the ecological literature, I found a vast body of data pertaining to this exact balancing act: predator sensitive foraging. Countless scientists working with diverse taxa – insects, fish, birds, rodents, ungulates – were already investigating the strategies that animals use to enhance foraging success under the constraints of predator pressure. But where was the research on primates? There were some important studies, but they were remarkably few.
My enthusiasm for exploring predator sensitive foraging among primates led me to propose a symposium on the subject.

Predator sensitive foraging represents the strategies that animals employ to balance the need to eat against the need to avoid being eaten. Ecologists working with a wide range of taxa have developed sophisticated theoretical models of these strategies, and have produced elegant data to test them. However, only recently have primatologists begun to turn their attention to this area of research. This volume brings together primary data from a variety of primate species living in both natural habitats and experimental settings, and explores the variables that may play a role in primates' behavioural strategies. Taken together, these studies demonstrate that predator sensitive foraging is relevant to many primates, of various body sizes and group sizes and living in different environments. Eat or be Eaten encourages further discussion and investigation of the subject. It will make fascinating reading for researchers and students in primatology, ecology and animal behaviour.
