Autonomic indices and loss-of-control eating in adolescents: an ecological momentary assessment study

Background
Loss-of-control (LOC) eating commonly develops during adolescence, and it predicts full-syndrome eating disorders and excess weight gain. Although negative emotions and emotion dysregulation are hypothesized to precede and predict LOC eating, they are rarely examined outside the self-report domain. Autonomic indices, including heart rate (HR) and heart rate variability (HRV), may provide information about stress and capacity for emotion regulation in response to stress.
MethodsWe studied whether autonomic indices predict LOC eating in real-time in adolescents with LOC eating and body mass index (BMI) ⩾70th percentile. Twenty-four adolescents aged 12–18 (67% female; BMI percentile mean ± standard deviation = 92.6 ± 9.4) who reported at least twice-monthly LOC episodes wore biosensors to monitor HR, HRV, and physical activity for 1 week. They reported their degree of LOC after all eating episodes on a visual analog scale (0–100) using a smartphone.
ResultsAdjusting for physical activity and time of day, higher HR and lower HRV predicted higher self-reported LOC after eating. Parsing between- and within-subjects effects, there was a significant, positive, within-subjects association between pre-meal HR and post-meal LOC rating. However, there was no significant within-subjects effect for HRV, nor were there between-subjects effects for either electrophysiologic variable.
ConclusionsFindings suggest that autonomic indices may either be a marker of risk for subsequent LOC eating or contribute to LOC eating. Linking physiological markers with behavior in the natural environment can improve knowledge of illness mechanisms and provide new avenues for intervention.
Early adulthood psychiatric diagnoses and the subsequent risk of life-time incarceration: a cohort study

Background
The association between incarceration and psychiatric disorders has been noted. Yet, existing studies are cross-sectional or examine the risk of recidivism, which has limited the predictive validity of psychiatric disorders as a risk factor for incarceration. To overcome this limitation, this study used a prospective cohort to examine whether psychiatric diagnoses in early adulthood predicted incarceration throughout a 30-year follow-up. It tested the association between psychiatric diagnoses with future incarcerations, their number and durations, controlling for education and ethnic status.
MethodsThis study merged data from three sources in Israel: a prospective 10-year birth cohort study of young adults aged 25–34, conducted in the 1980s (N = 4914) that included a psychiatric interview; data from the Prison Service, including the cause, number and duration of incarcerations; and from the Vital Statistics Registry on death records.
ResultsMultivariate analysis showed that substance-use disorders, antisocial personality and lower levels of education predicted future incarceration, their number and maximum duration. The remainder diagnoses were not significantly associated with future incarceration.
ConclusionsResults limited the prediction of future incarcerations to persons diagnosed with substance use and antisocial personality, and do not support an independent predictive association between additional psychiatric diagnoses and future incarceration.

The Protoplanetary Discussions conference—held in Edinburgh, UK, from 2016 March 7th–11th—included several open sessions led by participants. This paper reports on the discussions collectively concerned with the multi-physics modelling of protoplanetary discs, including the self-consistent calculation of gas and dust dynamics, radiative transfer, and chemistry. After a short introduction to each of these disciplines in isolation, we identify a series of burning questions and grand challenges associated with their continuing development and integration. We then discuss potential pathways towards solving these challenges, grouped by strategical, technical, and collaborative developments. This paper is not intended to be a review, but rather to motivate and direct future research and collaboration across typically distinct fields based on community-driven input, to encourage further progress in our understanding of circumstellar and protoplanetary discs.

Summary
Introduction
For the hospitalized patient, prevention of pressure ulcers requires vigilant surveillance by a dedicated care team. It is estimated that 10–15% of patients who are in the ICU for one week develop a pressure sore. Nursing-home patients have a 10–30% prevalence of pressure ulcers, whereas spinal cord injury patients have a 50–80% lifetime risk. Numerous patient factors, including malnutrition, obesity, smoking, immobility, diabetes, neurologic injury, and hip fractures, are associated with the development of pressure ulcers. Prolonged pressure with resultant tissue ischemia, shear stress, and edema all lead to tissue necrosis. Moisture and infection are factors which may cause additional progression of severity. Pressure ulcers typically occur over bony prominences where pressure and shear stress are the greatest. The most common sites are found over the ischial tuberosities, sacrum, greater trochanters, and heels. Without prompt recognition and treatment, pressure ulcers will progress through stages of increasing tissue damage. Table 94.1 shows the current pressure ulcer staging system, which is based on gross appearance.
Stages I and II will usually resolve with conservative treatment including topical creams, such as silver sulfadiazine, frequent changes in patient position, air fluid beds, protective bandages, and improved nutrition. Few stage III ulcers can be treated effectively with conservative measures and progression to stage IV generally requires operative intervention. Effective management requires a multidisciplinary team including nurses, dieticians, physical therapists, social workers, and surgeons. Because of the difficulty in treating these wounds and the high rate of recurrence, prevention is of paramount importance. Frequent weight shifts to relieve pressure, proper hygiene, optimal nutrition, and primary education are important measures for prevention.
