Scenario-Based Collaboration: Identifying the Role of the NSW Biocontainment Centre in the Response to High Consequence Infectious Diseases

Introduction:
The New South Wales (NSW) Biocontainment Centre (NBC) is the first high-level isolation unit in Australia. This state-wide referral facility, located at Westmead Hospital, Sydney, will provide care for patients with high-consequence infectious diseases (HCIDs), including viral hemorrhagic fevers (VHF). In preparation, a tabletop exercise with key stakeholders was held to introduce and socialize the NBC’s capacity to support NSW’s preparedness for the management of a patient with HCID.
Method:Invitations were provided to key stakeholders within Westmead Hospital (facility executive, emergency and ICU services, security, switchboard); NSW Preparedness and Response Branch; retrieval services; peripheral hospitals; pediatric hospital network; and NSW Public health units (state and district).
The scenario presented was an unwell patient with suspected VHF arriving at a peripheral hospital emergency department.
Discussion during the four-hour long exercise was facilitated with directed questions and injects, and was recorded. Recommendations and key learnings from the exercise and debrief provided opportunities to enhance current response assumptions.
Results:Forty-five people participated both face-to-face and virtually. Participant discussion showed increasing appreciation of patient presentations to any part of the NSW health system and available assistance. Recommendations included: enhanced access to NBC support with a direct “1-800 number”, coordination for communication, treatment, and transport, and if required, deployment of an NBC team to peripheral sites. Areas for future collaborative work were identified.
Conclusion:This exercise successfully achieved collaboration of key stakeholders to develop an updated, comprehensive and robust plan for management of HCID patients within NSW, regardless of their presentation site. It has created an opportunity to brainstorm and optimize how the NBC can interact with other agencies to maximize the NSW HCID response.

Summary
The smallest and least-studied of the Chelsea porcelain manufactory’s wares are seals for watch fobs and étui, first described in an advertisement of 1754 as “Trinkets for Watches (mounted in gold and unmounted) in various shapes.” Rival manufacturers, Charles Gouyn, near St. James’s Square, produced similar pocket-sized toys and trinkets. At less than an inch in height, these miniatures depicted birds, animals, and amatory subjects, alongside figurines reflecting eighteenth-century society and culture. Aesthetically crude in reduction and compression, the figures are distorted, and over time the details covered in glaze and paint are gradually softened through constant caressing or clinking against a metal fob or étui. Many survive with generic intaglio hardstone matrices mounted in precious metal. Over 200 models have been identified—the majority published in G. E. Bryant’s Chelsea Porcelain Toys (1925)—attesting to the importance of consumer culture at mid-century. Novelties intended as gifts or love tokens, they are inherently charming for their smallness, yet their subject matter frequently touches on bigger issues of globalization, empire, colonialism, and race. These themes privilege the elite market for these wares, exposing the passions, pursuits, prejudices, and obsessions of their customers, who literally held the world in their hands.
A State of Biopreparedness

Introduction:
Westmead Hospital (WMH) recognized gaps in its preparedness to respond to the Ebola 2014 outbreak in West Africa. A fragmented system was identified. A ‘State of Bio-preparedness’ project team convened to discuss all healthcare services in the planning, training, and implementation of a biopreparedness response.
Methods:A survey targeting the staff’s competence and confidence in biologically hazardous infection management was conducted. Semi-structured interviews explored staff members’ experiences and perspectives of biopreparedness response. The collaborative team called “State of Biopreparedness” (SOB) was assembled and a clinical practice improvement project was undertaken. To assess readiness, nine simulated Viral Haemorrhagic Fever (VHF) exercises involving staff and consumers were conducted. These exercises were debriefed by the multidisciplinary committee and themes and issues were identified. These nine simulation drills then assessed readiness and evaluated performance.
Results:A number of consistent issues continue to emerge including:
1. A standard communication pathway for notification was needed - use of the incident paging system (111 pages) to notify the hospital’s incident management team.
2. A consistent and coordinated approach to the training and maintenance of standardized and high-level Personal Protective Equipment (PPE) protocols for frontline clinical and clinical staff was required.
3. Clear delineation of roles and responsibilities and supporting these roles by translating the VHF Control Guideline and policy into task cards and checklists.
4. Strengthening intra- and interdepartmental staff collaboration and communication.
5. Infection control measures to be taken by staff after identifying a patient with possible VHF to reduce the risk of transmission of disease to staff, other patients, and visitors.
Discussion:Integrating disaster management processes with clinical protocols had a positive impact on the hospital’s biopreparedness response. Simulation exercises were a vital and practical way for staff to feel confident and competent to perform their roles.

Personality disorders have for many years been on the fringe of psychiatry, with considerable doubts expressed about the usefulness, implications and validity of the concept. It is argued here that developments in the past few years have brought personality disorders into the mainstream of psychiatric practice. In particular, the recognition that personality function can be separated usefully from clinical symptoms, and that both mental state and personality can be disordered simultaneously, has led to better assessment and understanding. Advances in the classification, epidemiology, treatment and prognosis of personality disorders show that these conditions are common, extensive in their pathology, and cause much suffering. They cannot be ignored or dismissed as peripheral to psychiatry for they are an essential part of good psychiatric practice.
