A five-year quasi-experimental study to evaluate the impact of empiric antibiotic order sets on antibiotic use metrics among hospitalized adult patients

Objective:
Evaluation of adult antibiotic order sets (AOSs) on antibiotic stewardship metrics has been limited. The primary outcome was to evaluate the standardized antimicrobial administration ratio (SAAR). Secondary outcomes included antibiotic days of therapy (DOT) per 1,000 patient days (PD); selected antibiotic use; AOS utilization; Clostridioides difficile infection (CDI) cases; and clinicians’ perceptions of the AOS via a survey following the final study phase.
Design:This 5-year, single-center, quasi-experimental study comprised 5 phases from 2017 to 2022 over 10-month periods between August 1 and May 31.
Setting:The study was conducted in a 752-bed tertiary care, academic medical center.
Intervention:Our institution implemented AOSs in the electronic medical record (EMR) for common infections among hospitalized adults.
Results:For the primary outcome, a statistically significant decreases in SAAR were detected from phase 1 to phase 5 (1.0 vs 0.90; P < .001). A statistically significant decreases were detected in DOT per 1,000 PD (4,884 vs 3,939; P = .001), fluoroquinolone orders (407 vs 175; P < .001), carbapenem orders (147 vs 106; P = .024), and clindamycin orders (113 vs 73; P = .01). No statistically significant change in mean vancomycin orders was detected (991 vs 902; P = .221). A statistically significant decrease in CDI cases was also detected (7.8, vs 2.4; P = .002) but may have been attributable to changes in CDI case diagnosis. Clinicians indicated that the AOSs were easy to use overall and that they helped them select the appropriate antibiotics.
Conclusions:Implementing AOS into the EMR was associated with a statistically significant reduction in SAAR, antibiotic DOT per 1,000 PD, selected antibiotic orders, and CDI cases.

Substantial progress has been made in the standardization of nomenclature for paediatric and congenital cardiac care. In 1936, Maude Abbott published her Atlas of Congenital Cardiac Disease, which was the first formal attempt to classify congenital heart disease. The International Paediatric and Congenital Cardiac Code (IPCCC) is now utilized worldwide and has most recently become the paediatric and congenital cardiac component of the Eleventh Revision of the International Classification of Diseases (ICD-11). The most recent publication of the IPCCC was in 2017. This manuscript provides an updated 2021 version of the IPCCC.
The International Society for Nomenclature of Paediatric and Congenital Heart Disease (ISNPCHD), in collaboration with the World Health Organization (WHO), developed the paediatric and congenital cardiac nomenclature that is now within the eleventh version of the International Classification of Diseases (ICD-11). This unification of IPCCC and ICD-11 is the IPCCC ICD-11 Nomenclature and is the first time that the clinical nomenclature for paediatric and congenital cardiac care and the administrative nomenclature for paediatric and congenital cardiac care are harmonized. The resultant congenital cardiac component of ICD-11 was increased from 29 congenital cardiac codes in ICD-9 and 73 congenital cardiac codes in ICD-10 to 318 codes submitted by ISNPCHD through 2018 for incorporation into ICD-11. After these 318 terms were incorporated into ICD-11 in 2018, the WHO ICD-11 team added an additional 49 terms, some of which are acceptable legacy terms from ICD-10, while others provide greater granularity than the ISNPCHD thought was originally acceptable. Thus, the total number of paediatric and congenital cardiac terms in ICD-11 is 367. In this manuscript, we describe and review the terminology, hierarchy, and definitions of the IPCCC ICD-11 Nomenclature. This article, therefore, presents a global system of nomenclature for paediatric and congenital cardiac care that unifies clinical and administrative nomenclature.
The members of ISNPCHD realize that the nomenclature published in this manuscript will continue to evolve. The version of the IPCCC that was published in 2017 has evolved and changed, and it is now replaced by this 2021 version. In the future, ISNPCHD will again publish updated versions of IPCCC, as IPCCC continues to evolve.

We describe an ultra-wide-bandwidth, low-frequency receiver recently installed on the Parkes radio telescope. The receiver system provides continuous frequency coverage from 704 to 4032 MHz. For much of the band ( ${\sim}60\%$ ), the system temperature is approximately 22 K and the receiver system remains in a linear regime even in the presence of strong mobile phone transmissions. We discuss the scientific and technical aspects of the new receiver, including its astronomical objectives, as well as the feed, receiver, digitiser, and signal processor design. We describe the pipeline routines that form the archive-ready data products and how those data files can be accessed from the archives. The system performance is quantified, including the system noise and linearity, beam shape, antenna efficiency, polarisation calibration, and timing stability.

Hyperspectral cathodoluminescence imaging provides spectrally and spatially resolved information on luminescent materials within a single dataset. Pushing the technique toward its ultimate nanoscale spatial limit, while at the same time spectrally dispersing the collected light before detection, increases the challenge of generating low-noise images. This article describes aspects of the instrumentation, and in particular data treatment methods, which address this problem. The methods are demonstrated by applying them to the analysis of nanoscale defect features and fabricated nanostructures in III-nitride-based materials.

Summary
Introduction
This chapter discusses the efforts being made in East Asia towards financial integration, and the prospects for it becoming generally accepted as a consensus goal of national policies. At present, financial integration in East Asia is largely a discussion topic for national officials in regional gatherings, both formal and bilateral and the subject of research, rather than a planned objective or even a limited agreed direction of policy. At the same time, several separate motives and common external forces have caused an increase in discussion of prospects for financial cooperation and integration in East Asia, and greater cooperation among national interests that traditionally compete in economic, financial and trade objectives. This has made the possibility of an agreement on financial integration as a deliberate objective more tangible than before.
This chapter begins with a discussion of the reasons underlying discussions relating to East Asian financial cooperation and integration (section II). It then reviews progress to date in a number of areas, of which the most significant are initiatives involving regional economic cooperation and Asian bond market development (section III). In looking to possible future developments, it is useful to compare the experience to date of the most successful regional financial integration initiative, that of the European Union (EU) (section IV).

Solid state crystallization of hydrogenated amorphous silicon (a-Si:H) prepared by hot-wire CVD is studied in solid phase epitaxy mode. By using a novel optical method combined with cross-sectional TEM and SIMS, a reduction of epitaxial growth speed is observed with increase in a-Si:H film thickness. Namely, in films thinner that 0.5 micron, solid phase epitaxy velocity depends linearly on film thickness. As the film thickness increases beyond 1 micron, the average velocity of solid phase epitaxy decreases considerably with respect to that in thinner films. In this regime, its velocity becomes also time-dependent: initial slow propagation of crystallization front gets considerably accelerated after the front has traveled above 400nm. SIMS thickness profiles of hydrogen shows considerably more residual hydrogen in thicker films after the start of solid phase epitaxy. In addition, prolonged pre-dehydrogenation at lower temperatures results in the increase in the average epitaxy speed in thicker films. These phenomena are likely related to delayed hydrogen outdiffusion in thicker films, which also leads to time-dependent speed of the solid-phase epitaxy front. We conclude that excess residual hydrogen reduces the rate of solid-phase crystalline growth.

ZnO films with orientations of (001), (110), and (100) were fabricated on silicon by different substrate biases at low temperature. Dynamic cathodoluminescence (CL) dependence on electron bombardment revealed unstable Zn-N bonding if N2 was used as a predecessor. CL under various accelerated voltages showed the possible energies of Zn-N. N-related photoluminescence (PL) at low temperature confirmed that nitrogen was released after annealing. These N-doping behaviors agreed to the theoretical calculation.

Diamond-like nanocomposite (DLN) film, by embedding the diamond-like carbon into an amorphous SiO2 network, is a special class of modified diamond-like carbon (DLC) coating, which shows many excellent properties and potential applications. In this work, DLN films were fabricated in an acetylene atmosphere by ion beam assisted deposition. However, in order to understand the mechanical and electronic properties of the DLN film separation of the sp3 carbon bond from sp2 bond is necessary, because the former determines the hardness and the latter is related to the optical gap and electrical conductivity. Here we report the effect of the annealing on the relative concentrations of sp3 and sp2 hybrids in the film by using the XPS technique to decompose the C ls signal into sp3 and sp2 components. The XPS results clearly indicate that the distribution of the sp3 and sp2 C species is modified by thermal annealing at 700 °C. More sp3 species accumulate at the interface and gradually decrease towards the film surface.

Radiation effects on insulators have been extensively studied in terms of radiation induced damage. However, little work has concentrated on in situ surface modification during irradiation and the evolution of surface relaxation after the termination of irradiation. In this work, 1.3 keV electrons and 1.2536 keV Mg Kα X-ray photons were applied to investigate radiation effects on surface structural changes during irradiation and relaxation. The microstructure and composition of lead silicate glass were investigated in situ in an ultra-high-vacuum (UHV) environment by X-ray Photoelectron Spectroscopy (XPS). Similar surface radiation effects were observed using both types of radiation which included permanent metallic lead separation from the glass network resulting from Pb-O bond breaking, bimodal distribution of the 0 is XPS signal due to bridging and non-bridging oxygens, gradual increase of total lead and silicon concentrations and gradual decrease of oxygen concentration during relaxation. Different radiation effects for electron beam and X-ray photons were also observed. For photon irradiation, silicon and lead continuously accumulated while the oxygen on the surface continuously decreased during irradiation and relaxation. In the case of electron irradiation, both silicon and lead were depleted from the surface during irradiation and accumulated during relaxation. Opposite concentration changes were observed for oxygen; it increased during irradiation and decreased during relaxation. The experimental results are discussed in terms of charge effects, secondary electron yield, field-enhanced diffusion, non-bridging oxygens and nucleation of metallic lead.
