Rapid cardiac MRI protocol for cardiac assessment in paediatric and young adult patients undergoing haematopoietic stem cell transplant: a feasibility study

Background:
Haematopoietic stem cell transplantation is an important and effective treatment strategy for many malignancies, marrow failure syndromes, and immunodeficiencies in children, adolescents, and young adults. Despite advances in supportive care, patients undergoing transplant are at increased risk to develop cardiovascular co-morbidities.
Methods:This study was performed as a feasibility study of a rapid cardiac MRI protocol to substitute for echocardiography in the assessment of left ventricular size and function, pericardial effusion, and right ventricular hypertension.
Results:A total of 13 patients were enrolled for the study (age 17.5 ± 7.7 years, 77% male, 77% white). Mean study time was 13.2 ± 5.6 minutes for MRI and 18.8 ± 5.7 minutes for echocardiogram (p = 0.064). Correlation between left ventricular ejection fraction by MRI and echocardiogram was good (ICC 0.76; 95% CI 0.47, 0.92). None of the patients had documented right ventricular hypertension. Patients were given a survey regarding their experiences, with the majority both perceiving that the echocardiogram took longer (7/13) and indicating they would prefer the MRI if given a choice (10/13).
Conclusion:A rapid cardiac MRI protocol was shown feasible to substitute for echocardiogram in the assessment of key factors prior to or in follow-up after haematopoietic stem cell transplantation.

Background
Clozapine is the only antipsychotic drug licensed for treatment-resistant schizophrenia but its use is often delayed. Since previous studies, national guidelines on the use of clozapine and other antipsychotics have been disseminated to clinicians.
AimsTo determine the theoretical delay to clozapine initiation and to quantify the prior use of antipsychotic polypharmacy and high-dose antipsychotic treatment.
MethodClinico-demographic data were extracted from the treatment records of all patients commencing clozapine in our centre between 2006 and 2010.
ResultsComplete records were available for 149 patients. The mean theoretical delay in initiating clozapine was 47.7 months (s.d. = 49.7). Before commencing clozapine, antipsychotic polypharmacy and high-dose treatment was evident in 36.2 and 34.2% of patients respectively. Theoretical delay was related to illness duration (β = 0.7, P<0.001) but did not differ by gender or ethnicity.
ConclusionsSubstantial delays to clozapine initiation remain and antipsychotic polypharmacy and high doses are commonly used prior to clozapine, despite treatment guidelines.

Background
Sexual dysfunction is common in psychotic disorder but it is not clear whether it is intrinsic to the development of the illness or secondary to other factors.
AimsTo compare sexual function in people at ultra-high risk (UHR) of a psychotic disorder, patients with first-episode psychosis predominantly taking antipsychotic drugs and healthy volunteers.
MethodSexual function was assessed in a UHR group (n = 31), a group with first-episode psychosis (n = 37) and a matched control group of healthy volunteers (n = 56) using the Sexual Function Questionnaire.
ResultsThere was a significant effect of group on sexual function (P<0.001). Sexual dysfunction was evident in 50% of the UHR group, 65% of first-episode patients and 21% of controls. Within the UHR group, sexual dysfunction was more marked in those who subsequently developed psychosis than in those who did not. Across all groups the severity of sexual dysfunction was correlated with the severity of psychotic symptoms (P<0.001). Within the first-episode group there was no significant difference in sexual dysfunction between patients taking prolactin-raising v. prolactin-sparing antipsychotics.
ConclusionsSexual dysfunction is present prior to onset of psychosis, suggesting it is intrinsic to the development of illness unlikely to be related to the prolactin-raising properties of antipsychotic medication.

The flute-shaped leaves of the pitcher plant Sarracenia purpurea L. are a fluidfilled microhabitat rich in invertebrate diversity (Bradshaw 1983; Heard 1994; Hardwick and Giberson 1996; Harvey and Miller 1996; Miller et al. 1994). They are the obligate larval habitat for Fletcherimyia fletcheri (Aldrich) (Forsyth and Robertson 1975). Most literature reports of F. fletcheri present information only on larval life history and population dynamics (Fish and Hall 1978; Hardwick and Giberson 1996). Forsyth and Robertson (1975) state anecdotally that they observed adults feeding on flowers and other nectar sources within their study area. We believe that our observations represent the first report of adult F. fletcheri roosting within the flower heads of the pitcher plant.

The Tarantula Survey is an ESO Large Programme which has obtained multi-epoch spectroscopy of over 1,000 massive stars in the 30 Doradus region of the Large Magellanic Cloud. The assembled consortium will exploit these data to address a range of fundamental questions in both stellar and cluster evolution.

INTRODUCTION
In the decade or so since the concept was formalized in landscape ecology (Taylor et al. 1993) the meaning of the term “landscape connectivity” has become rather diffuse and ambiguous. Many researchers continue to ignore key elements of the original concept, which greatly diminishes its potential utility for land management and the conservation of biodiversity. As originally defined, landscape connectivity is “the degree to which the landscape facilitates or impedes movement among resource patches” (Taylor et al. 1993; see also With et al. 1997). This definition emphasizes that the types, amounts, and arrangement of habitat or land use on the landscape influence movement and, ultimately, population dynamics and community structure. Landscape connectivity thus combines a description of the physical structure of the landscape with an organism's response to that structure. In contrast, common usage generally emphasizes only the structural aspect, where landscape connectivity is simply equated with linear features of the landscape that promote dispersal, such as corridors. Moreover, most commonly employed measures of connectivity focus only on how patch area and inter-patch distances affect movement (e.g., Moilanen and Hanski Chapter 3); such measures ignore the rich complexity of how organisms interact with spatial heterogeneity that may ultimately affect dispersal and colonization success (e.g., interactions with patch boundaries, matrix heterogeneity: Wiens et al. 1993; Wiens 1997; Jonsen and Taylor 2000a).
Atypical presentation of ataxia–oculomotor apraxia type 1

A subgroup of autosomal recessive cerebellar ataxias (ARCAs) associated with oculomotor apraxia (OMA) and other variable features has been reported. Ataxia–oculomotor apraxia types 1 and 2 (AOA1 and AOA2) belong to this subgroup and have been described in adults with early onset cerebellar ataxia. AOA1 is associated with oculomotor apraxia, severe sensorimotor neuropathy, choreiform movements, cognitive impairment, and cerebellar atrophy at an early age. We describe a male child with AOA1 who is homozygous for the G837A (W279X) mutation in the APTX gene. He presented at the age of 3 years 6 months with some atypical features including absence of OMA, chorea, and cerebellar atrophy. These manifestations, in addition to peripheral neuropathy, appeared at 8 years of age. We highlight the importance of considering the diagnosis of AOA1 in children with early-onset cerebellar ataxia, once other well-known disorders such as Friedreich's ataxia and ataxia–telangiectasia have been excluded.
