Late diagnosis of CHD and its associated factors in Kenya: an analytic cross-sectional study

Introduction:
Burden of CHD in Africa is generally underestimated mainly due to significant under-reporting and early-related fetal and neonatal mortality.
Objectives:Determine the prevalence and factors associated with late diagnosis of CHD seen at three tertiary care hospitals in Kenya.
Design:A cross-sectional study on paediatric patients with CHDs, aged 0–18 years, seen over a 5-year period, between January, 2011 and December, 2016.
Setting:Aga Khan University Hospital Nairobi, Mater Hospital, and Kenyatta National Hospital.
Methods:Patients were stratified into those diagnosed late (>1 year of age) and those diagnosed early (<1 year of age). Multiple logistic regression analysis was done to determine factors associated with late diagnosis.
Results:The study enrolled 411 patients, with equal gender distribution. Prevalence of late diagnosis (>1 year of age) of CHD was 60.6% (95% CI 55.7–65.3). Median age at diagnosis was 15 (IQR 5–48) months. Presence of a cardiac murmur (OR = 0.87; 95% CI 0.72–0.92, p-value = 0.016) and level of parental education (OR = 4.99; 95% CI 2.25–11.40, p-value <0001) were associated with a decreased odds of late diagnosis. Other factors like cyanosis, an increase in the number of healthcare workers and healthcare facilities per 10,000 population showed some association with decreased odds of late diagnosis of CHD, but these were not statistically significant.
Conclusion:Late diagnosis of CHD remains alarmingly high in our setting. Initiatives to enhance early detection and screening of CHD should be adopted to reduce related mortality and morbidity.

Coronavirus disease 2019 (COVID-19) emerged from a city in China and has now spread as a global pandemic affecting millions of individuals. The causative agent, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is being extensively studied in terms of its genetic epidemiology using genomic approaches. Andhra Pradesh is one of the major states of India with the third-largest number of COVID-19 cases with a limited understanding of its genetic epidemiology. In this study, we have sequenced 293 SARS-CoV-2 genome isolates from Andhra Pradesh with a mean coverage of 13324X. We identified 564 high-quality SARS-CoV-2 variants. A total of 18 variants mapped to reverse transcription polymerase chain reaction primer/probe sites, and four variants are known to be associated with an increase in infectivity. Phylogenetic analysis of the genomes revealed the circulating SARS-CoV-2 in Andhra Pradesh majorly clustered under the clade A2a (20A, 20B and 20C) (94%), whereas 6% fall under the I/A3i clade, a clade previously defined to be present in large numbers in India. To the best of our knowledge, this is the most comprehensive genetic epidemiological analysis performed for the state of Andhra Pradesh.

Indian gooseberry, popularly known as amla, is harvested from two species of Phyllanthus, namely Phyllanthus emblica and Phyllanthus indofischeri. Although these two species are characterized on vegetative and reproductive features, development of reliable and authentic identification based on molecular approaches is necessary for precise identification. Most of the commercial orchards of Indian gooseberry are established with mixture of the two species. Introduction of unauthentic clones and seedlings in the establishment of orchards and differences in the growth of these two species under different elevations are the major constrains in identification. The present work aims to study the changes in vegetative and reproductive features under different elevations and to develop a simple molecular tool for precise identification of these two species based on single nucleotide polymorphisms in trnL (UAA) intron sequences of chloroplast DNA. Genomic DNA of leaf tissues of P. emblica and P. indofischeri, collected from the authentic source, was subjected to polymerase chain reaction amplification using trnL (UAA) intron gene-specific primers. The amplified product with the molecular weight of 540 bp was sequenced and used as reference for identification of these two species. The trnL (UAA) introns sequenced from 60 individual trees in three different orchards were compared with trnL (UAA) intron of authentic samples, and confirmed that these orchards have 76% population of P. indofischeri and remaining 24% is occupied by P. emblica. Thus, trnL (UAA) intron is a potential DNA marker for precise identification of these two species. The importance of the present study and its practical application in genetic improvement of Indian gooseberry are discussed.
