Summary
Bipolar disorder is an affective disorder defined on the basis of the presence of periods of elevated mood. Patients often present with depression, and previous episodes of elevated mood may be missed if not specifically explored during assessment. Bipolar disorder may be difficult to differentiate from other conditions causing mood instability and impulsivity. It is important to identify comorbidities such as substance use, neurodiversity and physical illnesses. The first-line treatment for mania is antipsychotic medication. Antidepressants are reported to have little to no efficacy in treating bipolar depression on average. Lithium is not the only long-term prophylactic agent, but it remains the gold standard, with good evidence that it reduces mood episodes and adverse outcomes. Monitoring is required to ensure lithium level is optimised and potential side-effects minimised.

Lessons Learned From Implementing an Incident Command System During a Local Multiagency Response to a Legionnaires’ Disease Cluster in Sydney, NSW

The incident command system (ICS) provides a common structure to control and coordinate an emergency response, regardless of scale or predicted impact. The lessons learned from the application of an ICS for large infectious disease outbreaks are documented. However, there is scant evidence on the application of an ICS to manage a local multiagency response to a disease cluster with environmental health risks. The Sydney Local Health District Public Health Unit (PHU) in New South Wales, Australia, was notified of 5 cases of Legionnaires’ disease during 2 weeks in May 2016. This unusual incident triggered a multiagency investigation involving an ICS with staff from the PHU, 3 local councils, and the state health department to help prevent any further public health risk. The early and judicious use of ICS enabled a timely and effective response by supporting clear communication lines between the incident controller and field staff. The field team was key in preventing any ongoing public health risk through inspection, sampling, testing, and management of water systems identified to be at-risk for transmission of legionella. Good working relationships between partner agencies and trust in the technical proficiency of environmental health staff aided in the effective management of the response. Introduction
Conventional scientist-executed forms of monitoring are expensive and rely upon highly skilled scientists or technicians (Danielsen et al., 2000). As such only a small amount of this type of monitoring is undertaken in developing countries, where funding and available expertise are limited. This is unfortunate as the developing countries have the greatest importance globally in terms of their assemblages of species, including those threatened by extinction, and also have a great diversity of habitats and important ecosystem services (Millenium Ecosystem Assessment, 2005). They are also the parts of the world where habitat loss is currently proceeding at the greatest speed, and where we know least about the trends in species abundance (Balmford, Green and Jenkins, 2003; IUCN, 2007).
Recently, experiments have been made to involve less educated local people in monitoring of natural resources in developing countries. Although there are still a number of scientific questions surrounding these approaches, and many schemes are still at an early stage of development, the new approaches show a great deal of promise (Danielsen, Burgess and Balmford, 2005a; Danielsen, Burgess and Balmford, 2005c). This chapter analyses the success and challenges of four schemes that stand out from the majority, because they have been replicated and scaled-up, and are now institutionalized and adopted nationally in the respective countries. We begin by describing and explaining the activities and outcomes for each of the four schemes, before presenting our own cross-cutting analysis of the benefits and challenges of such approaches.
Sequence bias in edited kinetoplastid RNAs

Uridylate residues (Us) are inserted and deleted at precise positions in mitochondrial transcripts of Trypanosoma brucei. These sequence changes are determined by interactions with small guide RNAs (gRNAs) that are complementary to edited sequence. Adenylate (A) and guanylate (G) residues in gRNAs across from editing sites pair with inserted Us. We evaluate whether sequence bias exists in the bases surrounding insertion sites. Upon analyzing all reported insertion sites in T. brucei, we find that the predicted base pairs flanking insertion sites show a strong bias. Specifically, guiding As and Gs tend to be flanked by cytosine residues and Us. This bias is expected if precise base-pair interactions at the editing site determine the number of inserted Us.

During RNA editing in kinetoplastid parasites, trans-acting guide RNAs (gRNAs) direct the insertion and deletion of U residues at precise sites in mitochondrial pre-mRNAs. We show here that some modifications to the 3′ terminal ribose of gRNA inhibit its ability to direct in vitro U insertion. However, we found that gRNAs lacking this moiety in some circumstances support in vitro editing. Thus, the 3′ OH is not required. Inhibition resulting from gRNA modification can be overcome by increasing the gRNA–pre-mRNA base-pairing potential upstream of the editing site, suggesting an importance for this interaction to productive processing.
