Differences in the Clinical Outcome of Community-Acquired APN According to the Appropriateness of Antibiotic Use

Background: The purpose of this study was to find out the relationship between appropriateness of antibiotic prescription and clinical outcomes in patients with community-acquired acute pyelonephritis (CA-APN). Methods: A multicenter prospective cohort study was performed in 8 Korean hospitals from September 2017 to August 2018. All hospitalized patients aged ≥19 years diagnosed with CA-APN at admission were recruited. Pregnant women and patients with insufficient data were excluded. In addition, patients with prolonged hospitalization due to medical problems that were not associated with APN treatment were excluded. The appropriateness of empirical and definitive antibiotics was divided into “optimal,” “suboptimal,” and “inappropriate,” and optimal and suboptimal were regarded as appropriate antibiotic use. The standard for the classification of empirical antibiotics was defined reflecting the Korean national guideline for the antibiotic use in urinary tract infection 2018. The standards for the classification of definitive antibiotics were defined according to the result of in vitro susceptibility tests of causative organisms. Clinical outcomes including clinical failure (mortality or recurrence) rate, hospitalization days, and medical costs were compared between patients who were prescribed antibiotics appropriately and those who were prescribed them inappropriately. Results: In total, 397 and 318 patients were eligible for the analysis of the appropriateness of empirical and definitive antibiotics, respectively. Of these, 10 (2.5%) and 18 (5.7%) were inappropriately prescribed empirical and definitive antibiotics, respectively, and 28 (8.8%) were prescribed either empirical or definitive antibiotics inappropriately. Patients who were prescribed empirical antibiotics appropriately showed a lower mortality rate (0 vs 10%; P = .025), shorter hospitalization days (9 vs 12.5 days; P = .014), and lower medical costs (US$2,333 vs US$4,531; P = .007) compared to those who were prescribed empirical antibiotics “inappropriately.” In comparison, we detected no significant differences in clinical outcomes between patients who were prescribed definitive antibiotics appropriately and those who were prescribed definitive antibiotics inappropriately. Patients who were prescribed both empirical and definitive antibiotics appropriately showed a lower clinical failure rate (0.3 vs 7.1%; P = .021) and shorter hospitalization days (9 vs 10.5 days; P = .041) compared to those who were prescribed either empirical or definitive antibiotics inappropriately. Conclusions: Appropriate use of antibiotics leads patients with CA-APN to better clinical outcomes including fewer hospitalization days and lower medical costs.

To evaluate the appropriateness of the screening strategy for healthcare personnel (HCP) during a hospital-associated Middle East Respiratory Syndrome (MERS) outbreak, we performed a serologic investigation in 189 rRT-PCR–negative HCP exposed and assigned to MERS patients. Although 20%–25% of HCP experienced MERS-like symptoms, none of them showed seroconversion by plaque reduction neutralization test (PRNT).

Objective:
Although there are rapidly growing concerns about the high rates of cognitive dysfunction in Korea, the knowledge of risk factors for Alzheimer’s disease (AD) among the general public in Korea remains to be elucidated.
Methods:A total of 2767 randomly selected subjects from the Ansan Geriatric Study were questioned on their knowledge of putative risk factors for AD. Their answers were compared with their sociodemographic data and other variables.
Results:The most common stated risk factor was being older (59.6%), followed by head trauma (33.6%) and cerebrovascular disease (30.4%). However, a substandard education, which is a known risk factor, was considered significant by only 9.5% of the subjects. Predictors for a worse knowledge of the risk factors for AD were being older, a lower level of education, lower economic status and the attitude that dementia is not curable.
Conclusion:This study revealed that misunderstanding about AD is more prevalent in older subjects and those with a lower level of education, and so public health education on the basic concepts of AD should be targeted at this population.

Construction of a new science complex in Osong, Cheongwon-gun, Korea, has allowed the investigation of 14 different Paleolithic localities, excavated during 2005–2007. Here, we investigated localities 1 and 12 of the Mansuri Paleolithic site to obtain chronological information using radiocarbon dating. The soil deposition rates varied from 0.09 to 0.15 mm/yr over the period ranging from 33 to 31 kyr BP for locality 1. Locality 12 samples were more recent, <10 ka, and have similar accumulation rates, averaging 0.11 mm/yr. The soil ages of locality 12 were found to be younger than 10 kyr BP. Results for both soil and organic materials at this locality gave much younger ages at shallower depths than the ages expected by the Korean Paleolithic cultural history for this region. Therefore, these more recent deposits may not be associated with the cultural layers and are interpreted to have been hydrologically modified following emplacement. 14C dates of the soil and organic materials at locality 12 confirm that there is evidence for multiple human occupations throughout the last 9 kyr BP.
Synthesis of Zeolite as Ordered Multi-Crystal Arrays using Uniformly Aligned Polyurethane as Templates

Two types of glass plates coated with uniformly aligned polyurethane films were produced by repeating the cycles of the alternative exposures of the glass plates to 1,4-phenylene diisocyanate (PDI) and 2-butyne-1,4-diol (BDO) and to PDI and terephthalic acid bis-(2-hydroxy ethyl) ester (TBE) for 500 times at elevated temperatures. The glass plates coated with uniformly aligned poly-(PDI-BDO)500 (500 represents the number of the cycle) produced monolayers of closely packed 2D arrays of silicalite-1 crystals with the average size of 370 × 200 × 500 nm on the glass plates upon immersion of the glass plates into a dense gel consisting of tetraethylorthosilicate (TEOS), tetrapropylammonium hydroxide (TPA+OH-), and water (mole ratio = 7:1.5:330) followed by the hydrothermal reaction at 180°C for 2 h. The silicalite-1 crystals were aligned with the c-axes perpendicular to the substrate plane. Upon switching the polymer from poly-(PDI-BDO)500 to poly-(PDI-TBE)500 the orientations of the silicalite-1 crystals in the 2D arrays changed from c to a axes perpendicular to the substrate. This report therefore demonstrates that the uniformly aligned polyurethane films serve as the templates for the growth of closely packed multi-crystal arrays of silicalite-1 in uniform orientations and the nature of the polyurethane film affects the resulting orientations of the crystals. We propose that the supramolecularly organized organic-inorganic composites consisting of the hydrolyzed organic products and the seed crystals responsible for the above phenomena.
The Role of AlN Interlayer in AlxGa1-xN/GaN Heterostructures with high x from 0.35 to 0.50 Grown on Sapphire (0001)

We report the role of thin AlN interlayer in the fabrication of AlxGa1-xN/GaN heterostructures with high x from 0.35 to 0.50 on sapphire (0001) by MOVPE. After growing the AlN interlayer of 20 nm thick on GaN/sapphire (0001) epitaxy, the AlxGa1-xN epitaxial layers of 1.0 um thick were grown on it with increasing the flow rate of TMA. The measured Al mole fractions of AlxGa1-xN/GaN heteroepitaxy grown using the thin AlN interlayer from each TCD rocking curve are 0.35, 0.37, 0.45 and 0.50, respectively. As incorporation rate of Al in the AlxGa1-xN increases, the crystallinity is getting well and the RMS values scanned by AFM of their surfaces become lower. And also, the optoelectronic characteristics of those heteroepitaxy evaluated by cathodeluminescence(CL) are improved with the increase of x. To the contrary these trends, it is usually known that crystallinity, surface morphology and optical property of the AlxGa1-xN/GaN heteroepitaxy grown without AlN interlayer between them become worse with the increase of x above 0.2. Meanwhile, electrical resisitivities of Al0.35Ga0.65N, Al0.37Ga0.63N, Al0.45Ga0.55N and Al0.50Ga0.50N epitaxy, which were grown with the thin AlN interlayer, measured by four point probing method are 13.5, 18.1, 31.7 and 36.2 Mohm-cm, respectively. These resisitivities increase by degrees in spite of the advance of characteristics with the raising of x.
Time-Resolved Photoluminescence Measurements of In0.15Ga0.85N/In0.015Ga0.985N Quantum Wells with Si-doped Barriers

A systematic study of photoluminescence (PL) and time-resolved PL spectra from In0.15Ga0.85N/In0.015Ga0.985N quantum wells (QWs) with different Si doping concentration in the barriers has been carried out. As the Si doping concentration increases, the PL emission intensity was increased and the PL peak energy was blueshifted. The energy separation between the spontaneous emission (SPE) and stimulated emission (SE) peaks decrease with increasing Si doping concentration. We also observed that the slow decay time τ2 in the QWs decreases with increasing Si doping concentration, from ∼ 130 ns for [Si] = 2 × 1018 cm−3 to ∼ 30 ns for [Si] = 1 × 1019 cm−3. The PL emission peak shifts to lower energies with delay time after a pulsed excitation and this shift decreases with increasing Si doping concentration. The increased recombination rate, the decrease of peak shift with delay time, and the reduced separation between the SPE and SE peaks with increasing Si doping concentration are attributed to the screening of piezoelectric field by carriers originated from Si doped barriers.
Dry Etching of GaN Using Reactive Ion Beam Etching and Chemically Assisted Reactive Ion Beam Etching

Dry etching characteristics of GaN using reactive ion beam etching (RIBE) were studied. Etching profile, etching rate and etching selectivity to a photoresist (PR) mask were investigated as a function of various etching parameters. Characteristics of chemically assisted reactive ion beam etching (CARIBE) and RIBE were compared at varied mixtures of CH4 and Cl2. A highly anisotropie etching profile with a smooth surface was obtained for tilted RIBE with Ch at room temperature. Etching selectivity to a PR was dramatically improved in RIBE and CARIBE when a volume fraction of CH4 to the mixture of CH4 and Ch was larger than 0.83.
