Summary
This chapter assesses the political impact of the Sustainable Development Goals on global governance. We start by discussing the range of expectations for global governance arrangements, considering the stated objectives of the goals. We then assess the early performance of governance arrangements in terms of shifts in policy and practice against these expectations. Our research shows the impact of the Sustainable Development Goals is largely discursive, with limited transformative outcomes on governance practices. The High-level Political Forum, created to assess global progress towards the implementation of the goals, has failed to provide political leadership and promote coherence across the United Nations system. Our research also shows that the Sustainable Development Goals initiated peer-learning among governments and other actors, yet with limited evidence that this has led to structural transformation towards sustainability. As certain ambitions of the Global Goals have been part of ongoing debates in global governance, our review finally highlights that observable changes often reflect long-term reform trajectories that are not causally linked to the launch of the goals.

The USA has the world's largest economy and the highest per capita spending on healthcare, but it lags behind other countries on a number of key health measures. It ranks 23rd in healthy life expectancy and 32nd in infant mortality (World Health Organization, 2009). In 2000, the World Health Organization ranked the US healthcare system as 1st in responsiveness, 37th in overall performance, and 72nd by overall level of health (among 191 member nations in the study).
Mental health in the USA
Approximately 25% of US adults have a diagnosable mental disorder in a given year and approximately 6% have a serious mental illness. Mental disorders are the leading cause of disability for people aged 15–44 years (National Institute of Mental Health, 2010).
About 11% of adults experience serious psychological distress, such as anxiety and mood disorders, that result in functional impairment that impedes one or more major life activities. Rates of mental illness are highest for adults aged 18–25 years and lowest for those over 50; rates for women are significantly higher than for men. The most common mental illnesses are anxiety and mood disorders (Substance Abuse and Mental Health Services Administration, 2009).
Some 17% of inmates entering jails and prisons have a serious mental illness (which is nearly three times the rate in the general population) (Steadman et al, 2009). As many as 70% of those in the juvenile justice system have a diagnosable mental disorder and one in five has a mental disorder significant enough to impair functioning (Skowyra & Cocozza, 2006).
Unfortunately, the high rate of mental illness does not correlate with adequate treatment. Fewer than half of adults with a diagnosable mental disorder receive treatment in a given year (Kessler et al, 2005). The number of Americans under care for mental illnesses nearly doubled between 1996 and 2006 (from 19 to 36 million) (Agency for Healthcare Research and Quality, 2009). Among those with serious mental illnesses, adults aged over 50 were more likely to use mental health services (71%) than adults aged 18–25 (40%) (Substance Abuse and Mental Health Services Administration, 2009).

Damage in DNA of an infecting phage T4 shifts reproduction from asexual to sexual allowing rescue of its genes

The selective advantage of sexual reproduction is widely regarded as a major unsolved problem in biology. Recently, it has been proposed that the fundamental selective advantage of sex is its promotion of recombinational repair and hence survival of DNA in the germ line of organisms. A bacteriophage T4 system was set up to test this theory. After a phage T4 injects its DNA into an Escherichia coli cell it quickly establishes a barrier, through its immunity function, to infection by a second phage T4, arriving at a later time. This barrier causes phage T4 to reproduce asexually. The immunity barrier has a selective advantage in preserving the host cell as a sole resource for the first phage. If the first phage's DNA is damaged by UV irradiation, however, it has a reduced probability of being able to survive in the host cell when it is there alone. It was found that UV irradiation, in addition to reducing the first phage's viability, also prevents the first phage from raising an effective immunity barrier. This UV-induced reduction in immunity now allows sexual interaction with later-arriving secondary phage or sexual reproduction. It was found that these secondary phage enhanced survival of genes of the UV-damaged first phage. This supports the theory that under DNA-damaging conditions, which should be prevalent in nature, sex would have a selective advantage.
