Routine use of salivary bypass tubes in laryngectomy patients: systematic review

Background:
Pharyngocutaneous fistula is a cause of significant morbidity following laryngectomy. Routine use of salivary bypass tubes during laryngectomy has been proposed to reduce the incidence of fistulae and neopharyngeal strictures.
Method:Following a systematic search of Embase, Medline and Cochrane databases (1946 – current), included articles were assessed for bias according to the Cochrane Handbook for Systematic Reviews of Interventions.
Results:Three case–control trials showed reduced pharyngocutaneous fistula rates with the use of salivary bypass tubes; six case series reported widely varied fistula rates. With regards to stricture rates, the largest case–control trial found no improvement with salivary bypass tube use. No fatal adverse events were observed among the 204 patients who received a salivary bypass tube.
Conclusion:Low-level evidence suggests salivary bypass tubes may reduce the incidence of fistula in high-risk patient groups. A robust randomised controlled trial, or large, multicentre cohort studies, are needed to further examine this intervention.

The use of a land suitability model to predict where autumn-sown, determinate genotypes of the white lupin (Lupinus albus) might be grown in England and Wales

A model was developed to assess the suitability of land in England and Wales for growing newly developed genotypes of autumn-sown determinate white lupins. The model used soil pH, the number of degree-days accumulated for mainstem leaf production before the apical meristem of the mainstem became floral, and the number of machinery work days in autumn. Interactions between these three components were used to set thresholds to determine land suitability within 5 × 5 km grid squares of the National Soil Map.
Of the potential 13·75 Mha of arable land in England and Wales, a total of 7·54 Mha are well or moderately suited to growing these lupin genotypes. This is equivalent to c. 2 Mha of land within the arable rotation each year. It was estimated that, because of low soil pH, lupins would be the preferred legume on 0·3 Mha out of this 2 Mha. The model was also used to assess the risk of soil acidification and nitrate leaching following mineralization of lupin residues. This exercise indicated that there was little risk of either on much of the land suited to lupins.

Ruminant products are considered as a major source of saturated fatty acids (SFA) in the human diet and a reduction in the intake of SFA along with a concomitant increase in the intake of n-3 series PUFA is recommended by nutritionists (Department of Health, 1994). The major fatty acid classes in beef are the saturated fatty acids (SFA), monounsaturated fatty acids (MUFA) and polyunsaturated fatty acids (PUFA) and beef is a nutritionally important source of the beneficial n-3 series PUFA. Experiments investigating the effects of age on lipid composition in beef muscle have, in the main, used short time periods and also been subject to confounding effects of differences in growth rate (Rule et al., 1997). This study is part of a larger investigation into the effects of breed and diet, as well as age, on muscle lipids (Warren et al., 2003). This paper will focus on the effect of age.

The following paper gives the results of the first of a series of researches designed to investigate the composition and properties of normal milk.
Although it is realised that a complete study of lactation can only be made when physiological technique is available to follow the formation of milk to its natural source, yet the marked differences in reaction which had been observed in studying the pH of individual quarters by the changes in colour produced when the milk was tested with brom cresol purple paper, suggested the possibility of similar variations in other properties. It was, therefore, decided to make a comparison of the composition and physical properties of milk as it leaves the individual teats of a healthy cow.
