Prediction of depression treatment outcome from multimodal data: a CAN-BIND-1 report

Background
Prediction of treatment outcomes is a key step in improving the treatment of major depressive disorder (MDD). The Canadian Biomarker Integration Network in Depression (CAN-BIND) aims to predict antidepressant treatment outcomes through analyses of clinical assessment, neuroimaging, and blood biomarkers.
MethodsIn the CAN-BIND-1 dataset of 192 adults with MDD and outcomes of treatment with escitalopram, we applied machine learning models in a nested cross-validation framework. Across 210 analyses, we examined combinations of predictive variables from three modalities, measured at baseline and after 2 weeks of treatment, and five machine learning methods with and without feature selection. To optimize the predictors-to-observations ratio, we followed a tiered approach with 134 and 1152 variables in tier 1 and tier 2 respectively.
ResultsA combination of baseline tier 1 clinical, neuroimaging, and molecular variables predicted response with a mean balanced accuracy of 0.57 (best model mean 0.62) compared to 0.54 (best model mean 0.61) in single modality models. Adding week 2 predictors improved the prediction of response to a mean balanced accuracy of 0.59 (best model mean 0.66). Adding tier 2 features did not improve prediction.
ConclusionsA combination of clinical, neuroimaging, and molecular data improves the prediction of treatment outcomes over single modality measurement. The addition of measurements from the early stages of treatment adds precision. Present results are limited by lack of external validation. To achieve clinically meaningful prediction, the multimodal measurement should be scaled up to larger samples and the robustness of prediction tested in an external validation dataset.

Nguyễn Công Trứ, poet and songwriter, was an official at the Vietnamese court in the early nineteenth century who gained acclaim for settling landless peasants on abandoned land. This essay recounts and analyses his family background and the early part of his public career. It contrasts his initiatives in the countryside with criticism of them by officials at the royal court and examines his first major demotion in 1831. This study encompasses the contrasting career of Hoàng Quýnh, the official whose accusation caused Nguyễn Công Trứ's demotion. From this we gain some understanding of how King Minh Mạng maintained control of the royal court, through a system of promotions and demotions, amidst regional tensions and personality conflicts.

Summary
Southeast Asia can be visualized as the part of Asia that spills into the sea, comprised of long coasts, tidal plains, peninsulas, and islands. There are high mountains, inland plains, plateaus, and upland valleys; nonetheless, to a very large degree, human culture has developed with an acute awareness of water, from the sea, the rivers, and the monsoon rains. It is therefore not surprising that many of the endemic health problems in the region are related to water; indeed, since prehistoric times, nearly all major areas of habitation have been exposed to global contact by water transport.
Maritime routes linking the littoral civilizations of the Eurasian landmass have passed through Southeast Asia for more than two millennia. We can accordingly assume that from early times the region experienced all of the epidemic diseases familiar to the ancient world. What inhibits discussion of diseases in the earlier historical periods of Southeast Asia is the lack of data. Because of the prevailing tropical-equatorial climate, the preservation of written records has, until recently, required greater effort than most human societies were prepared to make. Our first information comes from the observations of Chinese annalists, whose works survived in the temperate climate of northern China. As the Chinese moved southward into what is today northern Vietnam, they recorded perceptions of disease associated with what for them were southern lands.
Most prominent among the health problems encountered by ancient Chinese armies in Vietnam were malaria and other “fevers” associated with the monsoon rain season. Chinese generals timed their expeditions into Vietnam to coincide with the dry season, from November to May.

Summary
An approach to the Lý dynasty
The conventional view of the Lý dynasty has been that it established a centralized state power in Vietnam during the course of the eleventh century. Whether arguing from the historical, bureaucratic model of a strong Chinese dynasty[1] or the theoretical, economic model of the Asiatic Mode of Production,[2] scholars in and out of Vietnam have favoured the idea that, beginning with the Lý, dynastic power in Vietnam was expressed as a centralized state that wielded effective administrative control over the different territories of the realm.
This view has in recent years been questioned by two Japanese scholars. Minoru Katakura, who has surveyed legal codes and their application in Vietnam during different historical eras, did not directly challenge the idea of a centralized political power, but he introduced fresh insights that stimulate doubt. In 1973, he used the term “centralized feudal system”[3] he believed that the distinctive characteristics of the Lý penal code (its relative “tolerance” or leniency) was related to “the power structure of the dynasty”, and “the political power basis of the Lý dynasty was founded on its acceptance of the traditions and the independence of villages”.[4] In 1977, he noted that the court chronicles record frequent appearances of dragons during the Lý dynasty and that the Lý kings often presided over traditional religious rites such as oaths and covenants or prayers for rain; he went on to surmise that the Ly family tried to establish imperialistic rule over rival clans by transforming traditional Vietnamese customs into national religious rites.[5] Yumio Sakurai interpreted Katakura's view of the Lý dynasty as a shamanistic, religious dynasty rather than a state based on laws.[6]
In 1980, Yumio Sakurai published his study on patterns of settlement and agriculture during the Lý dynasty, one in a series covering several epochs of Vietnamese history.[7] He based his work on a thorough search of historical sources and a detailed knowledge of premodern agrarian economy. He argues that the Lý dynasty was a “local dynasty”, meaning that it directly controlled only one of several political-economic regions in the plains of northern Vietnam.[8] He believes that the different regional powers that appeared during the tenth century were not completely suppressed until the thirteenth century by the Tran dynasty.
