  • Anorexia Nervosa and other eating disorders are arguably the most complex mental health problems that a child or adolescent may experience. Numbers seeking help are on the increase, and the complexity of these disorders challenges even the most experienced clinician.   • Summary

    This chapter describes the range of eating problems seen in middle childhood (latency) and early adolescence (prepubertal or early puberty) that do not fit within 'eating disorders' as a diagnostic category. One of many developmental tasks of childhood and early adolescence is managing the transition from feeding to eating. Over a period of years children with selective eating develop an avoidance reinforced anxiety associated with new foods. There may be anticipatory nausea (with sight or smell triggers), fear of vomiting (textures) or a fear of choking. The term food avoidance emotional disorder (FAED) has mainly been used to describe cases presenting in middle childhood and early adolescence in which age group it may be more common than anorexia nervosa or bulimia nervosa. In some children, food phobias may be a feature of more pervasive anxiety disorder or obsessive-compulsive disorder (OCD).

    Little is known about the current state of provision of child and adolescent mental health service in-patient units in the UK.

    Aims

    To describe the full number, distribution and key characteristics of child and adolescent psychiatric in-patient units in England and Wales.

    Method

    Following identification of units, data were collected by a postal general survey with telephone follow-up.

    Results

    Eighty units were identified; these provided 900 beds, of which 244 (27%) were managed by the independent sector. Units are unevenly distributed, with a concentration of beds in London and the south-east of England. The independent sector, which manages a high proportion of specialist services and eating disorder units in particular, accentuates this uneven distribution. Nearly two-thirds of units reported that they would not accept emergency admissions.

    Conclusions

    A national approach is needed to the planning and commissioning of this specialist service.

  • HoNOSCA: Is the Enthusiasm Justified?

  • I have what I consider to be a healthy scepticism regarding new assessment instruments. Many of these are announced with great enthusiasm but fairly rapidly disappear or are used mainly by those who have developed them and by their associates.

    Although the tone of the paper by Gowers et al. (2000) seems rather strong on enthusiasm and perhaps a little lacking in its critical view, I do have great respect for colleagues associated with development of HoNOSCA and other respected colleagues talk well of it. Clearly, it deserves careful scrutiny. For me relevant questions include: ‘Is it easy to use?’, ‘Is it useful in terms of describing a patient population?’ and ‘Is it useful in looking at outcome?’

    The way in which psychiatric services for adolescents in the UK are developing will be affected by recent changes in the organisation of the National Health Service.

    Method

    The history of these services, and the different opportunities for development are reviewed.

    Results

    Ways in which high-quality clinical care can still be provided are indicated.

    Conclusion

    Adolescent psychiatric services should be judged on their ability to provide such care, not merely on their ability to survive.
