Hemodialysis Clinics in Flood Zones: A Case Study of Hurricane Harvey

Introduction:
Hurricane Harvey (2017) forced the closure of hemodialysis centers across Harris County, Texas (USA) disrupting the provision of dialysis services. This study aims to estimate the percentage of hemodialysis clinics flooded after Harvey, to identify the proportion of such clinics located in high-risk flood zones, and to assess the sensitivity of the Federal Emergency Management Agency (FEMA) Flood Insurance Rate Maps (FIRMs) for estimation of flood risk.
Methods:Data on 124 hemodialysis clinics in Harris County were extracted from Medicare.gov and geocoded using ArcGIS Online. The FIRMs were overlaid to identify the flood zone designation of each hemodialysis clinic.
Results:Twenty-one percent (26 of 124) of hemodialysis clinics in Harris County flooded after Harvey. Of the flooded clinics, 57.7% were in a high-risk flood zone, 30.8% were within 1km of a high-risk flood zone, and 11.5% were not in or near a high-risk flood zone. The FIRMs had a sensitivity of 58%, misidentifying 42% (11 of 26) of the clinics flooded.
Conclusion:Hurricanes are associated with severe disruptions of medical services, including hemodialysis. With one-quarter of Harris County in the 100-year floodplain, projected increases in the frequency and severity of disasters, and inadequate updates of flood zone designation maps, the implementation of new regulations that address the development of hemodialysis facilities in high-risk flood areas should be considered.
Effect of Tornado Outbreaks on Morbidity and Mortality in Texas

Introduction:
In the United States, tornadoes are the third leading cause of fatalities from natural disasters1. To aid prevention and mitigation of tornado-related morbidity and mortality, improvement in standardizing tornado specific threat analysis terminology was assessed. The largest number of tornado-related fatalities has occurred in the state of Texas for over a hundred years. The occurrence of tornadic clusters or “outbreaks” has not been formally standardized. The concept of “tornado outbreaks” is better defined and its role in fatality mitigation is addressed in this Institutional Review Board (IRB) approved study.
Aim:To understand the role of “tornado outbreaks” related clusters in Texas in relationship to morbidity and mortality.
Methods:This IRB approved (IRB2017- 0507) research study utilized GIS tools and statistical analysis of historical data to examine the relationship between tornado severity (based on the Fujita Scale), the number of tornadoes, and the trends in morbidity and mortality. This study was funded in part from The National Science Foundation grant (NSF Grant #1560106) in support of the CyberHealthGIS Research Experience for Undergraduates (REU).
Results:A statistically significant difference was demonstrated between the severity of a tornado and related morbidity and mortality during “tornado outbreaks” in Texas during a defined 30-year period.
Discussion:Understanding the role and discerning the impacts of “tornado outbreaks” as related to tornado severity has critical implications to disaster preparedness. Applications of this conclusion may improve shelter planning/preparation, timely warning, and educating the at-risk public. Subsequently, examining the likelihood and improved descriptions of “tornado outbreaks” may aid in reducing the number of tornado-related injuries and fatalities nationally.

Mechanix: A natural sketch interface tool for teaching truss analysis and free-body diagrams

Massive open online courses, online tutoring systems, and other computer homework systems are rapidly changing engineering education by providing increased student feedback and capitalizing upon online systems' scalability. While online homework systems provide great benefits, a growing concern among engineering educators is that students are losing both the critical art of sketching and the ability to take a real system and reduce it to an accurate but simplified free-body diagram (FBD). For example, some online systems allow the drag and drop of forces onto FBDs, but they do not allow the user to sketch the FBDs, which is a vital part of the learning process. In this paper, we discuss Mechanix, a sketch recognition tool that provides an efficient means for engineering students to learn how to draw truss FBDs and solve truss problems. The system allows students to sketch FBDs into a tablet computer or by using a mouse and a standard computer monitor. Using artificial intelligence, Mechanix can determine not only the component shapes and features of the diagram but also the relationships between those shapes and features. Because Mechanix is domain specific, it can use those relationships to determine not only whether a student's work is correct but also why it is incorrect. Mechanix is then able to provide immediate, constructive feedback to students without providing final answers. Within this manuscript, we document the inner workings of Mechanix, including the artificial intelligence behind the scenes, and present studies of the effects on student learning. The evaluations have shown that Mechanix is as effective as paper-and-pencil-based homework for teaching method of joints truss analysis; focus groups with students who used the program have revealed that they believe Mechanix enhances their learning and that they are highly engaged while using it.
