Summary
Mobile devices (e.g., smartphones and tablets) have become a commodity in our daily lives. While these devices already support many different types of applications and services, there will be a continual increase in demand for mobile data traffic due to web applications, real-time and streaming video traffic, and applications related to the Internet of Things (IoT). The future fifth generation (5G) wireless cellular systems aim not only to provide a higher aggregate throughput, but also to support applications which have stringent quality of service (QoS) requirements, such as seamless mobility, ultra-low latency (e.g., Tactile Internet), and high reliability (e.g., vehicular communications). Further improvements in spectrum efficiency, energy efficiency, and cost per bit are also important. In order to meet these demands, fundamental changes to the network architecture and all layers of the protocol stack compared with fourth generation (4G) wireless systems are needed.
This book aims to provide a comprehensive treatment of the ongoing research into and state-of-the-art techniques for addressing the challenges arising from the design of 5G wireless systems. Written by leading experts on the subject, this book includes 22 chapters, which cover various aspects of 5G systems, including network architecture design, physical layer techniques, algorithms, and network protocol design. Chapter 1 serves as an introductory chapter and provides an overview of the different key technologies related to 5G systems. Each of the other chapters tackles one specific challenge for system design. The chapters can be read independently.
This book will be of interest to a readership from the communications, signal processing, and networking communities. The primary audience for this book is researchers and engineers who are interested in studying advanced communication and networking techniques, as well as state-of-the-art research on 5G systems. This book will serve as a resource for self-study and as a reference book for researchers and engineers involved in the design of wireless communication systems. It is also suitable for graduate students who are interested in 5G systems and the related communication and networking issues. It may serve as a reference book for graduate-level courses for students in electrical engineering, communication engineering, and networking.

Summary
Introduction
In recent years, wireless service providers in different countries have deployed both the Third Generation Partnership Project (3GPP) Long Term Evolution (LTE) and LTE-Advanced systems. Despite the unprecedented data rates and quality of service (QoS) provided by these new networks, user demand is beginning to exceed their capabilities. For example, the proliferation of smartphones and tablets has caused a significant and sustained increase in mobile data traffic. In fact, in 2015 alone, global mobile data traffic grew by 74% from 2.1 to 3.7 exabytes [1]. Furthermore, the existing networks are not well suited for the exceedingly large number of devices and appliances that are expected to be connected wirelessly to the Internet in future Internet of Things (IoT) applications and machine-to-machine (M2M) communications. Moreover, to make the growth of networks and the number of connected devices economically and ecologically sustainable, energy efficiency has to be substantially improved. Also, emerging new applications such as remote surgery in healthcare, autonomous driving, and wireless control of industrial robots require ultra-low latencies in the sub-millisecond range and ultra-high reliability, giving rise to the notion of the Tactile Internet. In order to support the exponential growth of existing mobile traffic and the emergence of new wireless applications and services, researchers and standardization bodies worldwide have set out to develop a fifth generation (5G) of wireless networks [2–6]. Some of the stringent requirements for this next generation of wireless networks are listed in Table 1.1 [7].
To meet these challenging requirements, a mere evolution of the current networks is not sufficient. Instead, a true revolution of technologies in both the radio access network and the mobile core network is needed (Figure 1.1). In the radio access network, fundamentally new physical layer technologies such as massive multiple-input multiple-output (MIMO), non-orthogonal multiple access (NOMA), full-duplex (FD) communication, millimeter wave (mmWave) communication, device-to-device (D2D) communication, and visible light communication (VLC) will be deployed. Furthermore, leveraging cloud computing, the cloud radio access network (C-RAN) has emerged as a promising and cost-efficient mobile network architecture to enhance the spectrum and energy efficiency of 5G networks. In addition, different access technologies, including LTE and Wireless Fidelity (Wi-Fi), may be integrated to guarantee seamless coverage, and to support high data-rate transmission and data offloading.

OBJECTIVE
To assess the effectiveness of infection control preparedness for human infection with influenza A H7N9 in Hong Kong.
DESIGNA descriptive study of responses to the emergence of influenza A H7N9.
SETTINGA university-affiliated teaching hospital.
PARTICIPANTSHealthcare workers (HCWs) with unprotected exposure (not wearing N95 respirator during aerosol-generating procedure) to a patient with influenza A H7N9.
METHODSA bundle approach including active and enhanced surveillance, early airborne infection isolation, rapid molecular diagnostic testing, and extensive contact tracing for HCWs with unprotected exposure was implemented. Seventy HCWs with unprotected exposure to an index case were interviewed especially regarding their patient care activities.
RESULTSFrom April 1, 2013, through May 31, 2014, a total of 126 (0.08%) of 163,456 admitted patients were tested for the H7 gene by reverse transcription-polymerase chain reaction per protocol. Two confirmed cases were identified. Seventy (53.8%) of 130 HCWs had unprotected exposure to an index case, whereas 41 (58.6%) and 58 (82.9%) of 70 HCWs wore surgical masks and practiced hand hygiene after patient care, respectively. Sixteen (22.9%) of 70 HCWs were involved in high-risk patient contacts. More HCWs with high-risk patient contacts received oseltamivir prophylaxis (P=0.088) and significantly more had paired sera collected for H7 antibody testing (P<0.001). Ten (14.3%) of 70 HCWs developed influenza-like illness during medical surveillance, but none had positive results by reverse transcription-polymerase chain reaction. Paired sera was available from 33 of 70 HCWs with unprotected exposure, and none showed seroconversion against H7N9.
CONCLUSIONSDespite the delay in airborne precautions implementation, no patient-to-HCW transmission of influenza A H7N9 was demonstrated.

Summary
Introduction
Demand-side management (DSM) is one of the key components of the future smart grid to enable more efficient and reliable grid operation [1]. To achieve a high level of reliability and robustness in power systems, the grid is usually designed for peak demand rather than for average demand. This usually results in an under-utilized system. To remedy this problem, different programs have been proposed to shape the daily energy consumption pattern of the users in order to reduce the peak-to-average ratio in load demand and use the available generating capacity more efficiently, avoiding the installation of new generation and transmission infrastructures. However, the increasing expectations of the customers both in quantity and quality [2], emerging new types of demand such as plug-in hybrid electric vehicles (PHEVs), which can potentially double the average household energy consumption [3], the limited energy resources, and the lengthy and expensive process of exploiting new resources give rise to the need for developing some more advanced methods for DSM.
Since electricity cannot be stored economically, wholesale prices (i.e., prices set by competing generators to regional electricity retailers) vary drastically between the low-demand times of day and the high-demand periods. However, these changes are usually hidden from retail users. That is, end users are usually charged with some average price. To alleviate this problem, various time-differentiated pricing methods have been proposed in the literature. Some examples include day-ahead pricing, time-of-use pricing, critical-peak-load pricing, and adaptive pricing [4–7].
By equipping users with two-way communication capabilities in smart grid systems and by adopting real-time pricing (RTP) methods, it is possible to reflect the fluctuations of wholesale prices to retail prices.
Prevention of Nosocomial Transmission of Norovirus by Strategic Infection Control Measures

Background.
Nosocomial outbreaks of norovirus infection pose a great challenge to the infection control team.
Methods.Between November 1, 2009, and February 28, 2010, strategic infection control measures were implemented in a hospital network. In addition to timely staff education and promotion of directly observed hand hygiene, reverse-transcription polymerase chain reaction for norovirus was performed as an added test by the microbiology laboratory for all fecal specimens irrespective of the request for testing. Laboratory-confirmed cases were followed up by the infection control team for timely intervention. The incidence of hospital-acquired norovirus infection per 1,000 potentially infectious patient-days was compared with the corresponding period in the preceding 12 months, and the incidence in the other 6 hospital networks in Hong Kong was chosen as the concurrent control. Phylogenetic analysis of norovirus isolates was performed.
Results.Of the 988 patients who were tested, 242 (25%) were positive for norovirus; 114 (47%) of those 242 patients had norovirus detected by our added test. Compared with the corresponding period in the preceding 12 months, the incidence of hospital-acquired norovirus infection decreased from 131 to 16 cases per 1,000 potentially infectious patient-days (P < .001 ), although the number of hospital-acquired infections was low in both the study period (n = 8) and the historical control periods (n = 11). The incidence of hospital-acquired norovirus infection in our hospital network (0.03 cases per 1,000 patient-days) was significantly lower than that of the concurrent control (0.06 cases per 1,000 patient-days) (P = .015). Forty-three (93%) of 46 norovirus isolates sequenced belonged to the genogroup II.4 variant.
Conclusions.Strategic infection control measures with an added test maybe useful in controlling nosocomial transmission of norovirus.
