Rates and methods of suicide during 1961–70 of migrants in Australia were compared with native born Australians. Large differences in both rates and methods of suicide were found in individual migrant groups. The rates were more akin to those in the country of origin than of the Australian-born, no matter how long the migrants had lived in Australia. By contrast, methods of suicide used by migrants approached more closely those of the Australian-born the longer they had lived in Australia. The change in methods used was greatest in those migrant groups coming from countries with a language and culture more akin to those in Australia. A discussion follows of various possible sociological and cultural factors, both within the countries of origin and within Australia, which could have contributed to these results.

Thirty-four women who had vaginal deliveries of live infants completed behavioural ratings and supplied blood and 24-hour urine samples on three occasions during pregnancy and on three occasions after delivery. Approximately one third of the women indicated a distinct upswing in mood between Days 2 and 4 following delivery. These ‘positive mood change’ subjects showed changes in urinary cyclic AMP (adenosine 3′5′ cyclic monophosphate), plasma cyclic AMP, whole blood cell cyclic AMP, whole blood cell ATP (adenosine triphosphate), haematocrit and urinary 11 OHCS (11-hydroxycortisol steroids) following delivery which were different from those observed in the rest of the subjects and comparable with the biochemical changes described during upswings in mood in short-cycle manic-depressive subjects. There was also an indication that the women showing this upswing in mood following delivery were distinct in some respects on both behavioural ratings and biochemical findings during pregnancy.

In a 2-year study of conjugal bereavement, 26 of 99 women remained highly distressed throughout the course of the study. Of the remaining 73 women, 30 never gave evidence of ‘high distress’. Deficits in social support, health and financial problems correlate with enduring ‘high distress’. Personality traits consonant with a socially acceptable and expected ‘widow role’ were found to differentiate the women with enduring ‘low distress’.

This study investigates the validity of the suggestion that food allergy is relevant to the manifestation of schizophrenic illness. Antibodies to oats, wheat, gluten, chicken, calf and milk protein were measured in 98 schizophrenics and in 90 unaffected close blood relatives. The majority of patients were male out-patients, and most were on neuroleptic medication. Antibodies were detected in only a minority of patients.

The Southampton case register and maternity records were used to identify all Southampton women who had babies in 1976 and 1977 and who had been in psychiatric care in the 2 years before and after the birth date. 127 such women were identified out of a total of 5200 having babies. An analysis of the timing of referral and the total number in care at various stages of the study period confirmed an increased rate of psychosis following childbirth. Rates for other diagnoses showed no such clear relationship. Obstetric and social factors were not related to postpartum morbidity. These results are discussed.

Disturbed behaviour and dissatisfaction were studied in relation to two contrasting ward environments, one representing the ‘medical model’ of psychiatry and the other being a modified form of ‘therapeutic community’. Specially designed forms were used for 18 months to obtain daily and weekly measures of behavioural disturbance from the nurses and of dissatisfaction with various aspects of the ward environment from the patients. The results suggested that the ‘therapeutic community’ ward was significantly more disturbed, and that disturbance on that ward varied according to the day of the week, something which did not happen on the ‘medical model’ ward. Differences in disturbance between diagnoses were also found. Disturbance was significantly correlated with, and tended to precede, dissatisfaction only in the ‘therapeutic community’ ward.

Acute schizophrenics were compared with depressed, normal and brain-damaged subjects on their ability to appreciate a meaningful picture. Their responses were measured for level of abstraction (how well the theme was conveyed), for strategy (whether details were reported before or after a global interpretation) and for appropriateness of content. Schizophrenics were less abstract than normals, more abstract than brain-damaged subjects, but no different from depressives on this measure. Their strategy was different from normals but comparable with that of depressives. The majority of schizophrenics gave inappropriate responses. It is suggested that meaningful picture interpretation might be a useful tool for evaluating the distortion of meaning which characterizes schizophrenia.

Two age cohorts of young people were interviewed at times up to 2½ years after leaving school to investigate the association between unemployment and risk of minor psychiatric morbidity, as assessed by the General Health Questionnaire (GHQ), taking into account differences due to sex, ethnic group and educational qualifications. Cross-sectional data are presented on 2767 interviews. The psychometric properties of the GHQ justified the use of a total score. Although females had a higher risk of psychiatric morbidity, the strongest association was between unemployment and GHQ. The association between unemployment and GHQ scores was shown to be present after controlling for sex, ethnic group and educational qualification differences. Longitudinal analyses showed that the experience of unemployment was more likely to create increased symptoms, rather than the reverse.

After the journalists of a national newspaper had been issued with redundancy notices, a prospective study was carried Out to examine the effect of the threat of their redundancy on minor psychiatric morbidity. Following the subsequent withdrawal of redundancy notices, there was considerable reduction in minor symptomatology, and the implications of this are discussed.

The effect of childhood experiences on adult psychiatric morbidity was examined in a community psychiatric survey. The Present State Examination was used to assess psychiatric morbidity. Childhood experiences assessed included childhood demographic factors and ‘loss and deprivation’ variables. The latter group comprised maternal and paternal deaths and separations and other disruptions in parental care. ‘Loss and deprivation’ in combination accounted for between 4·5 and 5·5% of the variance in adult psychiatric morbidity

An increasing interest in the psychological concomitants of neoplastic diseases has led to a burgeoning literature in recent years. The present review identifies five major areas of psychiatric research, documents the current state of knowledge, and indicates fruitful avenues for further study.

Thirty patients with cycloid psychosis were found among 244 general psychotic and schizo-affective patients studied in London. The main clues to the diagnosis were the presence of ‘confusion’, a pleomorphic clinical picture or an acute onset. Most of the patients were classified as schizophrenic by British psychiatrists and the Catego system, and as schizo-affective or mood-incongruent affective psychotics by the American Research Diagnostic Criteria and DSM-III. There was little overlap between the cycloids and any Anglo-American category, and cycloid psychosis is not synonymous with schizo-affective psychosis. The outcome of the cycloids was better than that of psychotic patients as a whole, and much better than schizophrenia as defined by Catego, Schneider's, Langfeldt's or Carpenter's rules, or by the guidelines set by the International Classification of Diseases. Compared with manic-depressive patients (defined by the presence of mania at some stage), cycloids had more schizophrenic and fewer depressive and manic symptoms. There was a negligible concordance between the diagnosis of cycloid psychosis and the final diagnosis of manic-depressive disease. It is concluded that these patients should not be diagnosed as schizophrenic, but that the relation of cycloid psychosis to manic-depressive disease is not yet resolved.

Emotional distress as assessed by the self-report Hopkins Symptom Checklist (HSCL) was examined in a sample of 607 urban black high school students aged 15–18. These adolescents reported high distress primarily about feelings of disadvantage, volatile anger, interpersonal sensitivity and loneliness. Females were significantly more likely than males to indicate emotional distress, although several of the leading distress items were endorsed equally by both groups. Male and female scores across the HSCL factors differed in degree rather than form. The great majority of these adolescents did not report significant emotional distress. These data provide a base from a non-clinical sample for comparison with other adolescent groups where emotional distress may play a role.

Seven cases of anorexia nervosa associated with a brief psychotic episode are described. The psychoses all resolved completely and were similar in their atypical form. In 5 cases an organic cause could not be established, but psychological stress may have been an aetiological factor.

Skin conductance habituation was measured in schizophrenic patients (N = 41) during an acute episode and compared with neurotic patients (N = 86) who were either predominantly anxious or predominantly depressed. All patients were tested before the onset of drug treatment. Between the 14th and 15th tone of the 17 tone simple habituation series a loud ‘dishabituating’ tone was inserted.

In terms of traditional measures based on response frequency, non-habituation was most frequent in the anxious patients and least frequent in the depressed patients, with schizophrenic patients being intermediate. However, in terms of response amplitude the schizophrenic patients showed significantly faster habituation than either neurotic group. This paradox seems to be a consequence of the frequency of spontaneous fluctuations which was highly related to habituation status in the schizophrenic group, but not in the neurotic groups. When habituation was defined in terms of response amplitudes falling below spontaneous fluctuation amplitudes (rather than zero) the schizophrenic group showed the lowest frequency of non-habituation.

None of the groups showed any evidence of dishabituation after the loud tone, but the schizophrenics showed a significantly smaller response to the loud tone itself. Following Groves & Thompson (1970), this is interpreted as showing that the loud tone induced a greater response because of its perceived difference from the other tones rather than a sensitization effect because of its loudness. In these terms the schizophrenic patients showed greater response generalization than the neurotic patients.

A procedural replication of the Camberwell retrospective community study of depression in women (Brown et al. 1975; Brown & Harris, 1978 a) was conducted in Calgary, Alberta. A random sample of 449 women between the ages of 18 and 65 were interviewed. The shorter form of the Present State Examination (PSE) and Brown's Interview Schedules for Life Events and Difficulties were used.

By contrast to the findings of Brown, none of the following factors was associated with the onset of depression in the 12 months prior to interview: social class, employment status, number of children at home, loss of mother before age 11. In agreement with Brown, a lack of intimacy with spouse/cohabitant/boyfriend increased the risk of depression. Also in agreement with Brown, severe life events and difficulties were associated with depression. The association was particularly strong for ‘possibly independent’ events and difficulties, i.e. events (and difficulties) that may or may not have been caused in part by the woman herself. It was concluded that the role of social factors is community-specific and that the causal roles of events and difficulties in relation to depression remain uncertain. The implications of the findings in relation to the locus of vulnerability to depression are briefly discussed

A study of psychotropic drug prescribing, derived from the computerized pricing data in Northern Ireland from 1966, showed that the use of these drugs reached a peak in 1975, when about 12·5% of the adult population were estimated to have been receiving them, and declined in the following 5 years. Benzodioazepines accounted for three-quarters of all psychotropic drugs prescribed in 1980. Benzodiazepine tranquillizer prescribing was consistently 20–30% higher than in the rest of the United Kindom, in contrast to hypnotic and antidepressant prescribing which has been consistently lower. The rate of increase in benzodiazepine tranquillizer prescribing was greater than in other European countries, but the level remains lower than in Iceland and Denmark. The influence of a number of demographic and socioeconomic variables was studied in an intra-regional analysis of the 1978 data for the 17 health districts in the province, using multivariate and multiple regression statistics. The prescribing of benzodiazepine hypnotics was almost entirely accounted for by the proportion of elderly (over 65 years) and women aged 45–59 years; neuroleptic prescribing was largely a function of factors associated with rural areas (overcrowding and unemployment) and the proportion of elderly; but neither tranquillizer, antidepressant, barbiturate hypnotic nor psychostimulant prescribing were satisfactorily explained by these variables.

Three experiments are reported in which schizophrenics are compared with other psychiatric patients on (i) the imagery value of words in their speech, (ii) their memory for words differing in imagery value, and (iii) their ability to distinguish the imagery value of two words. The study was designed to evaluate a neglected interpretation of Goldstein's ideas that schizophrenics are more concrete, in that their ‘action is determined by momentary sense impressions’. The results gave no support for this interpretation and it is suggested that an abnormal way of categorizing the world is more likely to be a useful way of formulating Goldstein's ideas.

The use of a standard intelligence test for detecting cognitive impairment has been re-examined in a cohort of 506 hospitalized alcoholics. The value of Vocabulary scores as indicators of original intelligence was tested by formulating the hypothesis that Vocabulary scores would remain constant despite increasing degrees of alcohol-induced cerebral dysfunction. Subjects were assigned to non-dysfunctional, minimal, moderate and severe dysfunctional groups on the basis of psychometric testing and their Wechsler Vocabulary scores categorized as either ‘abstract’, ‘concrete’ or ‘wrong’. Significant linear deficits in Vocabulary score and loss of abstract ability correlated with increasing dysfunction, irrespective of age but with greater frequency in females.

In three collaborating institutions 105 hospitalized female anorexia nervosa patients were assessed for depressive symptomatology periodically during treatment. As a whole, patients were mildly to moderately depressed, being as depressed as anxious neurotics and less depressed than depressed neurotics. The more depressed patients showed a variety of characteristics, many of which have previously been shown to be indicators of poor prognosis. Over the course of treatment patients became less depressed. Weight gain was correlated with a decrease in depression.
