- In our study, unstable suicidal ideation (i.e. daily fluctuation in suicidal ideation), in contrast to stable suicidal ideation (i.e. continuously either suicidal or not suicidal), was strongly associated with suicide. Suicidal ideation is not static. Rather, it is a balance between self-destructive and self-preservative wishes at that point in time. Stengel (1977) suggested that victims want neither to live nor to die, but desire both at the same time, usually one more than the other.

- Using two risk scales,one of 6 and the other of 18 socio-demographic and clinical items, weighted according to their value in discriminating between suicides and attempted suicides, and a rating of intent to die during the index suicide attempt, we followed up 1263attempted suicides for two years.

- In the Judaeo-Christian tradition, stigma against suicide is not evident until the fourth century; the Bible does not condemn suicide ( Barraclough, 1992 ), but St Augustine considered suicide as unacceptable within Christian values ( Pritchard, 1996 ). Gradually, the stigma against suicide intensified in Europe and became a great sin, shame and eventually a crime.

- Open verdicts should be therefore also considered before such an extreme decline in suicide rates is reported. Finally, Rihmer et al ( 2000 ) have thought about the possibility of a relationship between suicide rates in Hungary and recent improvements in mental health policy in that country.

- The results also confirmed that though parasuicide and suicide are different, they are clearly related. One per cent of detected para suicides committed suicide within the year of study. Likely pathways Pathways to suicide in people bereaved by suicide can be hypothesised from available evidence. A recent Danish study of adults bereaved by a partner's suicide identified specific psychiatric disorders associated with suicide bereavement 3 that are likely contributors to suicide risk.

- With regard to the four-week period before death, 7/56 (13%) of the younger suicides and 24/88 (27%) of the older suicides had had contact with mental health services (x2=4.42, d.f.=l, P=0.036). Fifteen of the 56 (27%) suicides under 35 years of age and 53/88 (60%) of the suicides 35 years and older had seen any kind of health professional in the same time period (x2=15.36, d.f.=l, Pc0.001).

- In the case of suicide, for example, the worldwide optimal treatment of depression would bring only a minimal reduction in suicide rates (further details available from the author upon request). None the less, fighting depression is generally perceived as the K constant of suicide prevention in existing national strategies.

- I would like to comment on the editorial by De Leo ( 2002 ) which came to the conclusion that little is new in suicide prevention. Since nothing was mentioned about pharmacotherapeutic advances in suicide, I would like to take the opportunity to discuss recent information concerning the role of novel antipsychotics in the reduction of suicidality.

- Suicidal ideation, attempted suicide and suicide are much more common among young Asian women than young white women. Holding non traditional values and being in conflict with parents are associatedwith suicidal ideation and, in those who have made suicide attempts, with high suicidal intent. Dr Soni Raleigh et a! reported an increased incidenceof suicidein young married Asian women and we found thosewho are married but separated and make suicide attempts have high suicidal intent.

- We read with interest the editorial on suicide prevention from a life course perspective ( Gunnell & Lewis, 2005 ). It offers a broad conceptual overview of the issues related to suicide. Recent reports from Vellore suggest that suicide rates in India are grossly underreported ( Joseph et al , 2003 ; Aaron et al , 2004 ; Abraham et al , 2005 ; Prasad et al , 2005 ). The average annual suicide rate was 95 per 100 000 for the years 1994-99.

- I read De Leo's ( 2002 ) editorial on preventing suicide with interest. However, I would like to raise a few concerns. In spite of much development and understanding in both biological and psychological causes for suicide, the prevention of suicide remains an imperfect art. However, the comparison of suicide prevention with that of ischaemic heart disease seems inappropriate. The risk factors for ischaemic heart disease are well known, stable and quantifiable.

- Suicidal behaviour (suicide attempts, suicide threats, self-harm) is considered to be an important risk factor for suicidality in general ( Ringel, 1969 ). In previous prison suicide studies, a high percentage (43–62%) of suicides were found to be of people with a history of suicidality ( Backett, 1987 ; Dooley, 1990 ; Marcus & Alcabes, 1993 ; Bogue & Power, 1995 ; Laishes, 1997 ; Fruehwald et al , 2003 ).

- Third, the recommendations on suicide prevention (below) have not been subject to major clinical trials. Even so, we believe that they represent good practice on suicide prevention and are also likely to benefit non-suicidal prisoners. Fourth, suicide rates were not standardised by age and are not strictly comparable with the general population figures presented here.
