The use of agile methods can also be advantageous in the development of physical products, as pilot projects have shown so far. However, the transfer of agile methods to the entire organisation is difficult, due to the different, prevailing circumstances of the various development activities. We have therefore defined a new approach subdividing product development into agile technology-driven and traditional product-oriented development. This model considers the methodological characteristics and thus enables the combination of the benefits of both agile and traditional development methods..

OBJECTIVE
To assess the impact of a novel, silver-coated needleless connectors (NCs) on central-line–associated bloodstream infection (CLABSI) rates compared with a mechanically identical NCs without a silver coating.
DESIGNProspective longitudinal observation study
SETTINGTwo 500-bed university hospitals
PATIENTSAll hospitalized adults from November 2009 to June 2011 with non-hemodialysis central lines
INTERVENTIONSHospital A started with silver-coated NCs and switched to standard NCs in September 2010; hospital B started with standard NCs and switched to silver-coated NCs. The primary outcome was the difference revealed by Poisson multivariate regression in CLABSI rate using standard Centers for Disease Control and Prevention surveillance definitions. The secondary outcome was a comparison of organism-specific CLABSI rates by NC type.
RESULTSAmong 15,845 hospital admissions, 140,186 central-line days and 221 CLABSIs were recorded during the study period. In a multivariate model, the CLABSI rate per 1,000 central-line days was lower with silver-coated NCs than with standard NCs (1.21 vs 1.79; incidence rate ratio=0.68 [95% CI: 0.52–0.89], P=.005). A lower CLABSI rate per 1,000 central-line days for the silver-coated NCs versus the standard NCs was observed with S. aureus (0.11 vs 0.30, P=.02), enterococci (0.10 vs 0.27, P=.03), and Gram-negative organisms (0.28 vs 0.63, P=.003) but not with coagulase-negative staphylococci (0.31 vs 0.36) or Candida spp. (0.42 vs 0.40).
CONCLUSIONSThe use of silver-coated NCs decreased the CLABSI rate by 32%. CLABSI reduction efforts should include efforts to minimize contamination of NCs.

Facial nerve outcomes in functional vestibular schwannoma surgery: less than total tumour excision significantly improves results

Objectives:
To determine the implications of a functional approach to vestibular schwannoma surgery, with facial nerve function prioritised higher than total tumour excision.
Study design:A case–control study in a tertiary referral neurotology clinic.
Patients:A ‘functional’ surgical group treated after April 2007 (n = 44, mean cerebellopontine angle dimension 27 mm), and an ‘excisional’ surgical group matched for tumour size, treated from 1997 to April 2007 (n = 115).
Intervention:Change to more functional surgical approach.
Main outcomes measured:Primary outcome: facial nerve status. Secondary outcome: tumour recurrence in less-than-total tumour excision.
Results:Facial nerve preservation: 77 per cent House–Brackmann grade I–II in functional group at 12 months, versus 57 per cent grade I–II in excisional group (p = 0.027). Tumour recurrence: 1 per cent in total excision group, 2 per cent in near-total group and 40 per cent in sub-total group.
Conclusion:A functional approach to vestibular schwannoma surgery improves facial nerve preservation outcomes and reduces the requirement for facial nerve rehabilitative interventions. Tumour recurrence rates are low in near-totally excised lesions but significant if only sub-total excision is achieved.

The use of exogenous hormone treatment as a means of increasing the efficiency of animal production has, in general, been rather poorly received by both the public and government agencies. This antipathy towards hormone treatment has tended to direct research effort into finding less contentious ‘hormonefree’ methods of improving animal production. Over the last 10 years, immunological manipulation of the endocrine system has emerged as an attractive possibility in this search. In this review we briefly outline three approaches under current consideration, whereby growth hormone (GH) concentrations or activity are modified. These methods involve immunological manipulation of GH release, use of antibodies as GH mimics and antibody-mediated enhancement of GH action.

Chondrodysplasia punctata is a term referring to a clinically heterogeneous group of bone and cartilage dysplasias which cause characteristic epiphyseal stippling. The condition can involve the ear, nose and throat in diverse ways at many levels. We present a case of X-linked brachytelephalangic chondrodysplasia punctata, which illustrates the features of this condition particularly relevant to the audiological physician, otolaryngologist and neonatologist.
Lifetime productivity in gilts previously selected for decreased age at puberty

A study was conducted to evaluate the effect of decreasing age of puberty on lifetime productivity in sows. Two lines of gilts from the Nebraska Gene Pool population were used in this study: a line that had been selected for decreased age at puberty (AP) and a line in which selection had been random (RS). The study was conducted in two parts. In part one, 75 gilts were mated at second oestrus and the productivity measured over five parities. A second experiment utilizing 68 gilts was conducted to provide further data for comparing litter size at parity 1, and also to compare ovulation rates in the two lines at second oestrus. Results showed that litter size was similar in both lines across parities. After five parities the percentage of sows farrowing relative to parity 1 was 58-8% for the AP line but only 39·4% for the RS line (P = 0·17). Litter birth weight, litter size and weight at 21 days, number weaned, and lactation food consumption were similar for both lines. Lactation weight loss was not significantly different between the two lines (60·9 (s.e. 5·9) v. 527 (s.e. 5·0) kg, for RS and AP gilts, respectively) but was consistent with the slightly longer weaning to remating intervals in the RS line (7·8 (s.e. 0·7) v. 6·6 (s.e. 0·7) days, P = 0·22). Ovulation rate at second oestrus did not differ between the two lines (14·1 (s.e. 0·9) v. 14·3 (s.e. 0·5), for RS and AP gilts, respectively). The regression of mean accumulative productivity on time was in favour of the AP line (P = 0·05). These results suggest that reproductive performance is not impaired in gilts which have been selected to reach puberty at earlier ages, and productivity at a specific age may be enhanced.
