Summary
Alexis de Tocqueville is often described as a critic of American culture and modern democracy. Yet, as Alan Levine argues, there is an important difference between Tocqueville’s friendly criticisms of parts of American culture he finds wanting and other ideological critiques by “anti-American” thinkers such as Martin Heidegger and the Frankfurt School. Several factors separate Tocqueville from this European tradition of “Anti-Americanism.” Tocqueville’s criticisms are balanced by an appreciation of the virtues of American democracy and a recognition that these defects are hardly unique to America. His criticisms also take their root in empirical considerations of the complexities of American culture. Although the Frankfurt School and other influential critics often claim Tocqueville as inspiration for their complaints about mass society, they are ideologically motivated, ignore America’s redeeming virtues, and fault America uniquely for widely shared flaws of modernity.

Little information exists on the state of infection control (IC) practices in ambulatory sites. In preparation for a JCAHO survey, 62 ambulatory sites were visited to monitor and improve IC practices. A dedicated IC practitioner was instrumental in improving practices at ambulatory sites and in successfully completing a JCAHO survey.

This study was designed to test the hypothesis that non-ambulatory patients with spastic quadriplegia will have reduced bone mass which worsens with increasing age. Forty-eight patients (age 5 to 48 years, median age 15 years; 19 females and 29 males) were studied. Anticonvulsants were used in 29 patients (60.4%). Lumbar spine bone mineral density (LS-BMD) was markedly reduced compared with age- and sex-matched control individuals with a z score of –2.37±0.21. Twenty-eight (58%) had z scores of less than –2. A history of documented previous fracture was present in 19 patients (39%). Patients with a history of fracture had significantly lower (p=0.05) LS-BMD z scores (–2.81±0.29) compared with those without a history of fracture (–2.11±0.26). Mean serum 25-OH vitamin D was 29.6±1.9ng/mL (normal 9 to 37.6ng/mL) with three patients having serum 25-OH vitamin D levels less than 15ng/mL. These findings indicate that BMD is markedly reduced in non-ambulatory children and adults with neuromuscular disease. Reductions in bone mass put them at greater risk for non-traumatic fractures.

The urinary bladder is a smooth muscle organ the function of which is to collect and store urine at low intravesical pressures, and then periodically to expel urine via a highly co-ordinated sustained contraction (Levin et al. 1990a; Anderson, 1993; Steers, 1994; Zderic et al. 1996a). The urethra functions to maintain continence during bladder filling, and then to provide a low-resistance pathway during micturition. The control of lower urinary tract function is through the autonomic nervous system. During bladder filling, the sympathetic nervous system is active and this produces an increase in tension of the bladder base and urethra via activation of a-adrenergic receptors, and relaxation of the bladder body via activation of ß-adrenergic receptors. In addition, sympathetic activity inhibits parasympathetic ganglionic transmission via postsynaptic sympathetic fibres which act upon a2-type receptors located in the ganglia. Micturition is mediated via the co-ordinated parasympathetic activation of cholinergic muscarinic receptors of the bladder body with the simultaneous inhibition of [alpha]-adrenergic tone of the bladder neck and urethra (Steers, 1994; Zderic et al. 1996a). Active urethral relaxation mediated by nitric oxide may also play a role during micturition (Anderson, 1993; Steers, 1994; Zderic et al. 1996a).
