Clozapine: How Well Are We Monitoring Patients?

Aims
Aims and auditable outcomes: We aim to ensure 100% patients on clozapine have annual physical health checks. By ensuring all patients prescribed clozapine therapy receive an annual physical health check and medic review, we aim to improve patient safety and prevent serious harm from occurring in cases that could be avoided.
MethodsAll patients aged over 18 years prescribed Clozapine, who were under the assessment and treatment service in Eastbourne, were identified using Carenotes, our electronic patient records system.
Results78% of patients on clozapine had been reviewed by a doctor in the past 12 months. 32% of patients had attended a physical health review within the past 12 months. One patient had not had a medical review for several years.
ConclusionOur audit has shown that there are no clear guidelines on the long term monitoring of clozapine in regards to physical health reviews and psychiatric assessment. Using best practice it appears annual review should be the minimal standard, however further evaluation of this is recommended at trust level.
In response to these results and the current guidance, we would like to implement the following:
• Create a database for all patients on Clozapine under the care of Eastbourne ATS.
• Create a spreadsheet looked after by one member of admin staff to be updated regularly
• The physical health lead nurse to be informed of physical health checks due by admin

OBJECTIVES/GOALS: This study aims to develop objectively scored histological characteristics of early oral leukoplakia, which may be correlated with molecular pathways predictive of progression into proliferative verrucous leukoplakia (PVL). The secondary aim is to develop a biomarker profile to be used in diagnosis, staging, and management of PVL. METHODS/STUDY POPULATION: Clinical and pathology records of 120 patients with oral leukoplakia and/or PVL were reviewed. Eight patients were selected—all had serial biopsies over time leading to PVL suspicion. Specimens were deidentified and subjected to blinded examination by a board certified oral pathologist, then scored relative to the extent of each of the commonly accepted histologic characteristics of PVL: hyperkeratosis, acanthosis, blunt rete ridges, hyperchromatic nuclei, increased nuclear-cytoplasmic ratio, dyskeratosis, and surface corrugation. Given these results, a larger subset of patient samples will be labeled and assayed for expression of epidermal growth factor receptor tyrosine kinases and downstream pro-oncogenic signaling mediators. Expression of these factors will be tested against progression to PVL. RESULTS/ANTICIPATED RESULTS: Histologically, in scoring the specimens from eight subjects, the characteristics of acanthosis, dyskeratosis, and blunted rete ridges had the strongest correlation with eventual progression to PVL. These criteria will therefore be recommended as an objective histopathologic method of identification of patients with high risk of development of PVL, and therefore malignant potential. We expect the results of the biomarker assay to provide a molecular basis for predicting PVL pathogenesis. Particularly, we anticipate pro-oncogenic targets such as EGFR, PI3K, Akt, and mTOR pathways will show increased expression as leukoplakic lesions progress. These results would then provide the basis for testing patient samples for expression of these markers in a longitudinal study of PVL emergence and progression. DISCUSSION/SIGNIFICANCE: The aggressive nature of PVL, with a rate of malignant transformation of 61% and mortality rate of 40%, requires close clinical monitoring in order to improve patient outcomes. Therefore, well defined objective clinical, histologic, and molecular criteria are critical for early detection of sites likely to progress to PVL and subsequent malignancy.

Objective:
This study investigates the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmission potential in North Dakota, South Dakota, Montana, Wyoming, and Idaho from March 2020 through January 2021.
Methods:Time-varying reproduction numbers, Rt, of a 7-d-sliding-window and of non-overlapping-windows between policy changes were estimated using the instantaneous reproduction number method. Linear regression was performed to evaluate if per-capita cumulative case-count varied across counties with different population size or density.
Results:The median 7-d-sliding-window Rt estimates across the studied region varied between 1 and 1.25 during September through November 2020. Between November 13 and 18, Rt was reduced by 14.71% (95% credible interval, CrI, [14.41%, 14.99%]) in North Dakota following a mask mandate; Idaho saw a 1.93% (95% CrI [1.87%, 1.99%]) reduction and Montana saw a 9.63% (95% CrI [9.26%, 9.98%]) reduction following the tightening of restrictions. High-population and high-density counties had higher per-capita cumulative case-count in North Dakota on June 30, August 31, October 31, and December 31, 2020. In Idaho, North Dakota, South Dakota, and Wyoming, there were positive correlations between population size and per-capita weekly incident case-count, adjusted for calendar time and social vulnerability index variables.
Conclusions:Rt decreased after mask mandate during the region’s case-count spike suggested reduction in SARS-CoV-2 transmission.

The increasing availability of antiretroviral therapy (ART) and drug regimens to prevent mother-to-child transmission (PMTCT) has probably changed the context of childbearing for people living with HIV. Using data from 2009–2010 community-based surveys in Nigeria and Zambia, this study explores whether women's knowledge about ART and PMTCT influences the relationship between HIV status and fertility preferences and contraceptive behaviour. The findings show that women living with HIV are more likely to want more children in Nigeria and to want to limit childbearing in Zambia compared with HIV-negative women. While there is no significant difference in contraceptive use by women's HIV status in the two countries, women who did not know their HIV status are less likely to use contraceptives relative to women who are HIV-negative. Knowledge about ART reduces the childbearing desires of HIV-positive women in Nigeria and knowledge about PMTCT increases desire for more children among HIV-positive women in Zambia, as well as contraceptive use among women who do not know their HIV status. The findings indicate that knowledge about HIV prevention and treatment services changes how living with HIV affects childbearing desires and, at least in Zambia, pregnancy prevention, and highlight the importance of access to accurate knowledge about ART and PMTCT services to assist women and men to make informed childbearing decisions. Knowledge about ART and PMTCT should be promoted not only through HIV treatment and maternal and newborn care facilities but also through family planning centres and the mass media.

Summary
abstract The Hudson River Estuary has been polluted for many decades with organic contaminants including PCBs, dioxins/furans, PAHs, pesticides, and a variety of toxic metals, including cadmium and mercury. Most of these toxicants are poorly metabolized, highly persistent, bioaccumulative, and biomagnify in Hudson River populations, sometimes to record high levels. Many surveys have quantified tissue levels of these contaminants in resource species, but despite public concern and a need to evaluate toxicities for regulatory actions, few studies have directly addressed their biological impacts on Hudson River populations. With several notable exceptions, toxicant-induced perturbations are not frequently observed in Hudson River populations, even those for which high levels of exposure have been documented. This may have resulted from the ability of populations to acquire resistance to high levels of contaminants, either through genetic adaptations or compensatory physiological acclimation responses. While offering short-term benefits to impacted populations, resistance may be associated with evolutionary costs to populations and may compromise the viability of affected communities. Ideally, in the future, contaminant studies should focus on those species for which toxic alterations have been observed which may impact population viability, their levels of contamination should be quantified, and controlled laboratory experiments should be conducted to confirm that the contaminants of concern are able to induce these toxic manifestations in the affected taxon.
The Problem: Assessment of Toxicity of Hudson River-borne Pollutants
There is a need for government agencies to evaluate the toxic impacts of contaminants on the Hudson River (HR) biota to determine if damage to its populations has occurred and to guide remediation efforts.

All else being equal, ectotherms should maintain body temperatures (Tbs) favourable for temperature-sensitive biological functions, such as digestion and locomotion. Physical environments in the temperate zone are often thermally variable, however, thus making it difficult to maintain optimum Tbs. Radiotelemetry and a semi-natural enclosure were used to monitor Tbs of grass snakes Natrix natrix at Canterbury, Kent, U.K. Operative temperatures (Tes) were measured using snake models (copper-pipe models) placed in a variety of microhabitats to determine availability of thermoregulatory opportunities. A modification of Huey & Slatkin's (1976) regression model, piecewise regression, was used to evaluate thermoregulatory behaviour. Grass snakes could achieve high mean Tbs (>30°C) most often during midday (08:00–20:00), but only for 55–61% of the study period. Overall, non-gravid snakes maintained higher and less variable mean daytime Tbs than gravid snakes. From piecewise regression, it was determined that grass snakes initiated thermoregulation at a Te of 38.44°C, corresponding to a Tb of 27.7°C. Three main conclusions can be drawn from this study: (1) thermoregulatory opportunities for grass snakes were limited; (2) nonetheless, when conditions were sufficiently warm, there was clear evidence that grass snakes could thermoregulate; (3) contrary to expectation, gravid females actually maintained lower and more variable Tbs than non-gravid females. The consequences of these thermoregulatory patterns for the fitness of snakes have yet to be determined.
Subjective imagery in obsessive–compulsive disorder before and after exposure therapy: Pilot randomised controlled trial

Background
Distressing mental imagery is hard to study experimentally in obsessive–compulsive disorder (OCD).
AimsTo develop a way to assess mental imagery in OCD during functional magnetic resonance imaging (fMRI).
MethodA small randomised study, controlled for type and order of mental imagery and for treatment condition (exposure therapy guided by a computer or by a therapist, or relaxation guided by audio-tape). Before and after treatment, during fMRI scanning, patients imagined previously-rehearsed scenarios that evoked an urge to ritualise or non-OCD anxiety or a neutral state, and rated their discomfort during imagery.
ResultsThe method evoked greater discomfort during OCD imagery and anxiety (non-OCD) imagery than during neutral imagery. Discomfort was reduced by cancelling imagery. Discomfort during OCD imagery (but not during anxiety non-OCD imagery) fell after exposure therapy but not after relaxation.
ConclusionsResults showed differences between OCD and non-OCD images and their change after successful treatment, and confirmed clinical suggestions that cancelling images reduced OCD discomfort. The method's success paves the way for further studies of mental imagery in OCD: for instance, during fMRI.

Background
The concept of fixed, unshakeable (delusional) beliefs within the context of obsessive–compulsive disorder (OCD) is one that has received varying amounts of attention in the literature, and has not yet received universal acknowledgement. There are good grounds for including these cases within the diagnostic concepts of OCD, with significant implications for clinical management.
AimsTo present cases with unusual OCD, in order to re-evaluate the issue of delusions and OCD.
MethodThe cases of five subjects with delusions in the course of obsessive–compulsive disorder are presented to illustrate ‘delusional’ OCD. The management and outcome of these cases are discussed.
ResultsFixity and bizarreness of beliefs in OCD occur on a continuum from ‘none’ to ‘delusional intensity’ and may fluctuate within subjects.
ConclusionsThe idea that these cases may represent a form of OCD has implications for management, as, if this is correct, they should be able to respond to appropriate behavioural and/or pharmacological strategies used in OCD.
