A Canadian National Survey of the Neurosurgical Management of Intracranial Abscesses

Objective:
Intracerebral abscess is a life-threatening condition for which there are no current, widely accepted neurosurgical management guidelines. The purpose of this study was to investigate Canadian practice patterns for the medical and surgical management of primary, recurrent, and multiple intracerebral abscesses.
Methods:A self-administered, cross-sectional, electronic survey was distributed to active staff and resident members of the Canadian Neurosurgical Society and Canadian Neurosurgery Research Collaborative. Responses between subgroups were analyzed using the Chi-square test.
Results:In total, 101 respondents (57.7%) completed the survey. The majority (60.0%) were staff neurosurgeons working in an academic, adult care setting (80%). We identified a consensus that abscesses >2.5 cm in diameter should be considered for surgical intervention. The majority of respondents were in favor of excising an intracerebral abscess over performing aspiration if located superficially in non-eloquent cortex (60.4%), located in the posterior fossa (65.4%), or causing mass effect leading to herniation (75.3%). The majority of respondents were in favor of reoperation for recurrent abscesses if measuring greater than 2.5 cm, associated with progressive neurological deterioration, the index operation was an aspiration and did not include resection of the abscess capsule, and if the recurrence occurred despite prior surgery combined with maximal antibiotic therapy. There was no consensus on the use of topical intraoperative antibiotics.
Conclusion:This survey demonstrated heterogeneity in the medical and surgical management of primary, recurrent, and multiple brain abscesses among Canadian neurosurgery attending staff and residents.1
Creating a Novel Disaster Medicine Virtual Reality Training Environment

Introduction:
Disasters are high-acuity, low-frequency events which require medical providers to respond in often chaotic settings. Due to this infrequency, skills can atrophy, so providers must train and drill to maintain them. Historically, drilling for disaster response has been costly, and thus infrequent. Virtual Reality Environments (VREs) have been demonstrated to be acceptable to trainees, and useful for training Disaster Medicine skills. The improved cost of virtual reality training can allow for increased frequency of simulation and training.
Problem:The problem addressed was to create a novel Disaster Medicine VRE for training and drilling.
Methods:A VRE was created using SecondLife (Linden Lab; San Francisco, California USA) and adapted for use in Disaster Medicine training and drilling. It is easily accessible for the end-users (trainees), and is adaptable for multiple scenario types due to the presence of varying architecture and objects. Victim models were created which can be role played by educators, or can be virtual dummies, and can be adapted for wide ranging scenarios. Finally, a unique physiologic simulator was created which allows for dummies to mimic disease processes, wounds, and treatment outcomes.
Results:The VRE was created and has been used extensively in an academic setting to train medical students, as well as to train and drill disaster responders.
Conclusions:This manuscript presents a new VRE for the training and drilling of Disaster Medicine scenarios in an immersive, interactive experience for trainees.

Water use by saltcedar, an invasive phreatophyte, is of significant concern in many riparian zones in the western United States. Diurnal groundwater fluctuations were analyzed to estimate evapotranspiration and water salvage (water available for other ecological functions) in saltcedar stands over a 6-yr period on a site along the Pecos River in Texas. Seasonal stand-level saltcedar water loss at an untreated control site ranged from 0.42 to 1.18 m/yr. Seasonal water salvage following application of imazapyr ranged from 31% 4 yr after treatment to 82% 2 yr after treatment. Significant water savings may be achieved by chemical saltcedar control, dependent upon water use by replacement vegetation and saltcedar regrowth. A regrowth management strategy is essential to maintain long-term water salvage.
