Summary
The role of climate (including abrupt changes and extreme weather) in modern-era violence and conflict has received considerable attention in the past two decades from scholars in multiple fields, yet the mechanisms underlying (and even the reality of) such a role remains contested. Concern over projected climatic changes as a trigger for intensified violence, including mass killing and genocide, nonetheless continues to propel research. Data limitations are frequently cited as a challenge, yet comparatively few studies have turned to the millennia of human history documenting a broad range of violence against diverse social and environmental backgrounds. This chapter reviews evidence for ‘pathways’ by which climate may have contributed to violence and conflict in the Ancient Near East and Egypt. It emphasized religious, ideological, and ethnic dimensions that may have been catalysed by the psychological and material impacts of extreme weather to promote violence and conflict. In particular, we study state-enacted violence by the Neo-Assyrian Empire (909-611 BCE) and internal revolt in Ptolemaic Egypt (305-30 BCE). Newly available ice-core-based dates of explosive volcanism allow the examination of societal responses to the ensuing hydroclimatic shocks (also potentially ‘ominous’ volcanic dust-veils). These can be shown to closely precede documented increases in violence and conflict, including external warfare and internal revolt.
Cost-effectiveness of mirtazapine for agitated behaviors in dementia: findings from a randomized controlled trial

Objectives:
To examine the costs and cost-effectiveness of mirtazapine compared to placebo over 12-week follow-up.
Design:Economic evaluation in a double-blind randomized controlled trial of mirtazapine vs. placebo.
Setting:Community settings and care homes in 26 UK centers.
Participants:People with probable or possible Alzheimer’s disease and agitation.
Measurements:Primary outcome included incremental cost of participants’ health and social care per 6-point difference in CMAI score at 12 weeks. Secondary cost-utility analyses examined participants’ and unpaid carers’ gain in quality-adjusted life years (derived from EQ-5D-5L, DEMQOL-Proxy-U, and DEMQOL-U) from the health and social care and societal perspectives.
Results:One hundred and two participants were allocated to each group; 81 mirtazapine and 90 placebo participants completed a 12-week assessment (87 and 95, respectively, completed a 6-week assessment). Mirtazapine and placebo groups did not differ on mean CMAI scores or health and social care costs over the study period, before or after adjustment for center and living arrangement (independent living/care home). On the primary outcome, neither mirtazapine nor placebo could be considered a cost-effective strategy with a high level of confidence. Groups did not differ in terms of participant self- or proxy-rated or carer self-rated quality of life scores, health and social care or societal costs, before or after adjustment.
Conclusions:On cost-effectiveness grounds, the use of mirtazapine cannot be recommended for agitated behaviors in people living with dementia. Effective and cost-effective medications for agitation in dementia remain to be identified in cases where non-pharmacological strategies for managing agitation have been unsuccessful.
Efficacy of five herbicide treatments for control of Pyrus calleryana

Callery pear (Pyrus calleryana Decne.) is rapidly spreading in the United States, gaining attention in the last two decades as a serious invasive pest. Recommended control methods include foliar, basal bark, cut stump, and hack-and-squirt application of herbicides, but there are few published studies with replicated data on efficacy. Four readily available herbicidal active ingredients and a combination of two active ingredients were tested for control efficacy against P. calleryana in old-field areas and loblolly pine (Pinus taeda L.) understory. Basal bark applications (triclopyr, triclopyr + aminopyralid), foliar applications (glyphosate, imazapyr), and a soil application (hexazinone) effectively killed P. calleryana with the exception of hexazinone at one site, where rainfall may not have been optimal. Foliar application of glyphosate provided the most consistent control. Our results demonstrate efficacy of registered herbicide formulations for P. calleryana control in two geographic locations and two habitat types. The need for development of integrated pest management programs for P. calleryana is discussed.

Physical infrastructure, including buildings, roads, pipelines, bridges, power lines, communications networks, canals, and waterways, make up a substantial fraction of worldwide material usage and flows. Consequently, the overall mass of materials and the associated environmental impacts must be addressed to achieve sustainable development of infrastructure. This article surveys the magnitude of material use for infrastructure, including trends in the use per person, environmental impacts of the production and use of infrastructure materials, variations in the longevity of physical infrastructure, and changes in the recycling of infrastructure materials.

Managing adequately the exploitation of commercial species subject to technical interactions is a key step towards the ecosystem approach to fisheries. We evaluate here, using a bio-economic modelling approach building on the ISIS-Fish simulation platform, the relative impact of total allowable commercial catches (TACC) and of taxes applicable to over-quota landings (deemed value) on the sustainability of a selection of species exploited by the New Zealand hoki (Macruronus novaezelandiae) fishery. We investigate some aspects of the hoki mixed fisheries, consisting of four fleets and nineteen métiers, by considering the technical interactions between hoki and Southern hake (Merluccius australis). The dynamics of effort allocation were modelled using gravity model, using value per unit of effort (VPUE) as attractivity coefficient. Several management scenarios, based on different levels of TACC, effort limits and deemed value have been defined. The impact of these strategies on spawning biomass and catches has been investigated. The results confirm that the deemed value may, in combination with TACC, be an efficient management tool. By increasing the hoki deemed value, we could somehow limit the decrease in the hoki TACC needed to harvest Western hoki sustainably. Constraining hoki fishing also restricts Southern hake fishing in the Chatham Rise and in the West Coast South Island, but leads to increased targeting of this species in the Sub-Antarctic area. The relative costs of renting/buying quota or paying the deemed value are most constraining when the hoki TACC is at relatively low level and when the hoki deemed value is more than 1.5 times the current value.

Summary
Depressive disorder is the most common psychiatric disorder among patients attending primary care worldwide. This chapter indicates the ways in which different definitions and different modes of measurement used in previous research can affect the prevalence of depression. It examines the prevalence of depression in different groups and reviews the few studies that have examined the incidence of depression in the medically ill. In published studies, the prevalence of depression in the medically ill ranges between 15% and 61%. A self-administered questionnaire is required to screen a large population of physically ill patients and may be used as the first stage of a two-phase survey, which includes research interviews to determine the actual cases of depressive disorder. Cross-sectional studies demonstrate a close association between depressive disorders and physical illness in population-based studies. Finally, the chapter explains the effect of depression in terms of health-related quality of life and healthcare costs.

Background
The extent to which depression impairs health-related quality of life (HRQoL) in the physically ill has not been clearly established.
AimsTo quantify the adverse influence of depression and anxiety assessed at the time of first myocardial infarction and 6 months later, on the physical aspect of HRQoL 12 months after the infarction.
MethodIn all, 260 in-patients, admitted following first myocardial infarction, completed the Hospital Anxiety and Depression Scale and the Medical Outcomes Study SF–36 assessment before discharge and at 6- and 12-month follow-up.
ResultsDepression and anxiety 6 months after myocardial infarction predicted subsequent impairment in the physical aspects of HRQoL (attributable adjusted R2=9%, P<0.0005). These negative effects of depression and anxiety on outcome were mediated by feelings of fatigue. Depression and anxiety present before myocardial infarction did not predict HRQoL 12 months after myocardial infarction.
ConclusionsDetection and treatment of depression and anxiety following myocardial infarction improve the patient's health-related quality of life.
