Incidence and risk factors for clinically confirmed secondary bacterial infections in patients hospitalized for coronavirus disease 2019 (COVID-19)

Objective:
The true incidence and risk factors for secondary bacterial infections in coronavirus disease 2019 (COVID-19) remains poorly understood. Knowledge of risk factors for secondary infections in hospitalized patients with COVID-19 is necessary to optimally guide selective use of empiric antimicrobial therapy.
Design:Single-center retrospective cohort study of symptomatic inpatients admitted for COVID-19 from April 15, 2020, through June 30, 2021.
Setting:Academic quaternary-care referral center in Portland, Oregon.
Patients:The study included patients who were 18 years or older with a positive severe acute respiratory coronavirus virus 2 (SARS-CoV-2) PCR test up to 10 days prior to admission.
Methods:Secondary infections were identified based on clinical, radiographic, and microbiologic data. Logistic regression was used to identify risk factors for secondary infection. We also assessed mortality, length of stay, and empiric antibiotics among those with and without secondary infections.
Results:We identified 118 patients for inclusion; 31 (26.3%) had either culture-proven or possible secondary infections among hospitalized patients with COVID-19. Mortality was higher among patients with secondary infections (35.5%) compared to those without secondary infection (4.6%). Empiric antibiotic use on admission was high in both the secondary and no secondary infection groups at 71.0% and 48.3%, respectively.
Conclusions:The incidence of secondary bacterial infection was moderate among hospitalized patients with COVID-19. However, a higher proportion of patients received empiric antibiotics regardless of an identifiable secondary infection. Transfer from an outside hospital, baseline immunosuppressant use, and corticosteroid treatment were independent risk factors for secondary infection. Additional studies are needed to validate risk factors and best guide antimicrobial stewardship efforts.

BACKGROUND
Influenza A (H1N1) pdm09 became the predominant circulating strain in the United States during the 2013–2014 influenza season. Little is known about the epidemiology of severe influenza during this season.
METHODSA retrospective cohort study of severely ill patients with influenza infection in intensive care units in 33 US hospitals from September 1, 2013, through April 1, 2014, was conducted to determine risk factors for mortality present on intensive care unit admission and to describe patient characteristics, spectrum of disease, management, and outcomes.
RESULTSA total of 444 adults and 63 children were admitted to an intensive care unit in a study hospital; 93 adults (20.9%) and 4 children (6.3%) died. By logistic regression analysis, the following factors were significantly associated with mortality among adult patients: older age (>65 years, odds ratio, 3.1 [95% CI, 1.4–6.9], P=.006 and 50–64 years, 2.5 [1.3–4.9], P=.007; reference age 18–49 years), male sex (1.9 [1.1–3.3], P=.031), history of malignant tumor with chemotherapy administered within the prior 6 months (12.1 [3.9–37.0], P<.001), and a higher Sequential Organ Failure Assessment score (for each increase by 1 in score, 1.3 [1.2–1.4], P<.001).
CONCLUSIONRisk factors for death among US patients with severe influenza during the 2013–2014 season, when influenza A (H1N1) pdm09 was the predominant circulating strain type, shifted in the first postpandemic season in which it predominated toward those of a more typical epidemic influenza season.

Aims and method
To develop a self-report questionnaire to measure mental health recovery from the service user viewpoint. Literature searches and scoping exercises indicated that psychological, social and spiritual issues should be included. The resultant provisional scale was completed by 107 service users.
ResultsThe provisional scale was shortened as a result of factor analysis. The finalised version was highly reliable (Cronbach's alpha 0.911) and valid, correlating significantly with an already established recovery scale. It contained nine recognisable subscales, the first two describing existential and religious well-being. Separate well-being and ill-being factors were also identified.
Clinical implicationsAn inclusive tool for service users' assessment of their own recovery, the Service User Recovery Evaluation (SeRvE) scale, has been validated. This can be used both as a research tool and clinically to monitor interventions. The importance of spiritual care for service users is highlighted.

Environmental education has shown lamentable inertia in scarcely capitalizing on the growth of interest in environmental issues since the 1960s. The present paper explores the general factors contributing to this, which include the failure of environmentalism as a movement to act as a catalyst for environmental education, and the problems of attempting to appeal to a wide audience of both specialists and non-specialists who each expect different things of environmental education. To date, education about the environment and education from the environment have been dominant, at the expense of education for the environment; and further problems arise in establishing both an educational niche and disciplinary horizons for the subject in order to increase its appeal to educational planners.

Properties of in Situ Doped Amorphous and Polycrystalline Silicon Deposited by Chemical Vapor Deposition from Tertiarybutylarsine and Tertiarybutylphosph INE

Tcrtiarybutylarsine (TBA) and tertiarybutylphosphine (TBP) were evaluated as n-type dopant precursors for low pressure chemical vapor deposition (LPCVD) of in situ doped amorphous and polycrystalline silicon from silanc. The deposition kinetics, film composition, and electrical properties obtained with TBA and TBP dopant precursors are compared to that obtained when using arsine and phosphine as dopant precursors. Compared to the hydride dopant sources, TBA and TBP provide increased safety due to their sub-atmospheric vapor pressure (≃ 200 Torr at 26°C) and lower toxicity. Using mixtures of TBA and TBP with silane and hydrogen, high quality films of arsenic and phosphorus in situ doped amorphous and polycrystalline silicon were obtained. Dopant incorporation levels ranged from 5×1019 to l.5×1021 cm−3 as measured by SIMS. The deposition rate of polycrystallinc films deposited from TBA and TBP was enhanced by a factor of-≃-3X compared to the hydride dopants for the same mole fraction of dopant precursor. The minimum resistivitics of 5.0 and 1.4 mΩ-cm were obtained, following a 1000°C, 30 second rapid thermal anneal, for ≃500Å thick films on SiO2 deposited with TBA and TBP respectively. A drawback of TBA and TBP is the incorporation of ≃1.0 atomic percent carbon which is > 15X higher than obtained when using the hydride dopant sources. Carbon was observed under all deposition conditions examined.

Films of in-situ phosphorus-doped hydrogenated microcrystalline silicon (n--μc-Si:H) were deposited by plasma enhanced chemical vapor deposition (PECVD) on Si(100) and fused quartz substrates over a range of substrate temperatures (100 - 500°C) and reactant gas dilutions (I - 100% of 1% PH3/SiH4 in H2) while maintaining a constant RF power density (0.1 W-cm−2) and total gas pressure (1 Torr). Some of the films were subjected to a rapid thermal anneal (RTA) at temperatures between 600- 1000°C for a duration of 10 seconds. The μ-Mc-Si:H films were characterized, before and after RTA, in terms of their microstructure, optical band-gap, electrical conductivity, and hydrogen and phosphorus content. The deposition rate was determined to be insensitive to substrate temperature and to decrease with increasing H2 gas dilution indicating that deposition kinetics are dominated by plasma chemistry and are not thermally activated. For pre-annealed films, cross-sectional TEM confirmed the presence of a mixed phase material at all deposition temperatures with gas dilutions ≤10%. The surfaces of thick films (>0.15 μm) were rough, giving them a hazy appearance, while thin μic-Si:H films (<0.15 μm) were smooth and mirror-like. The rough surfaces were correlated with voids and microcracks in the μuc-Si:H films observed by TEM. The optical band-gap of all pre-annealed films was 11.8eV and the electrical conductivity varied between 1 and 20 (Δ-cm)−l. The H content was found to be independent of gas dilution but decreased with increasing substrate temperature; the P content depended on both the gas dilution and substrate temperature, decreasing at high deposition temperatures. RTA was observed to significantly alter film morphology and microstructure, increase electrical conductivity, and decrease the optical band-gap.
