Aripiprazole once-monthly for treatment of schizophrenia: double-blind, randomised, non-inferiority study

Background
Long-acting injectable formulations of antipsychotics are treatment alternatives to oral agents.
AimsTo assess the efficacy of aripiprazole once-monthly compared with oral aripiprazole for maintenance treatment of schizophrenia.
MethodA 38-week, double-blind, active-controlled, non-inferiority study; randomisation (2:2:1) to aripiprazole once-monthly 400 mg, oral aripiprazole (10–30 mg/day) or aripiprazole once-monthly 50mg (a dose below the therapeutic threshold for assay sensitivity). (Trial registration: clinicaltrials.gov, NCT00706654.)
ResultsA total of 1118 patients were screened, and 662 responders to oral aripiprazole were randomised. Kaplan–Meier estimated impending relapse rates at week 26 were 7.12% for aripiprazole once-monthly 400mg and 7.76% for oral aripiprazole. This difference (−0.64%, 95% CI −5.26 to 3.99) excluded the predefined non-inferiority margin of 11.5%. Treatments were superior to aripiprazole once-monthly 50mg (21.80%, P⩽0.001).
ConclusionsAripiprazole once-monthly 400mg was non-inferior to oral aripiprazole, and the reduction in Kaplan–Meier estimated impending relapse rate at week 26 was statistically significant v. aripiprazole once-monthly 50 mg.

Contemporary open source JAVA technologies implemented within the EURANOS project for the cross-platform re-engineering of the decision support system RODOS (JRODOS) were used also to redesign the Hydrological Dispersion Module (HDM) of this system. JHDM – the hydrological model chain of JRODOS contains models to simulate the radionuclide transport in the system “atmospheric fallout on watershed – river net” and calculate the doses via aquatic pathways. JHDM used for this purpose a limited number of the input parameters to characterize the hydrological properties of the catchment/river network of interest. The pilot implementation of JHDM for the Vistula river basin carried out by POLATOM and supported by the national hydrological institute, demonstrated good perspectives of the approach used in JHDM for the decision support in cases of accidental contamination of water systems. Particular needs for further improvements of the JHDM software system have been identified.

We developed a mathematical model of the transmission dynamics of salmonella to describe an outbreak of S. Cerro infection that occurred in a Pennsylvania dairy herd. The data were collected as part of a cooperative research project between the Regional Dairy Quality Management Alliance and the Agricultural Research Service. After the initial detection of a high prevalence of S. Cerro infection in the herd, a frequent and intensive sampling was conducted and the outbreak was followed for 1 year. The data showed a persistent presence of S. Cerro with a high prevalence of infection in the herd. The dynamics of host and pathogen were modelled using a set of nonlinear differential equations. A more realistically distributed (gamma-distributed) infectious period using multiple stages of infection was considered. The basic reproduction number was calculated and relevance to the intervention strategies is discussed.
Chemical Structuring and Materials Design in the Shells of Modern Brachiopods

Compositional and structural variations at various scale levels have been investigated in the shells of the modern brachiopod Megerlia truncata combining transmission electron microscopy (TEM), cathodoluminescence (CL), laser-ablation-inductively-coupled-mass-spectrometric (LA-ICP-MS) and micro-Raman spectroscopical (µRS) analyses. Our results show that these shells can be addressed as hierarchially structured, multi-scaled organic/inorganic fibre composites. A significant chemical as well as a mechanical inhomogeneity is observable within the shells. This occurs on various scale levels ranging from less then one to a few houndred microns. In concert to systematic variations in micro- and nanohardness the µRS and CL analyses reveal a systematic distribution and incorporation of organic material within the shells, which is negligible within the nanocrystalline primary layer but is present in a significant amount in the innermost part of the secondary layer, next to the organic tissue of the animal. Mechanical performance (hardness and fracture toughness) of this biomaterial is mainly determined by textural features, an extreme variation of crystallite size and a purpose oriented interlikage of organic and inorganic components.

NASA’s Space Interferometry Mission (SIM), scheduled for launch in 2009, will determine the positions of thousands of stars as faint as V = 20 to a precision better than 4 microarcseconds (µas). A key part of the mission is the Astrometric Grid, which is a reference frame of several thousand stars with V ≤ 13 against which all relative measurements will be calibrated. To serve as a reliable inertial reference frame, the Grid must be astrometrically stable against photocenter jitter (from planets, binary companions, flaring or spotting) at the ~ 4µas level. Sub–solar metallicity giant stars, by virtue of their intrinsic luminosity, can probe the Galaxy to greater distances than almost any other stellar type at the same apparent magnitude. Thus, distant (> 3 kpc) giants with V < 13 will have proportionately smaller astrometric jitter compared to other potential Astrometric Grid star candidates. The Grid Giant Star Survey is a patchwork all-sky survey to find sub–solar metallicity K giants for the Grid, and to provide a unique database for studies of Galactic stellar populations. We describe here the survey characteristics and give examples of results to date.
Combined Cy3 / Nanogold Conjugates for ImmunocytoChemistry and in Situ Hybridization

Fluorescein and the 1.4 nm Nanogold® gold cluster label may be incorporated into a single Fab’ immunoprobe by separate cross-linking reactions, to give a probe which labels antigenic sites in a single step for correlative fluorescence and electron microscope visualization. These probes show high labeling density, labeling a pre-mRNA splicing factor in the HeLa cell nucleus; Microtubules were also densely labeled using fluorescence, other optical modalities, and electron microscopy; in a parallel experiment, a 5 nm colloidal gold probe gave only occasional labeling. We now describe Fab’ and streptavidin probes containing both Nanogold® and the fluorescent cyanine dye, Cy3.
F(ab’)2 Goat anti-Mouse IgG and F(ab’)2 goat anti-rabbit IgG fragments were reductively cleaved to Fab’ fragments using dithiothreitol (DTT) or mercaptoethylamine hydrochloride (MEA), which selectively reduce the F(ab’)2 hinge disulfide bonds, with 5 mm EDTA to prevent reoxidation. Fab’ fragments were isolated by gel filtration (coarse gel: GH25, Amicon) then labeled with Monomaleimido- Nanogold® which reacts site-specifically with thiols. Streptavidin was labeled using Mono- Sulfo-NHS-Nanogold® at pH 7.5. Nanogold® conjugates were isolated by gel filtration (Superose-12 column, Pharmacia), then reacted with excess Cy3 monofunctional NHS ester (labeling kit, Amersham Life Sciences) at pH 7.5; dual-labeled conjugates were isolated by gel filtration (Superose-12).
A Nano-Composite Poly(Methyl-Methacrylate) Bone Cement

Poly(methyl methacrylate) (PMMA) based orthopaedic bone cements contain 1-3 Pim size radiopacifier particles. Incomplete dispersion of these particles leads to the presence of 50-200 μm size agglomerates. These large defects are sites of high-stress concentration that reduce the fracture toughness of PMMA. In this study, the micrometer-sized radiopacifying particles of a commercial bone cement were replaced by nanosized fillers. Both, commercial and nanocomposite PMMA bone cements were characterized using ultra-small angle x-ray scattering and low voltage scanning electron microscopy; mechanical properties were evaluated using ASTM standard tensile testing. The results showed a substantial reduction of particle agglomerate size and a significant increase in tensile properties of the nanocomposite over that of the standard microcomposite bone cement.

Interlayer diffusion in Langmuir-Blodgett multilayers containing Cadmium Arachidate (CdA), polyglutamate (PG) copolymers, or both, was investigated using neutron and x-ray reflectometry. Measurements from multilayers comprised only of CdA or PG revealed that little interdiffusion occurred upon annealing at 70°C or 84°C. However, some relaxation of the intralayer microstructure in the PG samples was observed at both temperatures. In contrast, the neutron reflectivity of a multilayer sandwich containing a bilayer of PG between two multilayers of CdA showed gradual and progressive change with annealing at both temperatures for annealing times of up to several hours. This change is surprising as PG is expected to act as a barrier to interdiffusion of the CdA molecules at these temperatures.

Magnetic multilayers consisting of alternating layers of cobalt and either copper or palladium have been made by magnetically enhanced dc triode sputtering onto single crystal sapphire substrates. The structural, magnetic and magnetooptic properties of these multilayers have been studied. Both multilayer systems have fcc crystalline layering. We find the magnetization in the Cu/Co multilayers is due to the cobalt layers, whereas the magnetization in the Pd/Co system comes from both cobalt and palladium layers. Also, the magnetic anisotropy of the two systems is different. The Cu/Co multilayers have an inplane easy axis of magnetization due to the strong shape anisotropy. The Pd/Co multilayers exhibit a perpendicular easy axis of magnetization for thin cobalt layers due to a large interface anisotropy contribution. The Kerr rotation of the Cu/Co and Pd/Co multilayers has been studied to determine the effect of multilayer structure on magneto-optic properties. The results of the magnetooptic characterization of these multilayer systems are presented here.
