Neurodegenerative diseases are characterized by selective degeneration of certain biochemically distinct subpopulations of central neurons. Studies of the intrinsic vulnerability of such neurons to injury by excitatory amino acids in vitro, as well as study of neurologic syndromes produced in animals or humans by ingestion of environmental excitatory amino acid neurotoxins may suggest a link between excitotoxicity, and the pathogenesis of certain neurodegenerative diseases.
Improving mental health among people living with HIV: a review of intervention trials in low- and middle-income countries

People living with HIV (PLWH) experience greater psychological distress than the general population. Evidence from high-incomes countries suggests that psychological interventions for PLWH can improve mental health symptoms, quality of life, and HIV care engagement. However, little is known about the effectiveness of mental health interventions for PLWH in low- and middle-income countries (LMICs), where the large majority of PLWH reside. This systematized review aims to synthesize findings from mental health intervention trials with PLWH in LMICs to inform the delivery of mental health services in these settings. A systematic search strategy was undertaken to identify peer-reviewed published papers of intervention trials addressing negative psychological states or disorders (e.g. depression, anxiety) among PLWH in LMIC settings. Search results were assessed against pre-established inclusion and exclusion criteria. Data from papers meeting criteria were extracted for synthesis. Twenty-six papers, published between 2000 and 2014, describing 22 unique interventions were identified. Trials were implemented in sub-Saharan Africa (n = 13), Asia (n = 7), and the Middle East (n = 2), and addressed mental health using a variety of approaches, including cognitive-behavioral (n = 18), family-level (n = 2), and pharmacological (n = 2) treatments. Four randomized controlled trials reported significant intervention effects in mental health outcomes, and 11 preliminary studies demonstrated promising findings. Among the limited mental health intervention trials with PLWH in LMICs, few demonstrated efficacy. Mental health interventions for PLWH in LMICs must be further developed and adapted for resource-limited settings to improve effectiveness.

Summary
Ischemic stroke is a consequence of transient or permanent reduction of blood flow to a focal region of the brain, usually caused by the occlusion of an artery by an embolus or thrombus. Cerebral ischemia may also occur globally in the setting of cardiac arrest and resuscitation. While historically brain damage has been considered to be an inexorable consequence of focal or global ischemic insults, in recent years a growing understanding of the underlying mechanisms responsible for brain cell death has led to the identification of new therapeutic approaches. Two main approaches have emerged. The first aims to restore lost blood flow by dissolving the thrombus responsible for cerebral artery obstruction (in ischemic stroke). Thrombolysis has become an established treatment for ischemic stroke after the efficacy of intravenous tissue plasminogen activator (tPA) was demonstrated in a landmark study (The NINDS rt-PA Stroke Study Group, 1995). Promising results have also been reported for the thrombolytic agent pro-urokinase, delivered by intra-arterial catheter directly to the site of intravascular thrombus (Furlan et al., 1999). The second approach, neuroprotection, aims to reduce the intrinsic vulnerability of brain tissue to ischemia; it will be the topic of this chapter.
Mechanisms of ischemic neuronal death
The brain is more vulnerable to ischemia than many other tissues, so it seems plausible that the cellular mechanisms of this heightened vulnerability could be delineated and blocked. Over the last 15 years evidence has accumulated indicating that normal brain signaling and immune defense mechanisms may become harmful after ischemic insults (Rothman & Olney, 1986; Choi, 1988; del Zoppo et al., 2000). In particular, substantial evidence now implicates excitotoxicity, programmed cell death, and inflammation in the pathogenesis of ischemic neuronal death.
Excitotoxicity
The excitatory transmitter glutamate normally mediates most fast synaptic transmission throughout the CNS, but it also has a surprising ability to trigger central neuronal death upon prolonged exposure, a phenomenon called ‘excitotoxicity’ by Olney (1969). Excitotoxicity now appears to be involved in the pathogenesis of several CNS diseases including ischemic brain injury (Rothman & Olney, 1986; Choi, 1988). Under ischemic conditions, neurons deprived of oxygen and glucose r apidly lose ATP and become depolarized, leading to abnormally high levels of glutamate release (initially mediated by vesicular release from nerve terminals, and later by reverse transport from astrocytes).

Growing evidence indicates that the brain's heightened vulnerability to ischemia, in large part, reflects a propensity for its intrinsic cell–cell and intracellular signaling mechanisms, normally responsible for information processing, to turn lethal under ischemic conditions. The most extensively studied example of such a signaling mechanism is that mediated by the excitatory neurotransmitter glutamate. In health, glutamate mediates most fast excitatory neurotransmission, but, under ischemic conditions, glutamate floods out from both neurons and astrocytes, building up in the extracellular space and becoming a killer that facilitates excess calcium entry into neurons, contributing to their demise.
Two Princeton Conferences ago, that is in Memphis in 1996, I presented then emerging evidence from my laboratory supporting the idea that another neurotransmitter released from excitatory nerve terminals might become a killer in the ischemic brain: the metal zinc. Besides the “transmitter killer” parallel to glutamate, I noted that there was also a parallel between zinc and calcium, in that both were divalent cation metals mediating ischemic neuronal death via excess influx across the plasma membrane.
A substantial body of evidence suggests that zinc is a neurotransmitter/neuromodulator [reviewed by refs. 2–4], although this possibility has had, to date, a rather low profile within the scientific community. The central nervous system contains a pool of relatively free zinc, separate from the zinc tightly bound to metalloenzymes and transcription factors in all cells.
