Diagnostic stewardship for Clostridioides difficile testing in an acute care hospital: A quality improvement intervention

Objective:
To evaluate the impact of a diagnostic stewardship intervention on Clostridioides difficile healthcare-associated infections (HAI).
Design:Quality improvement study.
Setting:Two urban acute care hospitals.
Interventions:All inpatient stool testing for C. difficile required review and approval prior to specimen processing in the laboratory. An infection preventionist reviewed all orders daily through chart review and conversations with nursing; orders meeting clinical criteria for testing were approved, orders not meeting clinical criteria were discussed with the ordering provider. The proportion of completed tests meeting clinical criteria for testing and the primary outcome of C. difficile HAI were compared before and after the intervention.
Results:The frequency of completed C. difficile orders not meeting criteria was lower [146 (7.5%) of 1,958] in the intervention period (January 10, 2022–October 14, 2022) than in the sampled 3-month preintervention period [26 (21.0%) of 124; P < .001]. C. difficile HAI rates were 8.80 per 10,000 patient days prior to the intervention (March 1, 2021–January 9, 2022) and 7.69 per 10,000 patient days during the intervention period (incidence rate ratio, 0.87; 95% confidence interval, 0.73–1.05; P = .13).
Conclusions:A stringent order-approval process reduced clinically nonindicated testing for C. difficile but did not significantly decrease HAIs.

Six new species of fenestrate bryozoans, middle to late Wordian, occur in the Gerster Limestone of northeastern Nevada. These bryozoans are similar to lower Kazanian species described from the Russian Platform. The two species of Wjatkella and Polypora from the Gerster Limestone are much more similar to the Russian species than they are to the two species recently described from the allochthonous late Wordian Mission Argillite of northeastern Washington.
New species described from the Gerster Limestone are Rectifenestella cordiretiformis, Wjatkella hemiseptifera, Kingopora wardlawi, Kingopora inflata, Polypora keyserlingiformis, and Polyporella helgersoni. The presence of only six species of fenestrate bryozoans in nearly 260 m of limestone and siltstone is notable.

Fifteen species of Late Permian bryozoans occur in a biohermal bank in the Mission Argillite of northeastern Washington. These include two species conspecific with species described from Japan and 13 new species, one of which is the type species of a new genus. The presence of two species, Dyscritella iwaizakiensis Sakagami, 1961, and Hayasakapora cf. erectoradiata Sakagami, 1960, previously reported from Japan, and the similarity of new species with those previously described from Japan, China and Russia supports the idea that these rocks were originally deposited in the southeastern or central western Pacific Ocean and subsequently accreted to the North American Plate.
Bryozoans and previously reported fusulinids indicate that the biohermal bank is latest Wordian (Kazanian).
Newly described bryozoans include the new genus and type species Sakagamiina easternensis belonging to the Timanodictyidae. Other new species are Fistuliramus pacificus, Meekoporella inflecta, Neoeridotrypella missionensis, Coeloclemis urhausenii, Tabulipora colvillensis, Rhombotrypella kettlensis, Pamirella oculus, Pinegopora petita, Wjatkella nanea, Alternifenestella vagrantia, Polypora arbusca, and Mackinneyella stylettia.

Recently developed analytical procedures for meshwork fenestrate and pinnate bryozoans made it possible to recognize two Russian species in the Toroweap Formation of southern Nevada. This analysis separates zoarial and zooecial features and emphasizes the three-dimensional form and size of the zooecial chamber as well as accessory features. In this investigation, these analytical procedures are successful even when specimens are fragmented and embedded in a hard rock matrix.
The bryozoans in the Toroweap Formation in this study were found to be more similar to Permian species of the Russian platform than to species described in earlier North American Permian studies. Five species of fenestrate bryozoans are present in the Toroweap Formation. Wjatkella permiana Morozova, 1970, and Polypora sargaensis Trizna, 1948, originally described in Russia, are recognized in the Toroweap. Three new species, Penniretepora oppositus n. sp., Septopora bilateralis n. sp., and Reteporidra anaphora n. sp., are present and, along with the Russian species, are described in this paper.
The local range zones of the five species of fenestrates were identified in three measured sections. Penniretepora oppositus and Septopora bilateralis are present throughout the three sections; Reteporidra anaphora is present in the lower two-thirds of the three sections; Wjatkella permiana is present in the middle parts of the three sections; and Polypora sargaensis is present in the upper parts of the three sections. Concurrent range zones, based on ramose bryozoans, were previously identified in two of the three sections and, using the distribution of the fenestrates in these two sections, four of the five concurrent range zones were recognized in the third section.
