The importance of accounting for testing and positivity in surveillance by time and place: an illustration from HIV surveillance in Japan

The number of tests performed is an important surveillance indicator. We illustrate this point using HIV surveillance data, focusing on Tokyo and Okinawa, two prefectures with high HIV notification rates in Japan. Restricting to data reported from local public health centres and affiliate centres where testing data are accessible, we assessed HIV surveillance data during 2007–2014, based on the annual HIV notification rate (per 100 000 population), HIV testing rate (per 100 000 population) and proportion testing HIV-positive (positivity). Nationally, testing activity and positivity showed an inverse relationship; in 2008, the testing rate peaked, but positivity was lowest. While notification rates were higher for Tokyo (median = 0.98, range = 0.89–1.33) than Okinawa (median = 0.61, range = 0.42–1.09), Okinawa had slightly higher testing rates (median = 187, range = 158–274) relative to Tokyo (median = 172, range = 163–210). Positivity was substantially lower in Okinawa (median = 0.34%, range = 0.24–0.45%) compared with Tokyo (median = 0.57%, range = 0.46–0.67%). Relative to the national testing rate (median = 85, range = 80–115) and positivity (median = 0.34%, range = 0.28–0.36%), Tokyo had higher positivity, despite more testing. In 2014 in Okinawa, all three indicators increased, providing a strong reason to be concerned as positivity increased despite more testing. Together with other information, accounting for testing and positivity improve interpretation of surveillance data to guide public health assessments.

Background
Projected changes to post-traumatic stress disorder (PTSD) diagnostic criteria in the upcoming International Classification of Diseases (ICD)-11 may affect the prevalence and severity of identified cases. This study examined differences in rates, severity, and overlap of diagnoses using ICD-10 and ICD-11 PTSD diagnostic criteria during consecutive assessments of recent survivors of traumatic events.
MethodsThe study sample comprised 3863 survivors of traumatic events, evaluated in 11 longitudinal studies of PTSD. ICD-10 and ICD-11 diagnostic rules were applied to the Clinician-Administered PTSD Scale (CAPS) to derive ICD-10 and ICD-11 diagnoses at different time intervals between trauma occurrence and 15 months.
ResultsThe ICD-11 criteria identified fewer cases than the ICD-10 across assessment intervals (range −47.09% to −57.14%). Over 97% of ICD-11 PTSD cases met concurrent ICD-10 PTSD criteria. PTSD symptom severity of individuals identified by the ICD-11 criteria (CAPS total scores) was 31.38–36.49% higher than those identified by ICD-10 criteria alone. The latter, however, had CAPS scores indicative of moderate PTSD. ICD-11 was associated with similar or higher rates of comorbid mood and anxiety disorders. Individuals identified by either ICD-10 or ICD-11 shortly after traumatic events had similar longitudinal course.
ConclusionsThis study indicates that significantly fewer individuals would be diagnosed with PTSD using the proposed ICD-11 criteria. Though ICD-11 criteria identify more severe cases, those meeting ICD-10 but not ICD-11 criteria remain in the moderate range of PTSD symptoms. Use of ICD-11 criteria will have critical implications for case identification in clinical practice, national reporting, and research.

Visible and infrared satellite images reveal numerous lineations on the Siple Coast region of West Antarctica. We used 5 MHz ice-penetrating radar to probe the interior and the bed of the ice sheet beneath a lineation at the boundary between Engelhardt Ice Ridge and flat-ice terrain to the south of the Kamb Ice Stream (KIS) outlet. Results show curved reflectors that emerge from the bed beneath 600 m thick ice. The tops of the reflectors extend about 100m into the ice above the bed, where they become almost horizontal. Apparent reflectivity of the horizontal section is about 20 dB less than that of the bed. We conclude that the likely cause of such strong reflection is sea water that was accreted into basal crevasses when the flat-ice terrain was floating. Internal layers are warped downward just downslope from the basal reflectors. It is thought that the downwarping was caused by localized basal melting in the past. The spatial pattern of downwarping suggests that localized basal melting was stronger on the north side than on the south side of KIS; apparently ice/ocean interactions on the two sides of KIS were different.

Dopamine stabilizes milrinone-induced changes in heart rate and arterial pressure during anaesthesia with isoflurane

Summary
Background and objective: Phosphodiesterase-III inhibitors and dobutamine effectively improve cardiac function in patients with cardiac failure, but they are limited by possible hypotensive effects. We tested the hypothesis that dopamine contributes to stabilizing milrinone-induced haemodynamic changes.
Methods: Nine patients undergoing major surgery were anaesthetized using nitrous oxide and oxygen supplemented with isoflurane 1–2%. After baseline haemodynamics were recorded, milrinone (25 or 50 μg kg−1) was administered over 10 min, followed by a continuous infusion (0.5 μg kg−1 min−1). The second set of haemodynamic values was measured 50 min after beginning the continuous infusion of milrinone. Dopamine (4 μg kg−1 min−1) was then administered with milrinone.
Results: Milrinone significantly increased the heart rate from 81 ± 8 to 102 ± 16 beats min−1, but it decreased the mean arterial pressure from 83 ± 10 to 66 ± 10 mmHg and systemic vascular resistance (P < 0.05 for each). The pulmonary capillary wedge pressure, cardiac index and pulmonary vascular resistance did not change significantly. The addition of dopamine to the milrinone infusion significantly decreased the heart rate (94 ± 12 beats min−1) and increased the mean arterial pressure (82 ± 11 mmHg). Dopamine and milrinone, but not milrinone alone, significantly increased the cardiac index and the rate–pressure product.
Conclusions: The combination regimen of milrinone and dopamine improved cardiac function, and changes in heart rate and mean arterial pressure induced by milrinone were attenuated by dopamine. The results suggest that a combination regimen of milrinone and dopamine rather than milrinone alone should be used to maintain arterial pressure.

The High Energy Transient Explorer 2 is a small scientific satellite designed to detect and localize gamma-ray bursts (GRBs). The coordinates of GRBs detected by HETE-2 will be distributed to interested ground-based observers within seconds of burst detection, thereby allowing detailed observations of the initial phases of GRBs. HETE-2 was launched successfully on October 9, 2000. The GRB positions will start to be delivered after a few months of the complete testing and calibration of the spacecraft system and the science instruments.

Correlation between defects and electrical conduction in surface conductive layers of CVD diamond films has been studied using electron spin resonance ( ESR ) and two points probe technique methods. The ESR analysis revealed the presence of Pac-center with spin density of 1020 spins/cm3. The Pac-center is composed from two ESR signals: ESR signal from carbon dangling bond with carbon atom neighbors and ESR signal from carbon dangling bond associated with nearest neighbor hole ( hole associated Pac-center ). The hole associated Pac-center is an electrically active defect.
Modelling the Soft X-ray Excess in E1615+061

We report results of an international UV – X-ray campaign in 1990–1992 involving the IUE, Rosat and Ginga satellites to observe E1615+061, a Seyfert 1 galaxy with peculiar spectral and intensity behaviour over the last 20 years. The source has been found to be stable in its medium state during the observations. The Ginga (1–20 keV) spectrum of E1615+061 is adequately represented by a simple power law with a photon index α = 1.8 ± 0.1. However, α ∼ 2, as expected for the intrinsic power law component in a reflection model, cannot be ruled out statistically. The Rosat PSPC (0.1–2 keV) spectra collected during the All Sky Survey and the AO-1 phase can be well-described by a simple power law (α = 2.2 ± 0.1) with cold absorber (NH = 3.5 ± 0.3 · 10λ20 H/cmλ2). Both the photon index being significantly different than that obtained from the Ginga spectrum and the column density being smaller than the galactic column (NH ∼ 4.2 · 10λ20 H/cmλ2) give an indication of a soft excess over and above the hard component seen in the Ginga spectrum. E1615+061 has been observed with IUE in 1990 and in 1992. The source was stable and the colour excess E(B-V) derived from the data = 0.1 is in good agreement with that expected from the galactic absorption.
To parameterise the soft excess we fitted the Rosat data with a two-component model consisting of a power law, and a blackbody or thermal bremsstrahlung, with a single galactic absorption term. The column density and the slope of the power law were kept constant. The blackbody temperature was 80 ± 6 eV and 63 ± 12 eV for photon index equal to 1.8 and 2.0, respectively, whereas the bremsstrahlung temperature was 220 ± 40 eV and 115 ± 30 eV for the two cases.
An attempt to model the soft excess seen in the Rosat PSPC spectrum has been made assuming that the soft excess is the high energy tail of a disc spectrum which peaks in the UV part of the spectrum. Additionally it was assumed that there is a hard component contributing to the spectrum from UV to X-rays with parameters as described by the Ginga spectrum. The best fit parameters: the mass of the central source and the mass accretion rate were around 5 ± 1 · 10λ6 M⊙ and 0.2 ± 0.04 M⊙/yr, respectively.
Our modelling shows that the soft X-ray excess can be described (χredλ2 < 1.2) as the high energy tail of an accretion disk spectrum if the intrinsic power law is quite steep (α = 2). The main contribution to the residuals in the Rosat PSPC range comes from 0.3–0.6 keV, with a tendency for these residuals to increase when the slope gets flatter. The accretion luminosity is ∼ 6.5 · 10λ44 erg/s for the best fit parameters, i.e. about the Eddington luminosity.
A rocket observation of cosmic X-rays in the energy range between 0.15 and 20 keV

Cosmic X-rays were observed with three sets of proportional counters covering the energy range between 0.15 and 20 keV. The detector born on a spinning rocket scanned a celestial region in which the galactic latitude bII changed from 30° to −55° across the galactic plane in the Cygnus-Cassiopeia region. The spectrum of Cyg XR-2 thus obtained is represented by a thermal bremsstrahlung of temperature 3.4 keV modified by the interstellar absorption for the hydrogen column density of 3 × 1021 cm−2. The diffuse component showed an interstellar absorption effect, which was however found much weaker than one would expect if the diffuse component were due entirely to be of extragalactic origin. The spectrum obtained in the highest latitude region is represented approximately by a power law E−1.8 but shows a possible trough at about 1 keV.
