Finding Opportunities for Health-Care System Advancement in the COVID-19 Crisis

The coronavirus disease 2019 (COVID-19) pandemic forced American medical systems to adapt to high patient loads of respiratory disease. Its disruption of normal routines also brought opportunities for broader reform. The purpose of this article is to describe how the Carl R. Darnall Army Medical Center (CRDAMC), a medium-sized Army hospital, capitalized on opportunities to advance its strategic aims during the pandemic. Specifically, the hospital sequentially adopted virtual video visits, surged on preventative screenings, and made-over its image to appeal to patients seeking urgent care. These campaigns supported COVID-19 efforts and larger strategic goals simultaneously, and they will endure for years to come. Predictably, CRDAMC encountered obstacles in the course of its transformation. These obstacles and their follow-on lessons are provided to assist future medical leaders seeking quantum change in the opportunities made available by health crises.
Sweetpotato tolerance to saflufenacil and rimsulfuron/thifensulfuron-methyl applied pretransplanting

Field studies were conducted at the Pontotoc Ridge–Flatwoods Branch Experiment Station in Pontotoc, MS, in 2016 and 2017 to determine sweetpotato crop response to saflufenacil and rimsulfuron/thifensulfuron-methyl. Saflufenacil treatments consisted of a factorial of two rates (25 or 50 g ai ha–1) by three application timings [0, 3, or 6 wk before transplanting (WBP)]. Rimsulfuron/thifensulfuron-methyl treatments consisted of a factorial of two rates (18/18 or 35/35 g ai ha–1) by two application timings (3 or 6 WBP). A nontreated check was included for comparison. Saflufenacil resulted in as much as 20% stunting injury in 2016, but ≤4% crop injury in 2017. Compared to the nontreated check, saflufenacil did not reduce yield of any sweetpotato grade regardless of application rate or timing. Findings from this trial indicate that saflufenacil applied in pre-transplanting burndown and field preparation procedures did not have a negative impact on the subsequent sweetpotato crop and that the current plant-back interval (4 to 5 mo) may be excessive. Applications of rimsulfuron/thifensulfuron-methyl at 35/35 g ha–1 made 3 WBP resulted in significant crop injury but did not reduce yield of any sweetpotato grade. Findings from this trial suggest that rimsulfuron/thifensulfuron-methyl applications up to 35/35 g ha–1 applied at least 6 WBP and 18/18 g ha–1 applied at least 3 WBP had little impact on sweetpotato crop growth and may be a safe preplant burndown option.

Electronic Documentation of Central Venous Catheter—Days: Validation Is Essential

Background.
Measurement of central line-associated bloodstream infection (CLABSI) rates outside of intensive care units is challenged by the difficulty in reliably determining central venous catheter (CVC) use. The National Healthcare Safety Network (NHSN) allows for use of electronic data for determination of CVC-days, but validation of electronic data has not been studied systematically.
Objective.To design and validate a process to reliably measure CVC-days outside of the intensive care units that leverages electronic documentation.
Methods.Thirty-four inpatient wards at 2 academic hospitals using a common electronic platform for nursing documentation were studied. Electronic queries were created to capture patient and CVC information, and tools and processes for tracking and reporting errors in documentation were developed. Strategies to validate electronic data included comparisons with manual CVC-day determinations and automated data validation using customized tools. Interventions included redesign of documentation interface, real-time audit with feedback of errors, and education. The primary outcome was patient-level total error rate in electronic CVC-day measurement compared with manually counted CVC-days.
Results.At baseline, there were a mean (± standard deviation) of 0.32 ± 0.25 electronic CVC-day errors (omission and commission errors summed and counted equally) per manually counted CVC-day. After several process improvement cycles over 7 months, the error rate decreased to <0.05 errors per CVC-day and remained at or below this level for 2 years.
Conclusions.Baseline electronic CVC-day counts had a high error rate. Stepwise interventions reduced errors to consistently low levels. Validation of electronic calculation of CVC-days is essential to ensure accuracy, particularly if these data will be used for interinstitutional comparison.
