Primary care chaplaincy: an intervention for complex presentation

Aim
To determine the responsiveness of primary care chaplaincy (PCC) to the current variety of presenting symptoms seen in primary care. This was done with a focus on complex and undifferentiated illness.
BackgroundCurrent presentations to primary care are often complex, undifferentiated and display risk factors for social isolation and loneliness. These are frequently associated with loss of well-being and spiritual issues. PCC provides holistic care for such patients but its efficacy is unknown in presentations representative of such issues. There is therefore a need to assess the characteristics of those attending PCC. The effectiveness of PCC relative to the type and number of presenting symptoms should also be analysed whilst evaluating impact on GP workload.
MethodsThis was a retrospective observational study based on routinely collected data. In total, 164 patients attended PCC; 75 were co-prescribed antidepressants (AD) and 89 were not (No-AD). Pre- and post-PCC well-being was assessed by the Warwick–Edinburgh mental well-being score. Presenting issue(s) data were collected on a separate questionnaire. GP appointment utilisation was measured for three months pre- and post-PCC.
FindingsThose displaying undifferentiated illness and risk factors for social isolation and loneliness accessed PCC. PCC (No-AD) was associated with a clinically meaningful and statistically significant improvement in well-being in all presenting issues. This effect was maintained in those with multiple presenting issues. PCC was associated with a reduction in GP appointment utilisation in those not co-prescribed AD.

We give bounds on the distance from a non-zero idempotent to the set of nilpotents in the set of $n\,\times \,n$ matrices in terms of the norm of the idempotent. We construct explicit idempotents and nilpotents which achieve these distances, and determine exact distances in some special cases.

Whistle vocalizations of five odontocete cetaceans, the false killer whale P. crassidens, short-finned pilot whale G. macrorhynchus, long-finned pilot whale G. melas, white-beaked dolphin L. albirostris and Risso's dolphin G. griseus, were analysed and summarized quantitatively. Recordings were acquired from a number of locations and encounters. Significant differences were found between species and, to a lesser extent, between locations. The calls of the two pilot whale species are distinct despite their close relatedness, and similar size and morphology. This may be due to selection pressures to maintain distinctiveness. The variance was partitioned into between-species, between-location (within species) and within-location factors. For the frequency variables, variation between-species is high relative to variation between locations. Thus geographic variation is a relatively minor effect, compared to the many processes which cause interspecific differences. The within-location component includes such factors as social context, behaviour and group composition. This component is of a similar magnitude to the between-species component, indicating that whistles vary considerably with these factors. Significant between-location differences may be attributable to these confounding factors. For whistle duration, most of the variation occurred within location. There is less significant variation in duration across species compared with the frequency measures. This study highlights the need to collect samples across all potential strata whenever possible, and provides a framework for future, more comprehensive work.

The distance from an arbitrary rank-one projection to the set of nilpotent operators, in the space of k × k matrices with the usual operator norm, is shown to be sec(π/(k:+2))/2. This gives improved bounds for the distance between the set of all non-zero projections and the set of nilpotents in the space of k × k matrices. Another result of note is that the shortest distance between the set of non-zero projections and the set of nilpotents in the space of k × k matrices is .
