Neuropsychological Profiles of Patients with Progressive Apraxia of Speech and Aphasia

Objective:
To characterize and compare the neuropsychological profiles of patients with primary progressive apraxia of speech (PPAOS) and apraxia of speech with progressive agrammatic aphasia (AOS-PAA).
Method:Thirty-nine patients with PPAOS and 49 patients with AOS-PAA underwent formal neurological, speech, language, and neuropsychological evaluations. Cognitive domains assessed included immediate and delayed episodic memory (Wechsler Memory Scale-Third edition; Logical Memory; Visual Reproduction; Rey Auditory Verbal Learning Test), processing speed (Trail Making Test A), executive functioning (Trail Making Test B; Delis-Kaplan Executive Functioning Scale – Sorting), and visuospatial ability (Rey-Osterrieth Complex Figure copy).
Results:The PPAOS patients were cognitively average or higher in the domains of immediate and delayed episodic memory, processing speed, executive functioning, and visuospatial ability. Patients with AOS-PAA performed more poorly on tests of immediate and delayed episodic memory and executive functioning compared to those with PPAOS. For every 1 unit increase in aphasia severity (e.g. mild to moderate), performance declined by 1/3 to 1/2 a standard deviation depending on cognitive domain. The degree of decline was stronger within the more verbally mediated domains, but was also notable in less verbally mediated domains.
Conclusion:The study provides neuropsychological evidence further supporting the distinction of PPAOS from primary progressive aphasia and should be used to inform future diagnostic criteria. More immediately, it informs prognostication and treatment planning.

Summary
The culture of Iraq has always been of the utmost importance to its people and to the international community. Following the 2003 invasion of Iraq (and notwithstanding the priority requirements of life support, medical facilities and infrastructure repair that the Iraqi people required) it was important that cultural issues – issues that underpinned the historical identity of Iraq – were addressed, and to that end it was deemed appropriate that a cultural initiative be originated and developed with a long-term view to the future.
Before taking command of British troops in Multi-National Division (South East) (MND(SE)) in February 2008, Major General Barney White-Spunner, General Officer Commanding 3rd (United Kingdom) Division, met with Dr Neil MacGregor, Director of the British Museum, to discuss the feasibility of the British Army and the British Museum working jointly to assist the Iraqi Ministry of State for Antiquities and Tourism in developing a cultural heritage project in southern Iraq. From these discussions Operation Heritage was born. From the outset, its aim was to identify key heritage sites and museums in southern Iraq, assess the state that they were in and, in coordination with the Iraqi authorities and the British Museum, implement a plan to safeguard these sites and museums.
In my role as Military Assistant to Major General White-Spunner in Basra I was identified, jointly with Dr John Curtis, Keeper of the Middle Eastern Department of the British Museum, as the project manager for this initiative, and in meetings held at the British Museum and Bulford, home to the Headquarters of the 3rd (United Kingdom) Division, we drew up a plan for the project with the following nine initial objectives:
• to seek approval from the Government of Iraq for the project
• to establish key liaison routes between the British Museum, the British Army and the Iraqi Ministry of State for Antiquities and Tourism
• to secure funding for the different aspects of the project
• to nominate the important cultural heritage sites in southern Iraq and carry out assessments with a team of experts
• to establish training courses for museum curators at the British Museum
• to assess security at key heritage sites
• to assist with and help to coordinate infrastructure repair as required
• to liaise with Government of Iraq Ministries and other Coalition partners to ensure that there is a coordinated approach on heritage issues pan-Iraq

An assessment of archaeological sites in June 2008: An Iraqi-British project

The proposal to develop an Iraqi-British project to protect and promote cultural heritage in Southern Iraq was first mooted at a lunch in the British Museum on 24 September 2007, involving Major-General Barney White-Spunner, Charles Moore, former editor of the Daily Telegraph, Neil MacGregor, Director of the British Museum, and John Curtis, Keeper of the Department of the Middle East at the British Museum. The lunch had been arranged to provide Major-General White-Spunner with recent information about the state of the Iraqi cultural heritage, as he was due to be deployed to Iraq in February 2008 as Commander-in-Chief of British troops and General Officer Commanding the Multi-National Division South-East. At the lunch, it was suggested that the greatest need would be to arrange for the inspection of archaeological sites and, if necessary, to arrange for the protection of them, and also to consider facilitating the reopening of some provincial museums. It is known that archaeological sites particularly in Southern Iraq suffered grievously from looting, particularly after the Second Gulf War, and most provincial museums were sacked following the First Gulf War in 1991 and again in 2003. Major-General White-Spunner immediately recognised the importance of these proposals and appointed a project manager, Major Hugo Clarke, to work up a scheme with John Curtis. The project has been made possible by a generous grant from the Department of Culture, Media and Sport, that has covered all costs except those incurred in Iraq, which have been met by the British Army.
