In vitro and in sacco digestibility of almond hulls

The aim of this study was to determine the in vitro digestibility and in sacco disappearance of dry matter (DM) and neutral detergent fibre (NDF) in total almond hulls (TAH), pure almond hulls (PAH) or Debris. The TAH were used because there are no data on the effect of debris (non-hull material) on the nutritional value of almond hulls. Twelve samples of commercial almond hulls were used, with one subsample representing the TAH and the other subsample hand sorted to separate the hulls (PAH) from Debris. Gas production and Ankom Daisy method were used to determine in vitro digestibility, while two rumen-fistulated cows were used to measure in sacco disappearance of PAH and TAH. For in vitro digestibility, both PAH and TAH were more digestible and had greater gas production than Debris. The PAH had greater in vitro true digestibility on a DM basis and NDF digestibility at 48 and 72 h compared with TAH. Nonpareil hulls provided greater metabolizable energy (ME) concentration when compared with other almond varieties, with PAH supplying numerically more ME than TAH for both varieties. For in sacco disappearance, PAH had greater DM and NDF disappearance along with a greater rate of disappearance for NDF compared with TAH. This research demonstrated that Debris is highly indigestible; therefore, hulling, agronomic and harvesting practices should be focused on reducing Debris in commercial almond hulls to improve their nutritive value as a feedstuff for livestock.
Hospital-acquired influenza in the United States, FluSurv-NET, 2011–2012 through 2018–2019

Objective:
To estimate population-based rates and to describe clinical characteristics of hospital-acquired (HA) influenza.
Design:Cross-sectional study.
Setting:US Influenza Hospitalization Surveillance Network (FluSurv-NET) during 2011–2012 through 2018–2019 seasons.
Methods:Patients were identified through provider-initiated or facility-based testing. HA influenza was defined as a positive influenza test date and respiratory symptom onset >3 days after admission. Patients with positive test date >3 days after admission but missing respiratory symptom onset date were classified as possible HA influenza.
Results:Among 94,158 influenza-associated hospitalizations, 353 (0.4%) had HA influenza. The overall adjusted rate of HA influenza was 0.4 per 100,000 persons. Among HA influenza cases, 50.7% were 65 years of age or older, and 52.0% of children and 95.7% of adults had underlying conditions; 44.9% overall had received influenza vaccine prior to hospitalization. Overall, 34.5% of HA cases received ICU care during hospitalization, 19.8% required mechanical ventilation, and 6.7% died. After including possible HA cases, prevalence among all influenza-associated hospitalizations increased to 1.3% and the adjusted rate increased to 1.5 per 100,000 persons.
Conclusions:Over 8 seasons, rates of HA influenza were low but were likely underestimated because testing was not systematic. A high proportion of patients with HA influenza were unvaccinated and had severe outcomes. Annual influenza vaccination and implementation of robust hospital infection control measures may help to prevent HA influenza and its impacts on patient outcomes and the healthcare system.

Introduction:
Psychotic depression (PD) is classified as a sybtype of severe depression in the current diagnostic manuals. Accordingly, it is a common conception that psychotic features in depression arise as a consequence of depressive severity.
Objectives:To determine whether the severity of depression and psychosis correlate in accordance with the “severity-psychosis” hypothesis and to detect potential differences in clinical features of psychotic and non-psychotic depression (non-PD).
Aims:We aimed to answer the following questions:
Does the clinical profile differ between patients with PD and non-PD?
Is the severity of depression and psychosis correlated in patients with depression?
Methods:Quantitative analysis of Health of the Nation Outcome Scales (HoNOS) scores from all patients admitted to a Danish general psychiatric hospital between 2000 and 2010 due to a severe depressive episode.
Results:A total of 357 patients with severe depression, of which 125 (35%) were of the psychotic subtype, formed the study sample. Mean HoNOS scores at admission differed significantly between patients with non-PD and PD on the items hallucinations and delusions (non-PD = 0.33 vs. PD = 1.37, p < 0.001), aggression (non-PD = 0.20 vs. PD = 0.36, p = 0.044) and on the total score (non-PD = 10.55 vs. PD = 11.87, p = 0.024). the HoNOS scores on the two items “depression” and “hallucinations and delusions” were very weakly correlated (Spearman coefficient = 0.12).
Conclusions:The results suggest that the severity of depression is unlikely to be the key determinant for the development of psychosis and supports the hypothesis that the psychotic- and non-psychotic subtypes of depression are in fact distinct clinical syndromes.

At present, there is significant interest in the morphology of the coronary arteries, not least due to the increasingly well-recognised association between anomalous origin of the arteries and sudden cardiac death. Much has also been learnt over the last decade regarding the embryology of the arteries. In this review, therefore, we provide a brief introduction into the recent findings regarding their development. In particular, we emphasise that new evidence, derived using the developing murine heart, points to the arterial stems growing out from the adjacent sinuses of the aortic root, rather than the arteries growing in, as is currently assumed. As we show, the concept of outgrowth provides an excellent explanation for several of the abnormal arrangements encountered in the clinical setting. Before summarising these abnormal features, we draw attention to the need to describe the heart in an attitudinally appropriate manner, following the basic rule of human anatomy, rather than describing the cardiac components with the heart in the “Valentine” orientation. We then show how the major abnormalities involving the coronary arteries in humans can be summarised in terms of abnormal origin from the pulmonary circulation, abnormal aortic origin, or fistulous communications between the coronary arteries and the cardiac cavities. In the case of abnormal aortic origin, we highlight those malformations known to be associated with sudden cardiac death.

Although malformations involving the ventricular outflow tracts are often described as conotruncal malformations, there is no consensus as to the lesions included in, or excluded from, this category, reflecting, in part, the current lack of precise definitions of the embryonic truncus and conus. Analysis of development of the outflow tract in terms of proximal, intermediate, and distal components greatly facilitates understanding of the morphology of the aortopulmonary window and aortoventricular tunnels. The aortopulmonary windows reflect failure to close the embryonic aortopulmonary foramen, the space between the distal end of the cushions that divide the lumen of the outflow tract itself and the dorsal wall of the aortic sac. The aortopulmonary tunnels are produced subsequent to abnormal development of the cushions themselves. The distal ends of these cushions excavate to produce the sinuses and leaflets of the arterial valves. The proximal parts of the cushions muscularise to form the subpulmonary infundibulum. The middle part of the cushion mass disappears to provide a tissue plane between the infundibulum and the aortic root. Abnormal formation of this area accounts for the various types of aortoventricular tunnel. In our brief review, we show how the anatomy of these lesions correlates with development of the outflow tract.

INTRODUCTION
The unique ability of the anaerobic sulphate-reducing bacteria (SRB) to respire sulphate provides access to niches that may be restricted from other bacteria. However, environmental niches are by definition constantly in flux. Thus, for scientists to reach a level of understanding that will allow prediction and/or control of the activities of the SRB, it is necessary to learn how the bacteria respond to changes in environmental parameters; such as, nutrient availability, presence of toxic substances, altered salt concentrations, temperature fluctuations, and a myriad of other variables. With the recent sequencing of a number of SRB (Klenk et al., 1997; Heidelberg et al., 2004; Rabus et al., 2004), the available proteins and regulatory sites of the bacteria have been revealed. Nevertheless, a significant percentage of the predicted open reading frames (ORFs) encode hypothetical or conserved hypothetical proteins for which functions remain obscure. Much work is yet to be done to elucidate the interplay of functions that allow the SRB to survive or even flourish in the changing conditions prevailing in their environment. Here we discuss preliminary transcriptional analyses of the responses of Desulfovibrio vulgaris Hildenborough to a number of environmental stressors.
A description of optimal growth conditions for D. vulgaris Hildenborough is derived from its early characterization. This strain is a mesophilic Gram-negative anaerobe which was isolated in 1946 from Wealden Clay near Hildenborough, Kent, in the United Kingdom (Postgate, 1984).

Nanocrystalline thin films of tin (IV) oxide (cassiterite) have been deposited from aqueous solutions of tin (IV) chloride and hydrochloric acid at 80°C. Substrates were {100} single-crystal silicon wafers, with and without silanol-anchored, sulfonate-terminated organic self-assembled monolayers (SAMs). Using flowing solutions, films with thicknesses of up to 1 μtm have been grown, whereas the thickness of the films from static solutions is limited to about 80 nm. The films were characterized using transmission electron microscopy and Rutherford backscattering spectroscopy. The role of the flow rate and configuration of the deposition chamber is discussed.

Abstract
In a prospective, randomised, controlled trial to determine whether comprehensive lifestyle changes affect coronary atherosclerosis after 1 year, 28 patients were assigned to an experimental group (low-fat vegetarian diet, stopping smoking, stress management training, and moderate exercise) and 20 to a usual-care control group. 195 coronary artery lesions were analysed by quantitative coronary angiography. The average percentage diameter stenosis regressed from 40-0 (SD 16-9)% to 37-8 (16-5)% in the experimental group yet progressed from 42-7 (15-5)% to 46-1 (18-5)% in the control group. When only lesions greater than 50% stenosed were analysed, the average percentage diameter stenosis regressed from 61-1 (8-8)% to 55-8 (11-0)% in the experimental group and progressed from 61-7 (9-5)% to 64-4 (16-3)% in the control group. Overall, 82% of experimental-group patients had an average change towards regression. Comprehensive lifestyle changes may be able to bring about regression of even severe coronary atherosclerosis after only 1 year, without use of lipid-lowering drugs. Lancet 1990; 336: 129-33.
Introduction
The Lifestyle Heart Trial is the first randomised, controlled clinical trial to determine whether patients outside hospital can be motivated to make and sustain comprehensive lifestyle changes and, if so, whether regression of coronary atherosclerosis can occur as a result of lifestyle changes alone. Over twenty clinical trials are being carried out to determine whether the progression of coronary atherosclerosis can be modified; in all of these, cholesterol-lowering drugs, plasmapheresis, or partial ileal bypass surgery are the primary interventions.
We carried out trials in 1977 and 1980 to assess the short-term effects of lifestyle changes on coronary heart disease with non-invasive endpoint measures (improvements in cardiac risk factors, functional status, myocardial perfusion, and left ventricular function). However, the subjects of those studies were not living in the community during the trial, and we did not use angiography to assess changes in coronary atherosclerosis.
Patients and methods
Patients with angiographically documented coronary artery disease were randomly assigned to an experimental group or to a usual-care control group. Experimental-group patients were prescribed a lifestyle programme that included a low-fat vegetarian diet, moderate aerobic exercise, stress management training, stopping smoking, and group support. Control-group patients were not asked to make lifestyle changes, although they were free to do so. Progression or regression of coronary artery lesions was assessed in both groups by quantitative coronary angiography at baseline and after about a year.
