Somatic multicomorbidity and disability in patients with psychiatric disorders in comparison to the general population: a quasi-epidemiological investigation in 54,826 subjects from 40 countries (COMET-G study)

Background
The prevalence of medical illnesses is high among patients with psychiatric disorders. The current study aimed to investigate multi-comorbidity in patients with psychiatric disorders in comparison to the general population. Secondary aims were to investigate factors associated with metabolic syndrome and treatment appropriateness of mental disorders.
MethodsThe sample included 54,826 subjects (64.73% females; 34.15% males; 1.11% nonbinary gender) from 40 countries (COMET-G study). The analysis was based on the registration of previous history that could serve as a fair approximation for the lifetime prevalence of various medical conditions.
ResultsAbout 24.5% reported a history of somatic and 26.14% of mental disorders. Mental disorders were by far the most prevalent group of medical conditions. Comorbidity of any somatic with any mental disorder was reported by 8.21%. One-third to almost two-thirds of somatic patients were also suffering from a mental disorder depending on the severity and multicomorbidity. Bipolar and psychotic patients and to a lesser extent depressives, manifested an earlier (15–20 years) manifestation of somatic multicomorbidity, severe disability, and probably earlier death. The overwhelming majority of patients with mental disorders were not receiving treatment or were being treated in a way that was not recommended. Antipsychotics and antidepressants were not related to the development of metabolic syndrome.
ConclusionsThe finding that one-third to almost two-thirds of somatic patients also suffered from a mental disorder strongly suggests that psychiatry is the field with the most trans-specialty and interdisciplinary value and application points to the importance of teaching psychiatry and mental health in medical schools and also to the need for more technocratically oriented training of psychiatric residents.
Model of qualification for physical therapy program: experiences from the Mental Health Support Centre in Tarnowskie Góry, Poland.

Introduction
In terms of physical and mental health benefits, psychiatric rehabilitation requires diverse therapeutic activities offered by an interdisciplinary team. In order for patients to be able to fully participate in social life, it is necessary to strengthen health-promoting activities, control the symptoms and side effects of pharmacological treatment, effectively manage resources and counteract interpersonal and environmental barriers caused by disability. Physical activity and exercise programs answer both psychosocial and biological aspects of health leading to higher self-efficacy, lower self-perceived stigma, longer lifespan and overall better quality of life
ObjectivesThe aim of this study is to present qualification process of adult patients for physical therapy program at the Mental Health Support Centre’s (Centrum Wsparcia Zdrowia Psychicznego, CWZP) daily rehabilitation unit in Tarnowskie Góry.
MethodsAdult patients with diverse diagnoses (schizophrenia, affective disorders, anxiety disorders or organic mental disorders) and varied degree of functioning who met the admission criteria were accepted to the ward for a period of 12 weeks. During the stay, a wide range of therapeutic activities was offered, including individual psychological support, group work, art therapy, relaxation sessions, culinary/ dietary workshops, individual training and general fitness group exercises, as well as cognitive training (also via computer-based programs). Due to the COVID-19 epidemic, some of the activities have been limited.
ResultsPhysical exercises, just as any treatment, should be offered in appropriate doses. Patients with mental disorders, especially severe mental illnesses, experience many barriers in engaging in physical activity and are under a greater risk of sedentary lifestyle. Thus, for the qualification, exercise tolerance assessment was performed in the form of 6-min Walk Test (6MWT) with Borg scale for subjective fatigue. Through aerobic capacity and endurance assessment patients’ respiratory system, cardiovascular system, and neuromuscular system functions can be evaluated. Functional fitness was assessed through 3 elements of Senior Fitness Test (SFT) (Back Scratch test, Up and go test, Chair Sit-and-Reach test) along with Romberg Test to evaluate the static and dynamic balance as well as flexibility of the upper and lower body.
ConclusionsProperly planned and carried out qualification for the physical therapy program allows to adjust the activities to the needs of the patient. Additionally, it can be a tool to evaluate the achieved results at the end of the rehabilitation process.

Sexual dysfunctions among men with paroxysmal or persistent atrial fibrillation - a two-center study in the Polish population

Introduction
There are no comprehensive studies on sexual dysfunctions among people with paroxysmal and persistent atrial fibrillation after excluding concomitant somatic and mental disorders, even though their presence has a significant impact on observed changes in sexual activity and behaviour. Most of the available studies among males are focused on erectile dysfunction due to well established relationship of it with the occurrence of cardiovascular disorders in the future.
ObjectivesThe aim of the study was to assess the prevalence of sexual disorders among male patients treated for paroxysmal and persistent atrial fibrillation.
MethodsThe study group included 54 men diagnosed with paroxysmal and persistent atrial fibrillation, qualified for electrical cardioversion and/or ablation of circumferential pulmonary vein, not burdened with additional somatic diseases. The control group consisted of 55 men matched in terms of sex, age and health condition. The study used standard CSFQ-14 sexuality assessment questionnaires, and the WHOQoL-BREF quality of life survey. Mental status examination was performed to exclude those with mental disorders, currently or in the past receiving psychiatric and/or addiction treatment.
ResultsThe analysis of the survey studies showed that among the surveyed men with paroxysmal and persistent atrial fibrillation, the prevalence of sexual problems is high (61.1% study group vs 47.3% control group). There were significant differences between the study group with lower sexual function scores in pleasure, desire/interest, arousal/erection, orgasm/ejaculation, and in the overall CSFQ-14’s score. The desire/frequency ratio did not differ between the groups.
ConclusionsComprehensive care requires the assessment of sexual satisfaction and the presence of possible dysfunctions using standardized tools.

In Poland, the problem of alcohol dependence according to various statistics cover from 5 to 7% of the population, hence the number of centers for such people is growing. The most common method of the treatment of addiction at the moment is integrative psychotherapy of addiction based on psycho-social model. It turns out, however, that the method of BSFT has many advantages that make it effective and less expensive than traditional methods used.
The aim of this study was presenatation of patients’ and therapists’ needs as well as identification of existing barriers.
64 therapists were examined in total; 37 women (57%) and 27 males (43%). The study involved also 191 patients, 160 men (83.77%) and 31 women (16.23%). All the surveys were anonymous and were collected in health centers within the province of Silesia.
As it turns out, type of education and skills learned in the process of therapists’ training is the main factor in the choice of the form of therapy proposed to patients. When it comes to barriers for alcohol addiction treatment, the therapists often pointed to cultural barriers, such as shame of admitting the addiction or fear of stigmatization. As reasons why the alcohol addiction treatment centers’ do not change their treatment programs surveyed indicated as the most important answer’patients do not care what methods are used in’ (16.75%).
This points to the need for more programs tailored to the needs of respondents, since the standard treatment regimens lead to more frequent treatment failure.

Among people suffering from mental conditions increased prevalence of diabetes, cardiovascular disease, hypertension and respiratory disease is observed, with considerably higher levels of morbidity and mortality. Still, mental conditions are neglected and not treated equally to other medical states in Poland. It is commonly believed that psychiatric patients are violent and unpredictable, even though they are more likely to be a victim of violence themselves. This attitude, reinforced by media coverage and observed also among health care workers, results in unwillingness to have any relations with mental patients due to fear of them being aggressive. Connection between physical well-being and mental health is well known. Physical therapists have necessary knowledge and skills to support development of individual independence, anxiety management and lifestyle control in order to keep patients healthier. It can be done by specially designed treatment programmes consisting of exercises, manual techniques and physical medicine procedures. Lack of understanding of this valuable connection results in insufficient emphasis on the presence of physiotherapist in the multidisciplinary therapeutic team on psychiatric wards nationwide. The study focuses on summary of the current situation in Poland and starts discussion on possible areas of improvements.

Poor knowledge regarding mental health in general population in Poland, along with media coverage limited to repetition of harmful stereotypes towards patients treated at psychiatric wards and reinforcement of discriminative attitudes, results in an unfair evaluation and stigmatization of mental healthcare services. As a consequence, psychiatry, in comparison to many other medical fields, is unpopular among physiotherapy students, even though there is a compulsory subject in the university curriculum that covers, in theory, all the important knowledge that healthcare worker should possess in this regards. Young physical therapists are not taught about specific needs of the psychiatric patients. After graduation, they are lacking all basic skills on how to communicate with the patient. Being devoid of a direct contact with people suffering from mental disorders, physical therapists do not feel comfortable placed in the mental healthcare facilities. The aim of the study was to assess the extent of a basic psychiatric knowledge and general attitudes towards mentally ill of the physiotherapy students. The group consisted of 147 students. The pilot study has been limited to those studying physical therapy within borders of the Opole voivodship. Authors’ questionnaire has been developed in order to reach the aim and answers were gathered between January and June 2016. The results will be used to develop questionnaire suitable to share with physiotherapy students within the whole country and, consequently, formulate recommendations on necessary changes that must be introduced to the physical therapy curriculum in Poland by Polish Society of Physiotherapy (Psychiatry Section).
