Real-Time Bedside Root Cause Analysis (RCA) as a Catalyst for Clostridioides difficile Reduction

Background:C. difficile infection has been a significant cause of morbidity and mortality over the past decade. Our hospital had rates of hospital-onset, laboratory-identified, C. difficile infection (HO-CDI) that were significantly higher than our state and national benchmarks. HO-CDI is defined as a test positive for C. difficile occurring on or after day 4 of hospitalization, regardless of the presence of symptoms. New leadership at the hospital sought a creative way to engage staff in finding solutions to our high rates of HO-CDI. Objective: The purpose of this intervention was to engage frontline staff in reporting and solving patient care situations that may increase infection risk to decrease HO-CDI rates. Methods: Starting in July 2015, real-time bedside RCAs were performed weekly for any HO-CDI on the unit to which the infection was attributed and on any unit from which the patient had been recently transferred. Top clinical leadership of the hospital, and all services and departments, physicians, nurses, and others involved with the patient’s care were expected to attend and identify factors that may have contributed to the infection. The findings were documented, and changes to care were made based on the findings. The rate of incident hospital onset HO-CDI per 10,000 patient days was used to measure outcome because standardized infection ratios for the period before 2016 were not available. Results: Staff members suggested 6 specific actions that were undertaken to decrease HO-CDI risk (Table 1). The HO-CDI rate during the preintervention period (2012–2014) was 6.85 per 10,000 patient days (275 cases). In the postintervention period (2016–2018) the HO-CDI rate was 3.13 per 10,000 patient days (101 cases). There was a 54% reduction in the HO-CDI rate in the postintervention period (P < .001). Conclusions: The multidisciplinary bedside RCA process resulted in staff providing recommendations for actions to reduce HO-CDI risk. Implementation of staff suggestions resulted in a sustained, significant decrease in HO-CDI.

Summary
It is always a touching thing, when we lose someone, to see how we gather together, close ranks and share our grief; so, this morning, we are here to support Judy and the family and each other.
A human being is a mystery; none of us knows another person completely but, when they leave us, we have the opportunity to celebrate the life they have lived and to remember. When we lose someone we also lose a part of ourselves; the link we form with another is unique; when it is broken, that particular relationship is irreplaceable.
Some of us are born under a lucky star, born with a singular purpose, special gifts, the means to express them and the opportunity to do so. Charles was one of those people. During our work in re-creative groups, performers develop a bond that is difficult to describe; it grows out of a common purpose, to serve the composer as best we can, and is, I believe, our first duty.
I have never known a musician who fulfilled that duty more faithfully than Charles did; the burning intention that shaped and drove him had one purpose: to put his gifts at the composer's service before anything else. He demanded the same dedication from his singers: he drove us very hard because he wanted us to match his vision, his search for perfection, and we responded to it. Our reward was the sense of total security we felt with him, coming up to us from the pit of the opera house, in a concert hall or recording studio; he was always impeccably prepared and the support he gave was like standing on solid rock.
That standard of preparation didn't mean rigidity or a lack of spontaneity; I remember during rehearsals for Julius Caesar how he would present us with bars of complicated ornamentation and cadenzas, which we duly got into our heads, to be met the next day by an excited conductor who had had more interesting thoughts overnight that he wanted us to memorise instead.
Visualising conversations between care home staff and residents with dementia

People with dementia living in residential care often face the dual disadvantage of increasing difficulty with communication and reduced opportunities for conversation. Social interaction is central to wellbeing of residents with dementia, so it is important that care staff have the skills to engage in conversation with them. We studied conversations in 20 care staff–resident dyads, to examine conversation structure and content, patterns of engagement within conversations, including the topics around which engagement occurred, and communication behaviours by care staff that appeared to facilitate (or impede) participation by residents. The transcripts were analysed using Discursis, a computational information visualisation tool that allows interactive visual inspection, in context, of the contributions by each speaker, the turn-taking dynamics, and the content recurring within and between speakers. We present case examples (a) where care staff did most of the talking, initiated topics and were responsible for most recurrence of content; (b) where talk was more evenly shared between partners, with some topics initiated and/or elaborated by participants with dementia; and (c) where participants with dementia talked most, with care staff supporting the conversation. We identified accommodative strategies used by care staff, such as reflecting back the other person's responses to sustain engagement. We also noted care staff behaviours that impeded communication, such as not listening attentively and not allowing sufficient time for responses. The results from this study highlight aspects of social communication within the aged care context and suggest ways in which rewarding interactions between staff and residents with dementia might be encouraged.

Plasma chlorpromazine measured by a gas chromatographic procedure was found to disappear rapidly during the first week after withdrawal of chlorpromazine treatment in a group of 17 chronically hospitalized patients. The drug or its metabolites were no longer measurable in plasma after 8 days.
