Since the initial publication of A Compendium of Strategies to Prevent Healthcare-Associated Infections in Acute Care Hospitals in 2008, the prevention of healthcare-associated infections (HAIs) has continued to be a national priority. Progress in healthcare epidemiology, infection prevention, antimicrobial stewardship, and implementation science research has led to improvements in our understanding of effective strategies for HAI prevention. Despite these advances, HAIs continue to affect ∼1 of every 31 hospitalized patients,1 leading to substantial morbidity, mortality, and excess healthcare expenditures,1 and persistent gaps remain between what is recommended and what is practiced.
The widespread impact of the coronavirus disease 2019 (COVID-19) pandemic on HAI outcomes2 in acute-care hospitals has further highlighted the essential role of infection prevention programs and the critical importance of prioritizing efforts that can be sustained even in the face of resource requirements from COVID-19 and future infectious diseases crises.3
The Compendium: 2022 Updates document provides acute-care hospitals with up-to-date, practical expert guidance to assist in prioritizing and implementing HAI prevention efforts. It is the product of a highly collaborative effort led by the Society for Healthcare Epidemiology of America (SHEA), the Infectious Disease Society of America (IDSA), the Association for Professionals in Infection Control and Epidemiology (APIC), the American Hospital Association (AHA), and The Joint Commission, with major contributions from representatives of organizations and societies with content expertise, including the Centers for Disease Control and Prevention (CDC), the Pediatric Infectious Disease Society (PIDS), the Society for Critical Care Medicine (SCCM), the Society for Hospital Medicine (SHM), the Surgical Infection Society (SIS), and others.

From public health to political campaigns, numerous attempts to encourage behavior begin with the spread of information. Of course, seeding new information does not guarantee action, especially when it is difficult for receivers to verify this information. We use a novel design that introduced valuable, actionable information in rural Uganda and reveals the intermediate process that led many in the village to hear the information but only some to act on it. We find that the seeded information spread easily through word of mouth via a simple contagion process. However, acting on the information spread less easily; this process relied instead on endogenously created social information that served to vet, verify, and pass judgment. Our results highlight an important wedge between information that a policy intervention can best control and the behavior that ultimately results.

Prevalence and Clinical Relevance of Staphylococcus warneri in the Neonatal Intensive Care Unit

Objective.
To describe the prevalence of Staphylococcus warneri on the hands of nurses and the clinical relevance of this organism among neonates in the neonatal intensive care unit (NICU).
Design.Prospective cohort study that examined the microbial flora on the hands of nurses and clinical isolates recovered from neonates during a 23-month period (March 1, 2001, through January 31, 2003).
Setting.Two high-risk NICUs in New York City.
Participants.All neonates hospitalized in the NICUs for more than 24 hours and all full-time nurses from the same NICUs who volunteered to participate.
Intervention.At baseline and then every 3 months, samples for culture were obtained from each nurse's cleaned dominant hand. Pulsed-field electrophoresis compared S. warneri isolates from neonates and staff.
Results.Samples for culture (n = 834) were obtained from the hands of 119 nurses; 520 (44%) of the 1,195 isolates of coagulase-negative staphylococci recovered were identified as S. warneri. Of the 647 clinically relevant isolates recovered from neonates, 17 (8%) of the 202 isolates that were identified to species level were S. warneri. Pulsed-field electrophoresis revealed a common strain of S. warneri that was shared among the nurses and neonates. Furthermore, 117 (23%) of 520 S. warneri isolates from nurses' hands had minimum inhibitory concentrations for vancomycin of 4 μg/mL, which indicate decreasing susceptibility.
Conclusions.Our findings that S. warneri can be pathogenic in neonates, is a predominant species of coagulase-negative staphylococci cultured from the hands of nurses, and has decreased vancomycin susceptibility underscore the importance of continued surveillance for vancomycin resistance and pathogenicity in pediatric care settings.
Potential Nosocomial Exposure to Mycobacterium tuberculosis From a Bronchoscope

Objective:
To investigate a possible nosocomial outbreak of tuberculosis (TB).
Design:Retrospective cohort study.
Setting:Community hospital.
Methods:We reviewed medical records, hospital infection control measures, and potential locations of nosocomial exposure. We examined the results of acid-fast bacilli (AFB) smears, cultures, and drug susceptibility testing, and performed a DNA fingerprint analysis. We observed laboratory specimen processing procedures and bronchoscope disinfection procedures. We also reviewed bronchoscopy records.
Results:In October 2000, three patients had bronchoscopy specimen cultures that were positive for Mycobacterium tuberculosis. Of the three, only one had clinical signs and symptoms consistent with TB and positive AFB sputum smears. The other two did not have signs and symptoms consistent with TB and had no known exposure to individuals with infectious TB. The three M. tuberculosis isolates had matching DNA fingerprints. No evidence of laboratory cross-contamination was identified. The three culture-positive specimens of M. tuberculosis were collected with the same bronchoscope within 9 days. This bronchoscope was inadequately cleaned and disinfected between patients, and the automated reprocessor used was not approved for use with the hospital bronchoscope.
Conclusions:One of the bronchoscopes at this hospital was contaminated with M. tuberculosis during bronchoscopy of an AFB-smear-positive patient. Subsequent specimen contamination likely occurred because the bronchoscope had been inadequately cleaned and disinfected. Patients who subsequently underwent bronchoscopy were also potentially exposed to M. tuberculosis from this bronchoscope.

IN 1864 FRANCES POWER COBBE jumped into the still-churning waters of Essays and Reviews (1860) and Colenso's Pentateuch (1862) with a book that put the case against an infallible Revelation even more frankly than these men had liked or dared. Broken Lights, her main contribution to the mid-Victorian Bible wars, went through at least eight editions and, next to her Duties of Women (1881), was the “most successful” of her books (Life 2: 370). Identifying “Frances Power Cobbe” on the title page as the “Author of an ‘Essay on Intuitive Morals,’ ‘The Pursuits of Women,’ etc.,“ the material object in a stranger's hands left no doubt that here was a work by a woman who had written on likely female subjects. Yet the table of contents, which lays out a schema of Churchmen's positions on the Bible questions of the hour, yields no clue that a feminist advocate wrote this book. Even in perusing its discussions of high theological matters, most readers would not have suspected, as I will argue, that the analyses of these men's positions are colored by Cobbe's distinctive “woman's perspective” (Caine 147), and that public and personal gender issues are being negotiated through its modes of discourse and argument.
